Ten golden rules for individualized mechanical ventilation in acute respiratory distress syndrome

Considerable progress has been made over the last decades in the management of acute respiratory distress syndrome(ARDS).Mechanical ventilation(MV)remains the cornerstone of supportive therapy for ***-protective MV minimizes the risk of ventilator-induced lung injury(VILI)and improves *** parame-ters contribute to the risk of VILI and require careful setting including tidal volume(V_(T)),plateau pressure(P_(plat)),driving pressure(ΔP),positive end-expiratory pressure(PEEP),and respiratory *** of energy and mechanical power allows quantification of the relative contributions of various parameters(V_(T),P_(plat),ΔP,PEEP,respiratory rate,and airflow)for the individualization of MV *** use of neuromuscular blocking agents mainly in cases of severe ARDS can improve oxygenation and reduce asynchrony,although they are not known to confer a survival *** respiratory therapies such as prone positioning,inhaled nitric oxide,and extracorporeal support techniques may be adopted in specific ***,respiratory weaning protocols should also be *** on a review of recent clinical trials,we present 10 golden rules for individualized MV in ARDS management.

Three-dimensional(3D)printing is a rapidly evolving and promising field to improve outcomes of orthopaedic *** use of patient-specific 3D-printed models is specifically interesting in paediatric orthopaedic surgery,as limb deformity corrections often require an individual 3D *** this editorial,various operative applications of 3D printing in paediatric orthopaedic surgery are *** technical aspects and the imaging acquisition with computed tomography and magnetic resonance imaging are ***,there is a focus on the intraoperative applications of 3D printing during paediatric orthopaedic surgical *** overview of various upper and lower limb deformities in paediatrics is given,in which 3D printing is already implemented,including posttraumatic forearm corrections and proximal femoral *** use of patient-specific instrumentation(PSI)or guiding templates during the surgical procedure shows to be promising in reducing operation time,intraoperative haemorrhage and radiation ***,3D-printed models for the use of PSI or patient-specific navigation templates are promising in improving the accuracy of complex limb deformity surgery in ***,the future of 3D printing in paediatric orthopaedics extends beyond the intraoperative applications;various other medical applications include 3D casting and prosthetic limb *** conclusion,3D printing opportunities are numerous,and the fast developments are exciting,but more evidence is required to prove its superiority over conventional paediatric orthopaedic surgery.

Africa is the cradle of all human beings, and although it has been the focus of a number of genetic studies, there are many questions that remain unresolved. We have performed one of the largest and most comprehensive meta-analyses of mitochondrial DNA （mtDNA） lineages carried out in the African continent to date. We generated high-throughput mtDNA single nucleotide polymorphism （SNP） data （230 SNPs） from 2024 Africans, where more than 500 of them were additionally genotyped for the control region. These data were analyzed together with over 12,700 control region profiles collected from the literature, representing more than 300 population samples from Africa. Insights into the African homeland of humans are discussed. Phylogeographic patterns for the African continent are shown at a high phylogeographic resolution as well as at the population and regional levels. The deepest branch of the mtDNA tree, haplogroup L0, shows the highest sub-haplogroup diversity in Southeast and East Africa, suggesting this region as the homeland for modem humans. Several demographic estimates point to the coast as a facilitator of human migration in Africa, but the data indicate complex patterns, perhaps mirroring the effect of recent continental-scaled demographic events in re-shaping African mtDNA variability.

Embedding Knowledge Management in Public Sector Procurement： Redesigning for the Knowledge Economy

There is a perception in the public, political, and trade discourse that private sector procurement performs ＂better＂ than does public sector procurement. This research considers whether this perception is justified. This paper proposes a conceptual framework for assessing issues that influence procurement performance. The framework takes into account the organization＇s business goals, its procurement principles, the design of its procurement capability, the intellectual capital or knowledge that is used to support procurement operations, and the use of knowledge management methods. To represent these factors, the framework adapts the conceptual framework proposed by McElroy （2002）, leverages Andriessen＇s （2005） characterization of intellectual capital, and adopts Bedford＇s （2012） description of the practice of knowledge management. The results, though preliminary and exploratory, suggest that factors which are more often practiced in the private sector than the public sector contribute to higher performance.

Expanding the Definition and Measurement of Knowledge Economy： Integrating Triple Bottom Line Factors into Knowledge Economy Index Models and Methodologies

The application of knowledge is a primary source of growth in the knowledge economy. The World Bank Group has developed a rigorous assessment methodology for assessing a country＇s ability to access and use knowledge to become more competitive in the knowledge economy of the 21st century. The World Bank＇s annual knowledge economy index is grounded on a four-pillar model：（1） economic incentives and institutional regime; （2） education and skills; （3） information and communication infrastructure; and （4） innovation systems. An argument can be made that the model lacks coverage of some key factors that pertain to intellectual capital and the production and consumption of knowledge. The model＇s heavy focus on economic incentives and open institutional regimes comes at a societal cost. This paper proposes an alternative knowledge economy index which is grounded in a more holistic and balanced view of a knowledge society. Adopting the perspective of triple bottom line shifts the purpose and design of a knowledge economy from one of aggregation and reporting to action and involvement. The World Bank＇s scorecard and indexing methodology are adaptable to this new perspective and a new set of indicators.

BACKGROUND Colitis-associated cancer(CAC)accounts for 2%-3%of colorectal cancer(CRC)cases preceded by inflammatory bowel diseases(IBD)such as Crohn's disease and ulcerative *** microbiota has been reported to play a central role in the pathogenesis of IBD and ***,numerous prebiotics and probiotics have being investigated as antitumor agents due to their capacity to modulate inflammatory *** studies have indicated that lactic acid bacteria could be successfully used in managing sporadic CRC,however little is known about their role in *** To investigate the effect of the probiotic Lactobacillus bulgaricus(***)during the development of an experimental model of colitis associated colon cancer(CAC).METHODS C57BL/6 mice received an intraperitoneal injection of azoxymethane(10 mg/kg),followed by three cycles of sodium dextran sulphate diluted in water(5%w/v).Probiotic group received daily *** inflammation was determined by scoring clinical *** levels were determined from colon and/or tumor samples by ELISA BD OptEIATM *** level of significance was set at P<*** were generated and statistical analysis performed using the software GraphPad Prism *** *** treatment inhibited of total tumor volume and mean size of *** addition,the probiotic also attenuated the clinical signs of intestinal inflammation inducing a decrease in intestinal and tumor levels of IL-6,TNF-α,IL-17,IL-23 and IL-1β.CONCLUSION Our results suggest a potential chemopreventive effect of probiotic on *** regulates the inflammatory response and preventing CAC.
