CDX2 protein expression compared to alcian blue staining in the evaluation of esophageal intestinal metaplasia

AIM:To compare the sensitivity and specificity of CDX2 and alcian blue(AB) p H 2.5 staining in identifying esophageal intestinal ***:One hundred and ninty-nine biopsies from 186 patients were retrospectively reviewed and categorized as Barrett's esophagus(BE)(n =108); non-Barrett's esophagus(NBE)(n =48); columnar blue cells(CB) and esophageal glands(EG)(n =43).The biopsies were stained with AB and immunostained for CDX2 using a mouse monoclonal antibody from Biogenex(clone CDX2-88) and the Ventana Discovery X automated *** positive and negative predictive value of each group was used to determine the predictive power of CDX2 and AB in diagnosing intestinal ***:All of the 108 BE biopsies(100%) were positive for AB and 102 of them(94.4%) were positive for *** six BE patients(5.6%) who failed to stain with CDX2 were found to have lost the focus of intestinal metaplasia upon deeper sectioning for *** AB and CDX2 were negative in 43 out of 48(89.6%) NBE *** NBE patients(10.4%) were falsely positive for AB due to the presence of EG and CB in these *** cases were all CDX2 *** addition,5 AB negative NBE were found to be CDX2 *** on these results the CDX2 immunostain had similar sensitivity but higher specificity(100% vs about 91%) than AB in detecting intestinal type metaplasia in these *** data shows that CDX2 has a better PPV in detecting intestinal metaplasia as compared to AB(95.6% vs 71.5%,respectively).CONCLUSION:CDX2 has a better positive predictive value than AB in detecting intestinal ***2may be useful when challenged by gastro-esophageal biopsies containing mimikers of BE.

Airway management is a skill necessary for healthcare providers who manage patients. Intubation is a skill that both anesthesiologists and intensivists use daily. We present a case of a neonate that developed significant worsening stridor after a successful intubation for a hernia repair. After 4 intubation attempts, with easy visualization of the vocal cords and unsuccessful placement of the endotracheal tube, the patient was emergently brought to the operating room for evaluation.

Disease Cycle, Development and Management of Sclerotinia Stem rot of Potato

Potato is severely affected by Sclerotinia sclerotiorum in the US Pacific Northwest (PNW) and satisfactory disease management has not been achieved until recently. recent research has given a better understanding of key epidemiological factors which has resulted in improved disease management in the PNW. This work reviews the epidemiology of Sclerotinia stem rot and highlights information that has led to the better management of the disease on potato. The primary source of inoculum of Sclerotinia sclerotiorum for potato in the Columbia Basin of the PNW are ascospores produced within potato fields or carried by wind currents from neighboring fields planted to winter cereals or other crops. Ascospores are ejected from apothecia and disseminated throughout the Columbia Basin over an extended time period. Immature apothecia generally first emerge at or shortly after row closure in potato fields in the Columbia Basin. Ascospores of S. sclerotiorum are incapable of direct infection of intact green potato tissues, and flower blossoms are crucial for infection and development of the disease in potato. Airborne ascospores are deposited on open potato blossoms still attached to the canopy. Infested flowers fall and are trapped on stems, usually leaf axils, or fall on the ground, and fungal mycelia then rapidly colonize the blossoms when humidity is high in the plant canopy. Ascospores are also deposited on senescent and dead plant material on the ground, germinate, and produce mycelium. Infection occurs shortly after contaminated blossoms become lodged on stems in the plant canopy, or after stems come in contact with contaminated fallen blossoms or decomposing plant tissues on the ground. Infection can occur within 3 days after contact of contaminated blossoms with green tissue. Lesions initially appear on potato stems 14 to 30 days after row closure and 12 to 20 days following full bloom of primary inflorescences of potato in the Columbia Basin. Does Climatic Variability Influence Agricultural Land Prices under Differing Uses? The Texas High Plains Case

The Texas High Plains faces projections of increasing temperature and declining precipitation in the future on account of its semi-arid climate. This research evaluated the impact of climatic variability on agricultural land prices under different land uses in the Texas High Plains, employing the ricardian approach of land climate pricing over a study period of 1991-2011. The results indicate that climatic variability had a greater impact on irrigated land prices as compared to dryland and ranchland. This study could be instrumental in predictive market analyses of rural land values in semi-arid economies which are vulnerable to future climate change.

