Multidrug resistance extended spectrum β-lactamase and AmpC producing escherichia coli isolated from the environment of Bogor Slaughterhouse,Indonesia

Objective: To determine the multidrug resistance extended spectrum p-lactamase and AmpC(eSBL/AmpC producing) escherichia coli(e. coli) isolated from the environment of Bogor slaughterhouse, ***: A total of 35 samples from 7 locations in slaughterhouse i.e., source of water,slaughtering floor, swab of carcass area floor, swab of evisceration area floor, untreated waste water, treated waste water, drinking water for cattle were collected from March to April 2016. Presence of eSBL/AmpC producing e. coli and susceptibility testing against8 antimicrobial agents(penicillin G, streptomycin, gentamycin,ciprofloxacin, enrofloxacin, tetracycline, trimethoprim-sulfamethoxazole, and polymyxin B) were detected by disk diffusion test according to Clinical and Laboratory Standards ***: eSBL/AmpC producing e. coli were identified in 14.3%(5/35) of the collected samples from the environment of Bogor slaughterhouse. eSBL/AmpC-producing e. coli isolates were detected in untreated waste water(n=3), slaughtering floor(n=1), and carcass area floor(n=1). Most of eSBL/AmpC-producing e. coli isolates(80%) showed multidrug resistance phenotypes against at least three classes of antibiotics. The highest incidence of antibiotics resistance was against penicillin G(100.0%) and streptomycin(100.0%), followed by gentamicin(60.0%), trimethoprim-sulfamethoxazole(60.0%),tetracycline(40.0%), ciprofloxacin(40.0%), enrofloxacin(20.0%), and polymyxin B(0.0%).Conclusions: The transmission of antimicrobial resistant bacteria into the environment may be a potential risk for human health.

CTX-M producing escherichia coli isolated from cattle feces in Bogor slaughterhouse,Indonesia

Objective: To determine the occurrence of CTX-M producing escherichia coli(e. coli)from cattle feces in Bogor slaughterhouse, ***: A total of 220 cattle feces samples were collected from Bogor slaughterhouse from March to April 2015. Presence of extended-spectrum beta-lactamase(eSBL) producing e. coli was detected by disc diffusion test based on the recommendation from Clinical and Laboratory Standards Institute(2014). Bacterial strains which were confirmed as producing eSBLs were further analyzed for the presence of bla genes of the eSBL by ***: The results showed that CTX-M producing e. coli isolates were detected in 19 samples from 220 samples(8.6%). The b-lactamase genes detected were CTX-M-1(n = 10) and CTX-M-9(n = 9). All of the CTX-M producing e. coli isolates showed multidrug resistance phenotypes to at least four antibiotics. The highest incidence of antibiotics resistance was showed to ampicillin(100.0%), cefotaxime(100.0%), and cefpodoxime(100.0%), followed by streptomycin(84.3%), trimethoprim-sulfamethoxazole(73.7%), erythromycin(52.6%), kanamycin(26.3%), doxycycline(10.5%), and ceftazidime(0.0%).Conclusions: Detection of CTX-M-producing e. coli in cattle feces raises important questions as they can represent a potential risk factor to public health.

Therapeutic temperature modulation is associated with pulmonary complications in patients with severe traumatic brain injury

AIM: To examine complications associated with the use of therapeutic temperature modulation(mild hypothermia and normothermia) in patients with severe traumatic brain injury(TBI). MeTHODS: One hundred and fourteen charts were reviewed. Inclusion criteria were: severe TBI with Glasgow Coma Scale(GCS) ensive care unit(ICU) stay > 24 h and non-penetrating TBI. Patients were divided into two cohorts: the treatment group received therapeutic temperature modulation(TTM) with continuous surface cooling and indwelling bladder temperature probes. The control group received standard treatment with intermittent acetaminophen for fever. Information regarding complications during the time in the ICU was collected as follows: Pneumonia was identified using a combination of clinical and laboratory data. Pulmonary embolism, pneumothorax and deep venous thrombosis were identified based onimaging results. Cardiac arrhythmias and renal failure were extracted from the clinical documentation. acute respiratory distress syndrome and acute lung injury were determined based on chest imaging and arterial blood gas results. A logistic regression was conducted to predict hospital mortality and a multiple regression was used to assess number and type of clinical complications. ReSULTS: One hundred and fourteen patients were included in the analysis(mean age = 41.4, SD = 19.1, 93 males), admitted to the Jackson Memorial Hospital Neuroscience ICU and Ryder Trauma Center(mean GCS = 4.67, range 3-9), were identified and included in the analysis. Method of injury included motor vehicle accident(n = 29), motor cycle crash(n = 220), blunt head trauma(n = 212), fall(n = 229), pedestrian hit by car(n = 216), and gunshot wound to the head(n = 27). ethnicity was primarily Caucasian(n = 260), as well as Hispanic(n = 227) and African American(n = 223); four patients had unknown ethnicity. Allograft loss from acute Page kidney secondary to trauma after kidney transplantation

We report a rare case of allograft loss from acute Page kidney secondary to trauma that occurred 12 years after kidney transplantation. A 67-year-old Caucasian male with a past surgical history of kidney transplant presented to the emergency department at a local hospital with left lower abdominal tenderness. He recalled that his cat, which weighs 15 lbs, jumped on his abdomen 7 d prior. On physical examination, a small tender mass was noticed at the incisional site of the kidney transplant. He was producing a normal amount of urine without hematuria. His serum creatinine level was slightly elevated from his baseline. Computer tomography revealed a large subscapular hematoma around the transplant kidney. The patient was observed to have renal trauma grade Ⅱ at the hospital over a period of three days, and he was finally transferred to a transplant center after his urine output significantly decreased. Doppler ultrasound demonstrated an extensive peri-allograft hypoechoic area and abnormal waveforms with absent arterial diastolic flow and a patent renal vein. Despite surgical decompression, the allograft failed to respond appropriately due to the delay in surgical intervention. This is the third reported case of allograft loss from acute Page kidney following kidney transplantation. This case reinforces that kidney care differs if the kidney is solitary or a transplant. early recognition and aggressive treatments are mandatory, especially in a case with Doppler signs that are suggestive of compression.

