Compliance in patients with dietary hyperoxaluria: A cohort study and systematic review

Objective:Hyperoxaluria leads to calcium oxalate crystal formation and subsequent urolithiasis.This study aims to analyse the effect of treatment compliance in hyperoxaluria,firstly by analysis of patients with non-primary hyperoxaluria and secondly via systematic review in patients with any hyperoxaluria.Methods:In a retrospective cohort study,adults with non-primary hyperoxaluria managed with dietary counselling in 2013 were enrolled.Twenty-four-hour(24 h)urine collections initially and at 6 months were obtained.Compliance was assessed by self-reported dietary compliance and 24 h urinary volume>2 L.Patients were followed for 24 months.Primary outcomes were urinary oxalate and calcium 24 h load at 6 months,and urolithiasis-related procedural rates at 24 months.A Preferred Reporting Items for Systematic Reviews and Meta-Analyses(PRISMA)-compatible systematic review of compliance among hyperoxaluric patients was performed.Results:In the cohort study,of 19 eligible patients(4 female)with median age 52 years,10(53%)were considered compliant.Compared with the non-compliant group,these patients had significantly increased subsequent 24 h urinary volume(2250 mL vs.1600 mL;p=0.008)and lower procedural rates(10%vs.56%;p=0.033).Subsequent 24 h urinary oxalate load was nonsignificantly lower in compliant patients.Systematic review regarding compliance in hyperoxaluric patients revealed five studies.Only one utilised dietary counselling or analysed compliant vs.non-compliant patients,finding no difference.None examined the effect of compliance on procedural rates.

The impact of intra-operative cell salvage during open nephrectomy

Objective:To assess the impact of intra-operative cell salvage on outcomes in open nephrectomy.Methods:A retrospective cohort study was performed of all patients undergoing open nephrectomy for suspected malignancy from 1 October 2013 to 1 October 2017.Patients were grouped and compared based on whether they received intra-operative cell salvage(ICS).Primary outcomes were allogeneic transfusion rates(ATRs),and if histology confirmed cancer,disease recurrence.Secondary outcomes were complications and transfusion-related cost.Results:Forty patients underwent open nephrectomy for suspected malignancy during the enrolment period.Sixteen patients received ICS while 24 did not(standard group).Compared with the standard group,ICS patients had similar median age(63.5 vs.61.0 years;p=0.83)but fewer females(19%vs.58%;p=0.013).The groups were similar in pre-operative and discharge haemoglobin,Charlson Comorbidity Index,length of hospital stay and proportion with thoracoabdominal surgical approach.The ICS group had a smaller proportion undergoing partial nephrectomy(19%vs.54%;p=0.025)and shorter median follow-up(278 vs.827 days;p=0.0005).Histology was malignant for 14 ICS and 15 standard patients.The ICS group had more frequentT2 disease(79%vs.27%;p=0.005).There were no positive margins.both groups had similar ATRs(6%vs.4%;p=0.96),complication rates(19%vs.29%;p=0.46)and recurrence rates(18%vs.7%;p=0.40).Transfusion costs were higher amongst ICS patients(AUD$878.18 vs.$49.65 per patient).Conclusion:ICS appears safe,with low rates of recurrence and complication.both groups had low ATRs,and therefore cost benefit for ICS was not seen.

A systematic review of dedicated models of care for emergency urological patients

Objective:To systematically evaluate the spectrum of models providing dedicated resources for emergency urological patients(EUPs).Methods:A search of Cochrane,Embase,Medline and grey literature from January 1,2000 to March 26,2019 was performed using methods pre-published on PROSPERO.Reporting followed Preferred Reporting Items for Systematic Review and meta-analysis guidelines.Eligible studies were articles or abstracts published in English describing dedicated models of care for EUPs,which reported at least one secondary outcome.Studies were excluded if they examined pathways dedicated only to single presentations,such as torsion,or outpatient solutions,such as rapid access clinics.The primary outcome was the spectrum of models.Secondary outcomes were time-to-theatre,length of stay,complications and cost.Results:Seven studies were identified,totalling 487 patients.Six studies were conference abstracts,while one study was of full-text length but published in grey literature.Four distinct models were described.These included consultant urologists allocated solely to the care of EUPs(“Acute Urological Unit”)or dedicated registrars or operating theatres(“Hybrid structures”).In some services,EUPs bypassed emergency department assessment and were referred directly to urology(“Urological Assessment Unit”)or were managed by other dedicated means.Allocating services to EUPs was associated with reduced time-to-theatre,length of stay and hospital cost,and improved supervision of junior medical staff.Conclusion:Multiple dedicated models of care exist for EUPs.Low-level evidence suggests these may improve outcomes for patients,staff and hospitals.Higher quality studies are required to explore patient outcomes and minimum requirements to establish these models.

