Clinical course of ulcerative colitis patients who develop acute pancreatitis

To investigate the clinical course of ulcerative colitis (UC) patients who develop acute *** analyzed 3307 UC patients from the inflammatory bowel disease registry at Asan Medical Center from June 1989 to May 2015. The clinical course of UC patients who developed acute pancreatitis was compared with that of non-pancreatitis UC *** 51 patients who developed acute pancreatitis, 13 (0.40%) had autoimmune, 10 (0.30%) had aminosalicylate-induced, and 13 (1.73%) had thiopurine-induced pancreatitis. All 13 patients with autoimmune pancreatitis (AIP) had type 2 AIP. Two (15.4%) patients had pre-existing AIP, and three (23.1%) patients developed AIP and UC simultaneously. Compared to non-pancreatitis patients, AIP patients had UC diagnosed at a significantly younger age (median, 22.9 years vs 36.4 years; P = 0.001). AIP and aminosalicylate-induced pancreatitis patients had more extensive UC compared to non-pancreatitis patients. All patients with pancreatitis recovered uneventfully, and there were no recurrences. Biologics were used more frequently in aminosalicylate- and thiopurine-induced pancreatitis patients compared to non-pancreatitis patients [adjusted OR (95%CI), 5.16 (1.42-18.67) and 6.90 (1.83-25.98), respectively]. Biologic utilization rate was similar among AIP and non-pancreatitis patients [OR (95%CI), 0.84 (0.11-6.66)]. Colectomy rates for autoimmune, aminosalicylate-induced, and thiopurine-induced pancreatitis, and for non-pancreatitis patients were 15.4% (2/13), 20% (2/10), 15.4% (2/13), and 7.3% (239/3256), respectively; the rates were not significantly different after adjusting for baseline disease *** patients show a non-significant increase in colectomy, after adjusting for baseline disease extent.

Hydrochemistry for the Assessment of Groundwater Quality in Korea

Understanding of the aquifer hydraulic properties and hydrochemical characteristics of water is crucial for management plan and study skims in the target area, and flow motions and chemical species of groundwater are regarded as precious information on the geological history of the aquifers and the suitability of various usages. Cations and anions of groundwater are used to estimate the characteristics and origin of groundwater. In this study, we try to evaluate the quality of groundwater based on the comparison of the physiochemical characteristics and distribution of cations and anions in groundwater from rural areas. Therefore we focused on the evaluation of groundwater as some specific purposes such as agricultural and industrial use, general types of groundwater, lithological origin of chemical component in groundwater. In this point of view, major objectives of this study were grouped as following three categories: 1) quality assessment of groundwater as a special usage (agricultural, industrial);2) determination of groundwater types;3) tracing of ion sources of groundwater. The quality of agricultural water was evaluated using SAR, sodium (%), RSC, PI, SSP, MH, PS, and Kelly’s ratio, and was classified as SAR (Excellent (100%)), Sodium ((Excellent (34%), Good (55%), Permissible (9%), Doubtful (1.6%), Unsuitable (0.4%)), RSC (Good (95.7%), Medium (3.5%), Bad (0.8%)), PI((Excellent (40.6%), Good (59%), Unsuitable (0.4%)), SSP ((Excellent (26.3%), Good (59.8%), Fair (13.1%), Poor (0.8%)), MH ((Acceptable (94.4%), Non-Acceptable (5.6%)), Kelly’s Ratio ((Permissible (93%), Non-Permissible (7%)), PS ((Excellent to Good (98%), Good to Injurious (1.2%), and Injurious to Unsatisfactory (0.2%)). High-risk esophageal varices in patients treated with locoregional therapy for hepatocellular carcinoma:Assessment with liver computed tomography

AIM： To assess the diagnostic performance of follow- up liver computed tomography （CT） for the detection of high-risk esophageal varices in patients treated with Io- coregional therapy for hepatocellular carcinoma （HCC）. METHODS： We prospectively enrolled 100 patients with cirrhosis who underwent transcatheter arterial chemoembolization, radiofrequency ablation or both procedures for HCCs. All patients underwent upper endoscopy and subsequently liver CT. Three radiolo- gists independently evaluated the presence of high-riskesophageal varices with transverse images alone and with three orthogonal multiplanar reformation （MPR） images, respectively. With endoscopic grading as the reference standard, diagnostic performance was as- sessed by using receiver operating characteristic （ROC） curve analysis. REsuLTS： The diagnostic performances （areas under the ROC curve） of three observers with transverse im- ages alone were 0.947 ± 0.031, 0.969 ± 0.024, and 0.916 ＋ 0.038, respectively. The mean sensitivity, spec- ificity, positive predicative value （PPV）, and negative predicative value （NPV） with transverse images alone were 90.1%, 86.39%, 70.9%, and 95.9%, respectively. The diagnostic performances, mean sensitivity, specific- ity, PPV, and NPV with three orthogonal MPR images （0.965 ± 0.025, 0.959 ± 0.027, 0.938 ± 0.033, 91.4%, 89.5%, 76.3%, and 96.6%, respectively） were not su- perior to corresponding values with transverse images alone （P 〉 0.05）, except for the mean specificity （P = 0.039）. CONCLUSION： Our results showed excellent diagnos- tic performance, sensitivity and NPV to detect high-risk esophageal varices on follow-up liver CT after Iocore- gional therapy for HCC,