Biodegradation of Natural estrogens by Biofilms from Biological Activated Carbon: effect of Temperature

In order to obtain information on the biodegradation potential of biofilms involved in the removal of natural estrogens by biological activated carbon (BAC) columns, batch degradation of estrone (e1) and 17β-estradiol (e2) at temperature of 5℃, 20℃ and 35℃ by biofilms from four BAC columns (packed with activated carbon of particle size ranging from 0.5 - 0.59 mm and 1.0 - 1.19 mm into two bed depths) was studied. The results indicated that e2 was degraded faster by than e1 at all three temperatures and with the increasing of temperature, the amount of e1 converted from e2 increased. By fitting observed concentration data with a first-order rate expression, the bio-mass based degradation rate constants (kVSS) for e1 and e2 under all experimental conditions were estimated and linear relationship between lnkVSS and 1/T (T = absolute temperature) was demonstrated, resulting that with the increasing of the experimental temperature, degradation rate of biofilms for both e1 and e2 increased, and the increasing rate for e2 was higher than that for e1.

Numerical Study of Flow and Thermal Field on a Parallel Flow Vortex Tube

Flow and thermal field of a parallel flow vortex tube has been simulated and analyzed numerically. A secondary zone model is found at the core region near the inlet to the middle of the vortex tube. Blockage effect due to a narrow area of the hot exit has deflected air flow towards the cold exit, caused expansion and compression at the cold and hot outlet, respectively. The cooling and heating effect due to energy separation is contributed by expansion and compression of air near the outlet. Coeficient of performance (COP) for a refrigerator is higher as cold mass fraction increases due to a higher temperature difference and cold mass flow rate.

Ultrasound curriculum taught by first-year medical students： A four-year experience in Tanzania

BACKGROUND: Diagnostic imaging is an integral aspect of care that is often insufficient, if not altogether absent, in rural and remote regions of low to middle income countries(LMICs) such as Tanzania. The introduction of ultrasound can significantly impact treatment in these countries due to its portability, low cost, safety, and usefulness in various medical assessments. This study reviews the implementation of a four-week ultrasound course administered annually from 2013–2016 in a healthcare professional school in Mwanza, Tanzania by first-year allopathic US medical ***: Participants(n=582, over 4 years) were recruited from the Tandabui Institute of Health Sciences and Technology to take the ultrasound course. Subjects were predominantly clinical officer students, but other participants included other healthcare professional students, practicing healthcare professionals, and school employees. Data collected includes pre-course examination scores, post-course examination scores, course quiz scores, demographic surveys, and postcourse feedback surveys. Data was analyzed using two-tailed t-tests and the single factor analysis of variance(ANOVA).ReSULTS: For all participants who completed both the pre-and post-course examinations(n=229, 39.1% of the total recruited), there was a significant mean improvement in their ultrasound knowledge of 42.5%, Pests that trained first-year medical students can effectively teach a point of care ultrasound course to healthcare professional students within four weeks in Tanzania. Future investigation into the level of long-term knowledge retention, impact of ultrasound training on knowledge of human anatomy and diagnostic capabilities, and how expansion of an ultrasound curriculum has impacted access to care in rural Tanzania is warranted.

近十年一些研究显示：特异性抗原、抗体结合形成的免疫复合物ＩＣ，经经典途径激活补体，并由人或灵长类动物红血球　（Ｒｅｄｂｌｏｏｄｃｅｌｌｓ，Ｒｂｃ；Ｅｒｙｔｈｒｏｃｙｃｅ，Ｅ）上存在的Ｃ３ｂ受体ＣＲ１介导，先与循环中Ｒｂｃ结合，后由其携带，再经过肝、脾中的网状内皮等系统处理，而得以清除。本文报道，用先生物素化的抗－ＣＲＩ和抗－人ＩｓＭ（抗原）的两种单抗，后以链球亲合素（Ｓｔｒｅｐｔａｖｉｄｉｎ，ＳＡ）交联成的异源多聚体（Ｈｅｔｅｒｏｐｏｌｙｍｅｒ，ＨＰ）分别灌注到５只不同的在循环中预先注射了１２５Ｉ标记的血源性“病原体模式”抗原（人ＩｇＭ）的栗鼠猴体内，结果发现都促进了抗原（Ａg）与Ｒｂｃ的结合，其被清除的量约达７０％～９０％。同时，在每个Ｒｂｃ上ＣＲ1数目有一定的降低，它类似于被Ｃ３ｂ调理的ⅠC在通常被清除时ＣＲ１降低的一般观察结果；吸收Ｈｐ－ＡgⅠＣ的主要器官是肝和脾。这方面工作的最终目的是将可利用不同ＨＰ系统在体内清除Ａｇ，使其成为临床上对疾病的一种潜在性免疫学治疗手段。
