Predictors of poor outcome in gastrointestinal bleeding in emergency department

AIm: To determine the prognostic risk factors of gastrointestinal bleeding in emergency department ***: The trial was a retrospective single-center study involving 600 patients over 18-years-old and carried out with approval by the Institutional Ethics Committee. Patient data included demographic characteristics, symptoms at admission, past medical history, vital signs, laboratory results, endoscopy and colonoscopy results, length of hospital stay, need of intensive care unit(ICU) admission, and mortality. mortality rate was the principal endpoint of the study, while duration of hospital stay, required interventional treatment, and admission to the ICU were secondary ***: The mean age of patients was 61.92-yearsold. Among the 600 total patients, 363(60.5%) underwent upper gastrointestinal endoscopy and the most frequent diagnoses were duodenal ulcer(19.2%) and gastric ulcer(12.8%). One-hundred-and-fifteen(19.2%) patients required endoscopic treatment, 20(3.3%) required surgical treatment, and 5(0.8%) required angiographic embolization. The mean length of hospital stay was 5.21 ± 5.85 d. The mortality rate was 6.3%. The ICU admission rate was 5.3%. Patients with syncope, higher blood glucose levels, and coronary artery disease had significantly higher ICU admission rates(P = 0.029, P = 0.043, and P = 0.002, respectively). Patients with low thrombocyte levels, high creatinine, high international normalized ratio, and high serum transaminase levels had significantly longer hospital stay(P = 0.02, P = 0.001, P = 0.019, and P = 0.005, respectively). Spatially-engineered diffractive surfaces have emerged as a powerful framework to control light-matter interactions for statistical inference and the design of task-specific optical ***,we report the design of diffractive surfaces to all-optically perform arbitrary complex-valued linear transformations between an input(Ni)and output(No),where Ni and No represent the number of pixels at the input and output fields-of-view(FOVs),***,we consider a single diffractive surface and use a matrix pseudoinverse-based method to determine the complex-valued transmission coefficients of the diffractive features/neurons to all-optically perform a desired/target linear *** addition to this data-free design approach,we also consider a deep learning-based design method to optimize the transmission coefficients of diffractive surfaces by using examples of input/output fields corresponding to the target *** compared the all-optical transformation errors and diffraction efficiencies achieved using data-free designs as well as data-driven(deep learning-based)diffractive designs to all-optically perform(i)arbitrarily-chosen complex-valued transformations including unitary,nonunitary,and noninvertible transforms,(ii)2D discrete Fourier transformation,(iii)arbitrary 2D permutation operations,and(iv)high-pass filtered coherent *** analyses reveal that if the total number(N)of spatially-engineered diffractive features/neurons is≥Ni×No,both design methods succeed in all-optical implementation of the target transformation,achieving negligible ***,compared to data-free designs,deep learning-based diffractive designs are found to achieve significantly larger diffraction efficiencies for a given N and their all-optical transformations are more accurate for Nre generally applicable to various optical processors that employ spatially-engineered diffractive surfaces.

multispectral imaging has been used for numerous applications in e.g.,environmental monitoring,aerospace,defense,and ***,we present a diffractive optical network-based multispectral imaging system trained using deep learning to create a virtual spectral filter array at the output image *** diffractive multispectral imager performs spatially-coherent imaging over a large spectrum,and at the same time,routes a pre-determined set of spectral channels onto an array of pixels at the output plane,converting a monochrome focal-plane array or image sensor into a multispectral imaging device without any spectral filters or image recovery ***,the spectral responsivity of this diffractive multispectral imager is not sensitive to input polarization *** numerical simulations,we present different diffractive network designs that achieve snapshot multispectral imaging with 4,9 and 16 unique spectral bands within the visible spectrum,based on passive spatially-structured diffractive surfaces,with a compact design that axially spans ~72λ_(m),where λ_(m) is the mean wavelength of the spectral band of ***,we experimentally demonstrate a diffractive multispectral imager based on a 3D-printed diffractive network that creates at its output image plane a spatially repeating virtual spectral filter array with 2×2=4 unique bands at terahertz *** to their compact form factor and computation-free,power-efficient and polarization-insensitive forward operation,diffractive multispectral imagers can be transformative for various imaging and sensing applications and be used at different parts of the electromagnetic spectrum where high-density and wide-area multispectral pixel arrays are not widely available.

Let r be a commutative ring with 1≠ 0. A proper ideal I of r is a semiprimary ideal of r if whenever a, b ∈ r and a b ∈ I, we have a ∈ √I or b ∈√I; and I is a weakly semiprimary ideal of r if whenever a, b ∈ r and 0 ≠ ab ∈ √I, we have a ∈√I or b ∈ √I. In this paper, we introduce a new class of ideals that is closely related to the class of （weakly） semiprimary ideals. Let I（r） be the set of all ideals of r and let δ ： I（r） → I（r） be a function. Then δ is called an expansion function of ideals of r if whenever L, I, J are ideals of r with J I, we have L δ（L） and δ（J） δ（I）. Let δ be an expansion function of ideals of r. Then a proper ideal I of r is called a δ-semiprimary （weakly δ-semiprimary） ideal of r if ab ∈ I （0 ≠ ab ∈ I） implies a ∈ δ（I） or b∈ δ（I）. A number of results concerning weakly δ-semiprimary ideals and examples of weakly δ-semiprimary ideals are given.