The emerging fields of silicon(Si) photonic micro–electromechanical systems(MEMS) and optomechanics enable a wide range of novel high-performance photonic devices with ultra-low power consumption, such as integrated optical MEMS phase shifters, tunable couplers, switches, and optomechanical resonators. In contrast to conventional SiO;-clad Si photonics, photonic MEMS and optomechanics have suspended and movable parts that need to be protected from environmental influence and contamination during operation. Wafer-level hermetic sealing can be a cost-efficient solution, but Si photonic MEMS that are hermetically sealed inside cavities with optical and electrical feedthroughs have not been demonstrated to date, to our knowledge. Here, we demonstrate wafer-level vacuum sealing of Si photonic MEMS inside cavities with ultra-thin caps featuring optical and electrical feedthroughs that connect the photonic MEMS on the inside to optical grating couplers and electrical bond pads on the outside. We used Si photonic MEMS devices built on foundry wafers from the iSiPP50G Si photonics platform of IMEC, Belgium. Vacuum confinement inside the sealed cavities was confirmed by an observed increase of the cutoff frequency of the electro-mechanical response of the encapsulated photonic MEMS phase shifters, due to reduction of air damping. The sealing caps are extremely thin, have a small footprint, and are compatible with subsequent flip-chip bonding onto interposers or printed circuit boards. Thus, our approach for sealing of integrated Si photonic MEMS clears a significant hurdle for their application in high-performance Si photonic circuits.

Parenting Styles and Adolescent Misbehavior

Adolescence is a crucial period for character development and is particularly susceptible to parental influence due to the brain's malleability:This paper examines whether parenting styles can have a significant impact on adolescent misbehaviors such as disobedience and *** results show that,in comparison with negleciful parenting-characterized by low levels of demandingness and responsiveness-authoritative,authoritarian,and permissive parenting styles were associated with a significant reduction in behavioral issues among *** beneficial impact of authoritative parenting,marked by high demandingness and responsiveness,was particularly *** contrast,authoritarian parenting(high demandingness but low responsiveness)and permissive parenting(high responsiveness but low demandingness)were less effective in reducing behavioral *** results remained robust after addressing potential issues of reverse causality,omitted variable bias,and selective sorting between teachers and *** results also show that teachers and their interactions with parents played an important role in mitigating the adverse effects of neglectful ***,our findings suggest that adopting parenting styles that strike a balance between discipline and care can strongly fosterpositivebehaviors in adolescents.

Clinical Analysis of Microdebrider Removal of Recurrent Respiratory Papilloma: A Report of 33 Cases

OBJECTIVE To investigate the advantages of applying microdebrider re- moval of juvenile-onset recurrent respiratory papillary epithelioma (jo- RRP), using an endoscopy-assisted prop-up laryngoscope. METHODS The degree of severity of the neoplasms was divided into 3 scores (i.e. 1 point for a slight degree, 2 points for a moderate and 3 points for a severe degree). The involvement of 22 respiratory and diges- tive sub-areas was evaluated and the total accumulative scoring and the scores for the lesions in the vocal area were calculated and sub - grouped. All the papillary epitheliomas were excised using a laryngeal micro-debrider or the micro laryngeal forceps under endoscopy-assist- ed suspension laryngoscopy. The differences between applications of the two modes of treatment for the cases of the groups with same scoring were compared as follows: the operation time, interval of operation, recent vocal quality after operation, postoperative scarring and incision of the trachea, as well as the distribution of tumors in a re-operation. REsuLTS The 142 operative procedures were performed in 33 pediatric patients, with application of a laryngeal microdebrider in 14 cases and ex- cision in 19. Compared to the excision group (EG), the laryngeal mi- crodebrider group (MDG) displayed many superior features, such as a short operation time, long interval between operations, obvious improve- ment in vocal quality soon after operation, and a low frequency of subse- quent tumors. There was a significant difference between these modes of the operation. A postoperative incision of the trachea was required in 2 cases of EG, whereas no post-operation was needed in the MDG. CONCLUSION There are many advantages in using laryngeal micro-de- brider removal of jo -RRP. The procedure is simple and convenient, having a distinct operating field, precise incision, minor wounds, fewer complications and better vocal quality soon after operation, as well as quicker rehabilitation and longer inte