Critical Literature review of the Homeopathic Compound Traumeel for Treatment of Inflammation

Traumeel is a compound that is utilized by a wide variety of healthcare practitioners to treat inflammatory states. It is utilized extensively in Germany for multiple inflammatory conditions. The purpose of this document is to review and critique publications written about Traumeel and Traumeel S, a proprietary formulation. Furthermore, this review will determine if the literature supports substituting Traumeel for Non-Steroidal Anti-Inflammatory Drugs (NSAIDs) and corticosteroids or if adding Traumeel to NSAID and corticosteroid treatment protocols benefits patients with inflammation. Long-term use of NSAIDs and corticosteroids causes significant pathology leaving many patients without an effective treatment to manage their inflammatory condition. Traumeel does not have the severe side effect profile of NSAIDs and corticosteroids. There are important implications of the studies included in this review. The literature supports Traumeel as an effective alternative to NSAIDS and corticosteroids in preventing stomatitis for patients undergoing chemotherapy. Traumeel also provides pain-free outcomes following musculoskeletal and tissue injuries.

Structure and properties of bioabsorbable polyglycolide (PGA) and poly(glycolide-co-lactide) (PGA-co-PLA)fibers were investigated during several industrial processing stages and in vitro degradation by means of wide-angle X-raydiffraction (WAXD), dynamic mechanical analysis (DMA) and mechanical property tests. In the orientation stage, the PGAfibers were found to have higher degrees of crystallinity than corresponding PGA-co-PLA samples produced under similarconditions. In the hot-stretching and post-annealing stages, after fibers were braided, PGA samples were found to gain morecrystallinity and higher T_g than PGA-co-PLA samples. The higher crystallinity in PGA fibers resulted in a slower rate ofdegradation. DMA results showed that a great deal of internal stress that was built during orientation and hot-stretchingstages was released in the post-annealing stage for a1l PGA and PGA-co-PLA samples. During earlier stages of in vitrodegradation, both PGA and PGA-co-PLA samples exhibited the typical cleavage-induced crystallization mechanism. Theheat shrinkage in the glass transition area was found to disappear after 6-8 days of degradation for all PGA and PGA-co-PLAsamples, indicating the amorphous portions of the polymers lost orientation after a short period in the buffer solution, mostlikely due to relaxation of the cleaved chains.

High affinity soluble ILT2 receptor:a potent inhibitor of CD8^(+)T cell activation

Using directed mutagenesis and phage display on a soluble fragment of the human immunoglobulin superfamily receptor ILT2(synonyms:LIr1,MIr7,CD85j),we have selected a range of mutants with binding affinities enhanced by up to 168,000-fold towards the conserved region of major histocompatibility complex(MHC)class I *** in a dimeric form,either by chemical cross-linking with bivalent polyethylene glycol(PEG)derivatives or as a genetic fusion with human IgG Fc-fragment,the mutants exhibited a further increase in ligand-binding strength due to the avidity effect,with resident half-times(t1/2)on the surface of MHC I-positive cells of many *** novel compounds antagonized the interaction of CD8 co-receptor with MHC I in vitro without affecting the peptide-specific binding of T-cell receptors(TCrs).In both cytokine-release assays and cell-killing experiments the engineered receptors inhibited the activation of CD8^(+)cytotoxic T lymphocytes(CTLs)in the presence of their target cells,with subnanomolar potency and in a dose-dependent *** a selective inhibitor of CD8^(+)CTL responses,the engineered high affinity ILT2 receptor presents a new tool for studying the activation mechanism of different subsets of CTLs and could have potential for the development of novel autoimmunity therapies.