The precise engineering of materials and surfaces has been at the heart of some of the recent advances in optics and *** advances related to the engineering of materials with new functionalities have also opened up exciting avenues for designing trainable surfaces that can perform computation and machine-learning tasks through light-matter interactions and ***,we analyze the information-processing capacity of coherent optical networks formed by diffractive surfaces that are trained to perform an all-optical computational task between a given input and output *** show that the dimensionality of the all-optical solution space covering the complex-valued transformations between the input and output fields-of-view is linearly proportional to the number of diffractive surfaces within the optical network,up to a limit that is dictated by the extent of the input and output *** diffractive networks that are composed of larger numbers of trainable surfaces can cover a higher-dimensional subspace of the complex-valued linear transformations between a larger input field-of-view and a larger output field-of-view and exhibit depth advantages in terms of their statistical inference,learning,and generalization capabilities for different image classification tasks when compared with a single trainable diffractive *** analyses and conclusions are broadly applicable to various forms of diffractive surfaces,including,e.g.,plasmomc and/or dielectric-based metasurfaces and flat optics,which can be used to form all-optical processors.

Objective:To identify the clinical characteristics and outcomes of hospital-acquired SArS-CoV-2 infection during the vaccination period nationwide in ***:COVID-19 patients followed in the pandemic services across Turkey between January 1,2021,and march 31,2022 were investigated *** COVID-19 was defined as a patient neither diagnosed with COVID-19 nor suspected COVID-19 at the hospital admission and was confirmed COVID-19≥5 days after hospital *** primary outcome of this study was in-hospital mortality;demographic features and vaccination status was compared between survivors and ***:During the study period,15573 COVID-19 patients were followed in 18 centers and 543(3.5%)patients were nosocomial *** patients with nosocomial COVID-19(80.4%)were transferred from medical wards.162(29.8%)of the patients with nosocomial COVID-19 admitted to the intensive care unit due to disease severity and 138(25.4%)of the patients died during hospital *** age(≥65 years)and number of comorbid diseases(≥2)was found to be associated with mortality in nosocomial COVID-19(Or 1.74,95%CI 1.11-2.74 and Or 1.60,95%CI 1.02-2.56,respectively).Vaccination was associated with survival in nosocomial COVID-19(Or 0.25,95%CI 0.16-0.38).Conclusions:Patients with nosocomial COVID-19 had increased admission to intensive care units and higher mortality *** can decrease the in-hospital mortality rate.

This study aims to examine the time-varying efficiency of the Turkish stock market’s major stock index and eight sectoral indices,including the industrial,financial,service,information technology,basic metals,tourism,real estate investment,and chemical petrol plastic,during the COVID-19 outbreak and the global financial crisis(GFC)within the framework of the adaptive market *** study employs multifractal detrended fluctuation analysis to illustrate these sectors’multifractality and short-and long-term *** results show that all sectoral returns have greater persis-tence during the COVID-19 outbreak than during the ***,the real estate and information technology industries had the lowest levels of efficiency during the GFC and the COVID-19 ***,the fat-tailed distribution has a greater effect on multifractality in these *** results validate the conclusions of the adaptive market hypothesis,according to which arbitrage opportunities vary over time,and contribute to policy formulation for future outbreak-induced economic crises.

Lithium aluminum titanium phosphate(LATP)is one of the materials under consideration as an electrolyte in future all-solid-state lithium-ion *** ceramic processing,the presence of secondary phases and porosity play an important *** a presence of more than one secondary phase and pores,image analysis must tackle the difficulties about distinguishing between these microstructural *** this study,we study the phase evolution of LATP ceramics sintered at temperatures between 950 and 1100℃by image segmentation based on energy-dispersive X-ray spectroscopy(EDS)elemental maps combined with quantitative analysis of LATP *** found aluminum phosphate(AlPO4)and another phosphate phase((Lix)PyOz).The amount of these phases changes with sintering ***,since the grains act as an aluminum source for AlPO4 formation,the aluminum content in the LATP grains ***,the amount of secondary phase changes from more(Lix)PyOz at 950℃to mainly AlPO4 at 1100℃sintering *** also used scanning electron microscopy(SEm)and confocal laser scanning microscopy(CLSm)to study the evolution of the LATP grains and AlPO4,and LATP grain size increases with sintering *** addition,transmission electron microscopy(TEm)was used for the determination of grain boundary width and to identify the amorphous structure of AlPO4.
