A novel expressed prostatic secretion(EpS)-urine metabolomic signature for the diagnosis of clinically significant prostate cancer

Objective:Significant efforts are currently being made to identify novel biomarkers for the diagnosis and risk stratification of prostate cancer(pCa).Metabolomics can be a very useful approach in biomarker discovery because metabolites are an important read-out of the disease when characterized in biological *** aimed to determine a metabolomic signature which can accurately distinguish men with clinically significant pCa from those affected by benign prostatic hyperplasia(BpH).Methods:We first performed untargeted metabolomics using ultrahigh-performance liquid chromatography tandem mass spectrometry on expressed prostatic secretion urine(EpS-urine)from 25 patients affected by BpH and 25 men with clinically significant pCa(defined as Gleason score≥3+4).Diagnosis was histologically confirmed after surgical *** EpS-urine metabolomic approach was then applied to a larger,prospective cohort of 92 consecutive patients undergoing multiparametric magnetic resonance imaging for clinical suspicion of pCa prior to ***:We established a novel metabolomic signature capable of accurately distinguishing pCa from benign tissue.A metabolomic signature was associated with clinically significant pCa in all subgroups of the prostate Imaging Reporting and Data System(pI-RADS)classification(100%and 89.13%of accuracy when the pI-RADS was in range of 1–2 and 4–5,respectively,and 87.50%in the more critical cases when the pI-RADS was 3).Conclusions:A combination of metabolites and clinical variables can effectively help in identifying pCa patients that might be overlooked by current imaging *** from EpS-urine should help in defining the diagnostic pathway of pCa,thus improving pCa detection and decreasing the number of unnecessary prostate biopsies.

Surface Coating of NiTi Shape Memory Alloys with Calcium phosphates by Dip-coating or plasma-spraying-biological Characterization Examined by in vitro Testing Methods

The influence of different surface coatings of NiTi shape memory allays was examined using in vitro testing methods. plates of superelastic nickel-titanium shape memory allay （ NiTi ） were coated with calcium phosphates （ hydroxyapatite ） by high-temperature plasma-spraying or by dip-coating. The biocompatibility was tested in vitro by cultivation of isolated human granulocytes and whole blood cells. As substrates, pure NiTi, plasma-spray-coated NiTi and dip-coated NiTi were used. Isolated granulocytes showed an increased adhesion to both calcium phosphate-coated NiTi samples. Compared to non-coated NiTi or dip-coated NiTi, the number of dead granulocytes adherent to plasma-sprayed surfaces was significantly increased （ p 〈 0.01 ）. Whether the d/f- ferences in apoptosis of granulocytes on dip-coated vs plasma-sprayed coatings observed are due to differences in material surface morphologies has to be analyzed in further studies. Because of the cellular interactions with the coating layers, h is likely that the results obtained are not caused by the underlying NiTi but due to the coating itself.

Can bacterial virulence factors predict antibiotic resistant Helicobacter pylori infection?

AIM To evaluate the association between virulence factor status and antibiotic resistance in Helicobacter pylori(H. pylori)-infected patients in Ireland. METHODS DNA was extracted from antral and corpus biopsies obtained from 165 H. pylori-infected patients. Genotyping for clarithromycin and fluoroquinolone-mediating mutations was performed using the Genotype Helico DR assay. cag A and vac A genotypes were investigated using pCR. RESULTS primary, secondary and overall resistance rates for clarithromycin were 50.5%(n = 53/105), 78.3%(n = 47/60) and 60.6%(n = 100/165), respectively. primary, secondary and overall resistance rates for fluoroquinolones were 15.2%(n = 16/105) and 28.3%(n = 17/60) and 20%(n = 33/165), respectively. Resistance to both antibiotics was 12.4%(n = 13/105) in treatment-na?ve patients, 25%(n = 15/60) in those previously treated and 17%(n = 28/165) overall. A cag A-positive genotype was detected in 22.4%(n = 37/165) of patient samples. The dominant vac A genotype was S1/M2 at 44.8%(n = 74/165), followed by S2/M2 at 26.7%(n = 44/165), S1/M1 at 23.6%(n = 39/165) and S2/M1 at 4.8%(n = 8/165). primary clarithromycin resistance was significantly lower in cag A-positive strains than in cag A-negative strains [32%(n = 8/25) vs 56.3%(n = 45/80); p = 0.03]. Similarly, in patients infected with more virulent H. pylori strains bearing the vac A s1 genotype, primary clarithromycin resistance was significantly lower than in those infected with less virulent strains bearing the vac A s2 genotype, [41%(n = 32/78) vs 77.8%(n = 21/27); p = 0.0001]. No statistically significant association was found between primary fluoroquinolone resistance and virulence factor *** Genotypic H. pylori clarithromycin resistance is high and cag A-negative strains are dominant in our population. Less virulent(cag A-negative and vac A S2-containing) strains of H. pylori are associated with primary clarithromycin resistance.