Evaluation of the performance of yielding rockbolts during rockbursts using numerical modeling method

Rockburst;Rockburst damage;Yielding rockbolt;Numerical modeling;UDEC;Underground miningThe assessment of yielding rockbolt performance during rockbursts with actual seismic loading is essential for rock-burst supporting designs.In this paper,two types of yielding rockbolts(D-bolt and Roofex)and the fully resin-grouted rebar bolt are modeled via the"rockbolt"element in universal distinct element code(UDEC)after an exact calibration procedure.A two-dimensional(2D)model of a deep tunnel is built to fully evaluate the performance(e.g.,capacity of energy-absorption and control of rock damage)of yielding and traditional rockbolts based on the simulated rockbursts.The influence of different rockburst magnitudes is also studied.The results suggest that the D-bolt can effectively control and mitigate rockburst damage during a weak rockburst because of its high strength and deformation capacity.The Roofex is too"soft"or"smooth"to limit the movement of ejected rocks and restrain the large deformation,although it has an excellent deformation capacity.The resin-grouted rebar bolt can maintain a high axial force level during rockbursts but is easy to break during dynamic shocks,which fails to control rapid rock bulking or ejection.Three types of rockbolts cannot control the large deformation and mitigate rockburst damage effectively during violent rockbursts.The rockburst damage severity can be significantly reduced by additional support with cable bolts.This study highlights the effectiveness of numerical modeling methods in assessing the complex performance of yielding rockbolts during rockbursts,which can provide some references to improve and optimize the design of rock supporting in burst-prone grounds.

Shear behavior of ultrafine magnetite tailings subjected to freeze-thaw cycles

To study the shear behavior of the ultrafine magnetite tailings subjected to freeze-thaw cycles,unconsolidated-undrained shear tests were conducted on ultrafine-grained tailings that were subjected to 1-11 cycles of freeze-thaw and defined as a type of clayey silt under confining pressures of 100,200,and 300 kPa.Taking the number of freeze-thaw cycles,cooling temperature,initial dry density,and moisture content as the four main influencing factors of shear behavior of the tailings samples,the shear stress-strain curve,compression modulus,failure strength,cohesion,and internal friction angle were measured.The results show that the freeze-thaw cycle has an obvious weakening effect on the shear behavior of the tailings material,and the shear mechanical parameters are affected by a combination of confining pressure,freeze-thaw cycle condition,and initial physical-mechanical properties of the tailings samples.Through the microstructural analysis of the tailings samples subjected to freeze-thaw cycles,it shows that the freeze-thaw cycle mainly affects the porosity,bound water,and arrangement of the tailings particles.Subsequently,the macroscopic changes in shear strength indexes emerge,and then the stability of the tailings dam will decrease.

Shear properties of cemented rockfills

Application of cemented rockfilling to underground mining could not be separated from the corresponding backfill’s shear strength properties. The shear of cemented rockfill(CRF)-rock wall and the shear interaction occurring within CRFs both have some disadvantageous failure chances. In this study,we tried to investigate the complete shear properties of CRFs using direct shear and triaxial tests of cemented granite rockfill. Large-scale triaxial testing was held to accommodate the large CRF sample.Direct shear testing on the prepared flat and smooth surfaces was assessed with brief conversions and their corrections were used to approximate the shear strength envelopes of CRF joint interfaces. Two types of CRFs with the same aggregate size and distribution but different unconfined compressive strengths(UCSs) due to different mixture designs indicated insignificant differences between their basic friction angles, and also their asperity inclination angles. Nevertheless, investigation between direct shear test and triaxial test showed that the specimen with higher UCS tended to have a slightly lower friction angle but a higher cohesion than the other one.

Performance evaluation of near real-time condition monitoring in haul trucks

Strategic maintenance plays a key role in ensuring high availability and utilization of the haul trucks,and as equipment began to grow more complex towards the end of the 20th century,there was a need for a proactive maintenance strategy,which led to the development of condition-based maintenance.Realtime condition monitoring(RTCM)is the ability to perform condition monitoring in real-time and has the ability to alert maintenance and operations of abnormal conditions.These alarms can be used as an indication leading to a problem,and if a suitable corrective action is initiated in time,it could result in significant savings of equipment downtime and repair costs.This study aims to compare some maintenance performance indicators prior to and after implementation of RTCM strategy at a mine site using some tests of statistical significance.The study also indicated the presence of seasonality in the data,and thus the data was deseasonalized and detrended prior to being subjected to the statistical tests.Finally,the results indicated that RTCM strategy has proven to be successful in improving the availability for some of the failure categories chosen in this study.

Use of machine learning algorithms to assess the state of rockburst hazard in underground coal mine openings

The risk of rockbursts is one of the main threats in hard coal mines. Compared to other underground mines, the number of factors contributing to the rockburst at underground coal mines is much greater.Factors such as the coal seam tendency to rockbursts, the thickness of the coal seam, and the stress level in the seam have to be considered, but also the entire coal seam-surrounding rock system has to be evaluated when trying to predict the rockbursts. However, in hard coal mines, there are stroke or stress-stroke rockbursts in which the fracture of a thick layer of sandstone plays an essential role in predicting rockbursts. The occurrence of rockbursts in coal mines is complex, and their prediction is even more difficult than in other mines. In recent years, the interest in machine learning algorithms for solving complex nonlinear problems has increased, which also applies to geosciences. This study attempts to use machine learning algorithms, i.e. neural network, decision tree, random forest, gradient boosting, and extreme gradient boosting(XGb), to assess the rockburst hazard of an active hard coal mine in the Upper Silesian Coal basin. The rock mass bursting tendency index WTGthat describes the tendency of the seam-surrounding rock system to rockbursts and the anomaly of the vertical stress component were applied for this purpose. Especially, the decision tree and neural network models were proved to be effective in correctly distinguishing rockbursts from tremors, after which the excavation was not damaged. On average, these models correctly classified about 80% of the rockbursts in the testing datasets.
