Piroxicam is a non-steroidal anti-inflammatory drug (NSAID) exhibiting analgesic and antipiretic properties and widely used in the management of chronic diseases. Associated with these use, there are registers of adverse reactions. Microparticle formulations in hydrogel matrix can be used to form a semi-permeable barrier which enables their actions and can reduce the effects. This work presents a study on the effect of gelatin/carboxymethyl cellulose (Gel/CMC) semi-IPN matrix composition on the retention and kinetic behavior of releasing this drug. The microparticles were obtained through the emulsion-crosslinking method using 23 factorial planning and the piroxicam was added as solid particles. In order to characterize the interaction between matrix-active agent and quantification of the drug, the following techniques were applied: SEM, DSC and XRD. SEM micrographs revealed microparticles with regular and spherical shape and that in some compositions the drug is partially absorbed and not encapsulated. Beyond that DSC and XRD analyses indicate that the piroxicam remained in the matrixes, maintains the same crystalline form. The factorial planning analysis indicated that matrix obtained a maximum encapsulation efficiency (EE%) of 10.64% and became possible to create a response surface graph using a EE% as answer. In addition to this, release kinetics analyses demonstrated that the release process seems to be governed by distinctly kinetic models, considering the composition of the sample. In some samples the release can be driven by Fickinianan diffusion, others by anomalous transport or swelling.

Purpose: Children with liver impairment are likely to develop changes in autonomic nervous function and delay in motor development. The assessment and identification of these dysfunctions may allow an appropriate physiotherapeutic care. Method: Cross sectional study of 18 infants, 11 controls and 7 infants (post-liver transplantation) with an average age of 10 ± 4.5 months, was evaluated in pre- and post-liver transplant. All infants underwent to assessments of motor skills, body composition, chest and abdominal motion, and cardiac autonomic modulation was measured by heart rate variability. Results: Motor delay and malnutrition were found in all infants. The gravity index (PELD)—pediatric end-stage liver disease—showed a negative correlation with the Alberta Infant Motor Scale (r = 0.83, p = 0.01). In addition, reduced parasympathetic modulation was demonstrated by the rMSSD, SD1 and ApEn, pre- and post-transplant. Conclusion: Infants with liver disease, even after transplantation, have delay in motor development, as well as changes in their nutritional and autonomic dysfunction.

Introduction: Stroke incidence in young patients is about 10 cases in 100.000, according to several European studies. In this age group arterial dissection is one of the main pathological mechanisms involved. The internal carotid’s artery (ICA) main supraclinoid branch is the anterior choroidal artery (AChA). The occurrence of infarction in its territory due to internal carotid dissection is considered to be a rare event and may have different clinical presentations due to anatomical variability. Clinical case: A 31-year-old male patient, without any known cardiovascular risk factors or chronic medication, presented with acute onset of stabbing right sided headache while practicing football. Visual disturbances and hemiparesis with hypesthesia of his left arm were also mentioned. On admission left homonymous hemianopsia, left hemiparesis and left extensor plantar reflex were present. Brain magnetic resonance showed hyperintensity of T2 and FLAIR signals and restricted diffusion pattern suggested acute/subacute infarctions in the thalamic and subcapsular area, corpus callosum, splenium and subcortical parietal right region. Magnetic resonance angiography (MRA) of the brain showed reduction of the right ICA’s caliber, mainly of its supraclinoid segment in which a marked irregular stenosis was visualized, suggestive of arterial dissection. This stenotic segment included the origin of the AChA and of the posterior communicating cerebral artery with an exchange in their territories. Lumbar puncture results were normal as were analytical investigations which included CBC, sedimentation rate, syphilis serology and immunologic and prothrombotic screen. There were no phenotype characteristics suggestive of connective tissue disease. Conclusion: Trauma seems to be the most probable lesion mechanism for the occurrence of intracranial carotid’s dissection in this particular case, as the patient was practicing vigorous sports at time of onset. COVID-19: Feelings Experienced by Nurses, Physiotherapists, and Medical Doctors in a Brazilian Field Hospital

Facing critical situations such as the ones triggered by COVID-19 may take healthcare professionals to confront their psychological resources, which may induce a higher stress level. We aimed to understand the feelings experienced by nurses, physiotherapists, and medical doctors in a field hospital dedicated to patients with COVID-19 in the biggest city of Brazil. Methods: We performed a qualitative study in a state field hospital in S?o Paulo city, Brazil, in a sample of 25 volunteer healthcare professionals. Participants answered to a sociodemographic electronic survey and were subjected to an individual semi-structured on-site interview. Results: Participants were mostly female (68%), comprised eight nurses, eight physiotherapists, and nine medical doctors, had an average age of 36.8 years, and were professionally experienced. Speech analysis revealed two thematic categories (related to work and assistance in the pandemic) and 11 subcategories: adaptation to the scenario, stressing experience, psychological support, difficulties with workwear, safety with workwear, non-verbal communication, empathy, patient comfort, outstanding experiences, major challenges, and teamwork. Conclusion: Nurses, physiotherapists, and medical doctors’ experiences while working in the field hospital were permeated by both positive and negative feelings. Those feelings originated directly from the fear of the unknown when dealing with a new disease without well-established scientific evidence, the use of medical workwear for long periods, and communication and environmental difficulties. In contrast, positive feelings, related to teamwork and empathy, arose and overlapped with the difficulties.

The Pan American Health Organization(PAHO)defines congenital malformation as any functional or structural anomaly in the development of the fetus,due to factors originating before birth,whether genetic,environmental or *** aim of the present study was to identify the frequency of malformations,the type of congenital malformations and to correlate this with risk factors in live-born infants,using SLB *** data were collected through consultation of the medical records of live-born infants of the Balbina Mestrinho Maternity Hospital,15,621 live births were reported,of which 248(1.58%)presented congenital *** was a higher prevalence of malformations among live-born males(49.7%),with Apgar≥7 at the first and fifth minute,gestational age ranging from 37-41 weeks,with 46%being born with appropriate weight between 3,000-4,000 *** association of two or more defects was observed in 38.7%of the total cases and isolated anomalies in 67.3%,with predominance of alterations of the digestive system(26.3%),followed by malformations of the musculoskeletal system(21.2%),nervous system(20.2%)and cleft lip/cleft palate(9.1%).The results presented here may guide strategic actions to improve care for families of people with congenital malformations.
