PURPOSE: The aim of this retrospective study was to evaluate technical efficacy and the impact of CT-guided pulmonary radiofrequency ablation(RFA) on survival in patients with pulmonary metastases from nasopharyngeal carcinoma(NPC). MATERIALS AND METHODS: Between 2000 and 2009,480 patients were pathologically or clinically confirmed pulmonary metastases from NPC. And ten included patients of them had a total of 23 pulmonary metastases treated with percutaneous RFA under the realtime CT ***,local tumor progression,and survival were evaluated in our ***-pair survival was compared using Kaplan-Meier analysis. RESULTS: A total of 25 ablations were performed to 23 pulmonary metastases in 13 RFA *** requiring chest tube placement developed in 3 of 13(23.1%) RFA *** median metastatic overall survival was 36.1 months for all the 480 NPC patients with pulmonary ***,matched-pair analysis demonstrated patients with RFA treatment had a greater metastatic overall survival than patients without RFA treatment(77.1 months vs 32.4 months,log-rank test,p=0.009).There were no statistically significant differences in the survival probability of patients with RFA treatment(n=10) and surgical resection of pulmonary metastases(n=27)(log-rank test,p=0.75). CONCLUSION: CT-guided pulmonary RFA is safe and offers a treatment alternative for local tumor control,providing promising survival in selected patients with pulmonary metastases from NPC.

Hepatocellular carcinoma(HCC),commonly known as primary liver cancer,is a major cause of malignant tumors and cancer-related deaths in china.accounting for approximately 85％of all cancer cases in the *** guidelines have been used to diagnose and treat liver ***,these guidelines provide a broad definition for classifying advanced liver cancer,with an emphasis on a singular approach,without considering treatment options for individual ***,it is necessary to establish a comprehensive and practical expert consensus,specifically for china.to enhance the diagnosis and treatment of HCC using the Delphi *** classification criteria were refined for Chinese patients with HCC,and the corresponding optimal treatment regimen recommendations were *** recommendations took into account various factors,including tumor characteristics,vascular tumor thrombus grade,distant metastasis,liver function status,portal hypertension,and the hepatitis B virus replication status of patients with primary HCC,along with treatment *** findings and rec-ommendations provide detailed,scientific,and reasonable individualized diagnosis and treatment strategies for clinicians.

Transarterial embolization ablation of hepatocellular carcinoma with a lipiodol-ethanol mixture

AIM: To determine the safety and effectiveness of transarterial embolization ablation (TEA) of hepatocellular carcinoma (HCC) with a lipiodol-ethanol mixture. METHODS: Between January 1 and December 31, 2009, 15 patients with HCC (13 men/two women, aged 38-75 years) accepted TEA treatment and were enrolled in this study, including five newly diagnosed patients and 10 with refractory disease. Two months after TEA, angiography and contrast computed tomography (CT) were performed, and responses were assessed using a modified version of Response Evaluation Criteria in Solid Tumors (RECIST version 1.1). The follow-up period was to June 30, 2010. RESULTS: Every new case was treated once. Angiography was performed immediately after TEA, and showed that the tumor-feeding vessels were completely embolized and that lipiodol was densely deposited inside tumors. Two months after treatment, contrast CT showed no enhanced lesions. Alpha fetoprotein levels returned to normal in four patients and markedly decreased in another. mean ± SD survival after treatment was 10.8 ± 4.5 mo. All five patients survived during the follow-up period. Ten patients with refractory disease were treated a total of 14 times. Angiography immediately after TEA showed that blood flow to the tumors was obviously decreased in all cases, and contrast CT showed obvious depositions of lipiodol. Two months after treatment, the tumors had shrunk (6/10) or were stable (3/10). One had progressed after 2 mo and died of tumor rupture 3 mo after TEA. mean ± SD survival after treatment was 8.6 ± 4.3 mo; two patients survived during the follow-up period. Adverse effects included reversible hepatic decompensation, upper abdominal pain, and fever. CONCLUSION: TEA is an effective therapy for patients with HCC and might be more effective than transcather arterial chemoembolization for treating refractory disease.

Either cetuximab or bevacizumab can improve the survival of patients with metastastic colorectal cancer (mCRC) if administered combided with cytotoxic agents. However, the effect of two or more target agents in combination is uncertain in these patients. Here, we reported a patient with mCRC successfully treated by a combination of target agents after the failure of chemotherapy. The patient received palliative resection of primary tumor followed by 9 cycles of postoperative XELOX regimen, cytokine-induced killer cell (CIK)-based biotherapy, traditional Chinese medicine, particle implantation in the lung metastatic lesions. The tumor progressed 20 months after the standard treatments. Then, the regimen cetuximab, bevacizumab and cefitinib was applied. During the treatment with targeted agents, grade IV acne-like rash and relatively severe parionychia of the toes occurred. Both of them recovered smoothly. The PET-CT reexamination at 40 days after the target treatment showed that the metabolism of mediastinal lymph nodes basically recovered to a normal level. The combination of multiple targeted agents obtained a progression-free survival(PFS) of 11 months and the patient with a good quality of life during this period.