Effect of introducing an online system on the follow-up of elbow arthroplasty

AIM To evaluate the effect of introducing a structured online follow-up system on the response *** Since June 2015 we have set up an electronic followup system for prosthesis in orthopedic patients. This system allows prospective data gathering using both online and paper questionnaires. In the past all patients received questionnaires on paper. This study includes only patients who received elbow arthroplasty. Response rates before and after introduction of the online database were compared. After the implementation,completeness of the questionnaires was compared between paper and digital versions. Organizational commitment and turnover intention among rural nurses in the philippines: Implications for nursing management

Intermittent injections of parathyroid hormone(ipTH) are applied clinically to stimulate bone formation by osteoblasts, although continuous elevation of parathyroid hormone(pTH) primarily results in increased bone resorption. Here, we identified Calca,encoding the sepsis biomarker procalcitonin(proCT), as a novel target gene of pTH in murine osteoblasts that inhibits osteoclast formation. During ipTH treatment, mice lacking proCT develop increased bone resorption with excessive osteoclast formation in both the long bones and axial skeleton. Mechanistically, proCT inhibits the expression of key mediators involved in the recruitment of macrophages, representing osteoclast precursors. Accordingly, proCT arrests macrophage migration and causes inhibition of early but not late osteoclastogenesis. In conclusion, our results reveal a potential role of osteoblast-derived proCT in the bone microenvironment that is required to limit bone resorption during ipTH.

Background：pediatric emergency rooms （pERs） in Chinese hospitals are perpetually full of sick and injured children because of the lack of sufficiently developed community hospitals and low access to family *** aim of this study was to evaluate the clinical value of a new five-level Chinese pediatric emergency triage system （CpETS）,modeled after the Canadian Triage System and Acuity ***：In this study,we compared CpETS outcomes in our pER relative to those of the prior two-level *** who visited our pER before （January 2013-June 2013） and after （January 2014-June 2014） the CpETS was implemented served as the control and experimental group,*** flow,triage rates,triage accuracy,wait times （overall and for severe patients）,and patient/family satisfaction were compared between the two ***：Relative to the performance of the former system experienced by the control group,the CpETS experienced by the experimental group was associated with a reduced patient flow through the pER （Cox-Stuart test,t =0,p 〈 0.05）,a higher triage rate （93.40% vs.90.75%;χ^2 =801.546,p 〈 0.001）,better triage accuracy （96.32% vs.85.09%;χ^2 =710.904,p 〈 0.001）,shorter overall wait times （37.30 ± 13.80 min vs.41.60 ± 15.40 min;t =11.27,p 〈 0.001）,markedly shorter wait times for severe patients （2.07 [0.65,4.11] min vs.3.23 [1.90,4.36] min;z =-2.057,p =0.040）,and higher family satisfaction rates （94.23% vs.92.21%;x2 =321.528,p 〈 0.001）.Conclusions：Implementing the CpETS improved nurses＇ abilities to triage severe patients and,thus,to deliver the urgent treatments more *** system shunted nonurgent patients to outpatient care effectively,resulting in improved efficiency of pER health-care delivery.