Purpose To evaluate the effectiveness and safety of a PET/CT-guided,multidisciplinary,minimally invasive approach to the treatment of patients with large,unresectable,malignant fibrous *** approach includes microwave ablation and absolute alcohol therapy combined with 125I seed *** and Methods Seven patients(5 males and 2 females,42-48 years-old,mean 49.7 years-old) with large,unresectable malignant fibrous histiocytoma participated in this *** tumors had an average maximum diameter of 14.1 cm(10.0-19.0 cm) and were treated using a PET/CT-guided,multidisciplinary,minimally invasive *** patients received a PET/CT scan 2 months after the ***-up was conducted for an average of 35.7(2-45) months to determine the local control rate,overall survival rate,and clinical *** Follow-up times ranged from 2 to 45 *** was significantly relieved in patients treated with multidisciplinary,minimally invasive *** on PET/CT, complete response was achieved in 5 patients(71.4%),partial response in the other 2 patients(28.6%).The response rate of this multidisciplinary,minimally invasive treatment was 100%.The median survival time was 35.7 *** patients were alive 2 years after the *** patients were still alive after 3 *** patient died from cachexia and infection in the 3rd year,and another died of lung dysfunction from pulmonary metastases in the 2nd *** 3-year survival rate was 71.4%.The long-term complications included hyperpigmentation at the operative sites(n = 5) and insensible feeling at the ablation sites(n=3).Conclusion:PET/CT-guided multidisciplinary,minimally invasive approach is an effective,safe,and feasible means of treating large,unresectable malignant fibrous histiocytoma with minimal damage and few complications,but large-scale randomized clinical trials are necessary to confirm this assessment.

Objective To investigate the feasibility,short-term efficacy and adverse effects of 125I seeds implantation in the treatment of recurrent ovarian cancer and cervical *** A total of 12 patients with recurrent ovarian cancer and 17 patients with cervical cancer treated in sun yat-sen university cancer center from June 2009 to December 2010 enrolled into the *** there were 49 lesions including 25 ovarian cancer lesions and 24 cervical ***-five lesions were treated with 125 I seed *** ovarian cancer lesions in liver and one lesion in spleen were treated with microwave ***-four lesions were treated under the guidance of CT while only one lesion in one patient was treated under the guidance of real-time *** treating plan was calculated using computerized treatment planning system(TPS) and Memorial Sloan-Ketterin nomograph according to the physiologic 18FDG uptake image in the *** prescribed matched peripheral dose(MPD) was 100～160 Gy. The total number of sources implanted were 6～68 with a median number of *** chemotherapy was given to 13 patients after the interventional *** status were evaluated with CT and 18F-FDG PET/CT findings. Results One patient with recurrent ovarian cancer and 9 patients with recurrent cervical cancer died and the other patients were still alive after a median follow-up of 15 mouths(range from 5 to 19mouths).Thirteen patients showed complete remission,seven patients showed partial remission,nine patients had progressive *** effective rate was 69.0% (20/29).There were three patient suffered from Sciatic nerve injury *** complication was found in the other *** 125 I seed implantation was feasible,effective and safe as a salvage local treatment for patients with recurrent gynecologic malignancies,it was worth of development and *** efficacy seems better in recurrent ovarian cancer than in cervical cancer patients.

The purpose of this study is to evaluate the effectiveness and safety of PET-CT guided 125I seed implantation in the treatment of patients with refractory nasopharyngeal carcinoma after repeated traditional ***-six patients with refractory nasopharyngeal carcinoma after repeated traditional radiochemotherapy,18 males and 8 females,aged 51.3(42- 68),totaling 53 lesions 4.3(0.5-7.8) cm in diameter,were treated by PET-CT guided 125I seeds implantation according to treatment planning *** the patients received PET-CT scan 2 months after the ***-up was conducted for 30.7(2-43) months to observe the local control rate,overall survival rate,and clinical *** follow-up time ranged from 2 to 43 *** local control rates of refractory nasopharyngeal carcinoma after repeated traditional radiochemotherapy after 3,6,12,24 and 36 months were 90.6%(48/53),79.3%(42/53),71.7% (38/53),62.3%(33/53) and 56.6%(30/53),*** overaU 1-,2-,and 3-year survival rates were 87.2%,71.3%,and 56.5%respectively with a median survival time of *** all patients,19.2%(5/26) died of local recurrence,15.4%(4/26) died of *** patient died of hypertensive Cerebral Hemorrhage,another patient died from cachexia and *** long-term complications included hyperpigmentation at operative sites(n=5),insensible feeling on lateral cheek (n=2),dryness of oral cavity(n=1),and headache(n=l).PET-CT guided 125I seed implantation is effective,safe and feasible method in treating refractory nasopharyngeal carcinoma with minimal damage and few complications.
