Evaluation of the Strategy and Efficacy of Treatment of Chronic Viral Hepatitis C with the Sofosbuvir/Daclatasvir Combination in a Resource-limited Country

Introduction: The treatment of viral hepatitis C (HCV), a major public health problem, has evolved considerably since the introduction of direct-acting anti-virals (DAAs). The aim of this study was to evaluate the strategy for initiating treatment with Sofosbuvir/Daclatasvir, and also to assess its efficacy. Patients and Methods: Included were patients aged at least 15 years, with detectable hepatitis C viremia and treated with a pan-genotypic Sofosbuvir/Daclatasvir regimen at the Centre “Hospitalier Universitaire la Référence Nationale de N’Djamena” between October 2019 and October 2023. The APRI score was used to assess fibrosis. Cure was defined by an undetectable viral load 12 weeks after the end of treatment. Results: A total of 835 patients were included (sex ratio 1.55, mean age 50.5 ± 13.73, extremes 20 and 84 years). The 60 - 69 age group was the most represented. The mean APRI score was 0.42 ± 0.29, with extremes of 0.019 and 1.84. The mean viral load was 2,316,193.086 IU/ml. All subjects had good renal function. Cytolysis was absent in 70% of cases. Without genotyping, a pan-genotypic regimen was used in all patients: Sofosbuvir (400 mg)/Daclatasvir (60 mg). The cure rate was 99.4%, with good tolerability. However, four cases of failure were recorded out of the 668 patients who underwent follow-up HCV-PCR testing after 12 weeks of treatment. Conclusion: The Sofosbuvir/Daclatasvir regimen is highly effective, with an excellent safety profile. However, the still high cost of these molecules limits their accessibility to a large proportion of patients.

Characterization and Assessment of the Quality of the Water and Sediments of the Islands (Moudou and Birguime) of lake Fitri in Chad and Confirmation of the Strains Isolated by the PCR Method

The problem of access to quality water is a major challenge, as it has a major impact on the socio-economic conditions of people in developing countries. The water from the islands of lake Fitri is one of the main sources of drinking water for the population. The aim of this study is to characterise and assess the quality of the water and sediment from the islands (Moudou and Birguime) of lake Fitri, and to confirm the strains isolated using the PCR method. A total of fifty (50) samples of water and fifty (50) sediments from the islands of lake Fitri were analyzed. Standard methods of microbiological and biochemical analysis of water and sediments were used. Isolated Enterobacteriaceae strains were characterized by API 20 E and API Staph galleries and Salmonella was confirmed by PCR method. Antibiotic resistance was determined using a technique recommended by the antibiogram committee of the French microbiology society (CA-SFM, 2019). The microbiological results for the water showed an abundance of total aerobic mesophilic flora (TAMF) (4.31 × 106 ± 8.05 × 105 and 5.29 × 106 ± 2.55 × 105) on the Birguime and Moudou islands successively. The microbiological results for the sediment from Birguime and Moudou islands showed an abundance of thermotolerant coliforms (E. coli) (2.05 × 105 ± 5.43 × 104 and 2.27 × 105 ± 3.49 × 104) alternately. The results obtained after incubation of the biochemical tests by the API 20E, API Staph gallery and their numerical profile proposed by the Api software confirmed the contamination. The antibiogram results showed the emergence of certain resistances to the antibiotics Tobramycin, Flucytosine, Amikacin and Teicoplani. The PCR results for Salmonella spp strains were confirmed. As a result, strict monitoring of the water on the various islands of lake Fitri must be carried out throughout the annual cycle, by specialized personnel, to ensure proper bio-monitoring of these ecosystems.

Projected Changes in the Climate Zoning of Côte d’Ivoire

This study assesses the projected changes in the climate zoning of Côte d’Ivoire using the hierarchical classification of principal components (HCPC) method applied to the daily precipitation data of an ensemble of 14 CORDEX-AFRICA simulations under RCP4.5 and RCP8.5 scenarios. The results indicate the existence of three climate zones in Côte d’Ivoire (the coastal, the centre and the north) over the historical period (1981-2005). Moreover, CORDEX simulations project an extension of the surface area of drier climatic zones while a reduction of wetter zones, associated with the appearance of an intermediate climate zone with surface area varying from 77,560 km2 to 134,960 km2 depending on the period and the scenario. These results highlight the potential impacts of climate change on the delimitation of the climate zones of Côte d’Ivoire under the greenhouse gas emission scenarios. Thus, there is a reduction in the surface areas suitable for the production of cash crops such as cocoa and coffee. This could hinder the country’s economy and development, mainly based on these cash crops.

Ethnobotanical and Phytochemical Study of Medicinal Plants Sold in the Markets of the City of N’Djamena

The use of medicinal plants occupies an important place in traditional Chadian medicine, this study was carried out with the aim of listing the medicinal plants sold in the markets of the city of N’Djamena, and knowledge of the medicinal flora Chad, such as the leaves of Guiera senegalensis in Arabic called Khibeche, the roots of Cassia occidentalis called Am kwala-Kinkéliba in Arabic;the bark of Khaya senegalensis (Desv) A. Juss called Muraї in Arabic and the seeds of Trigonella foenum-graecum called Helbé in Arabic in order to treat the most frequently cited diseases such as malaria, the common cold, mother and newborn care, and typhoid. Ethnobotanical surveys were conducted from September to December 2022, in three markets in the city of N’Djamena, based on individual interviews using a semi-structured questionnaire, with 30 herbalists and traditional practitioners. Phytochemical screening analyses of the organs of four plants were carried out in the chemistry and biology laboratory of the Faculty of Exact and Applied Sciences (FSEA) following a standard protocol established by [1] and slightly modified by [2] [3]. The results of the surveys identified 68 species divided into fifty-five (55) genera and twenty-seven (27) families, the most represented of which were Caesalpiniaceae (10 species), Mimosaceae (5 species) and Combretaceae (4 species). The leaves were the most commonly used parts (43%), with decoction (70%) the most frequently used method of preparation. The oral route was the most popular with 73% of recipes. The phytochemical study revealed a wealth of secondary metabolites such as Trapezoidal Sterols, Alkaloids, Tannins, Flavonoids, Free Quinones, Anthocyane, Anthraquinones, Saponosides and Heterosides. These results can be considered as a source of information for scientific research in the field of phytochemistry and pharmacology.

Interest of Transcranial Doppler (TCD) in Emergency in Traumatic Brain Injury Patients at Gabriel Touré University Hospital, Bamako, Mali

Introduction: Transcranial Doppler is a simple, non-invasive and inexpensive examination which allows the assessment of cerebral perfusion. In countries with limited resources, which do not have a means of monitoring intracranial pressure, this examination offers hope of survival for patients with traumatic brain injury. This study was designed to investigate the incidence of early cerebral circulation abnormalities after traumatic brain injury using transcranial Doppler (TCD). Methodology: A descriptive and analytical study was conducted over one year, including patients with traumatic brain injury and an initial Glasgow Coma Scale (GCS) score of less than 15. Non-inclusion criteria: Stroke, brain tumor, cerebral abscess. Exclusion criteria: Inadequate insonation window. Results: Out of 854 traumatic brain injury patients admitted to the emergency department, 112 were included in the study. The average age was 30.14 years, with a sex ratio of 4.1. Initially, 83.5% had moderate traumatic brain injury, and 12.1% had severe traumatic brain injury. Brain CT scans were performed in 95.7% of the patients. Edematous-hemorrhagic contusion was observed in 95% of the patients. On initial TCD, the pulsatility index in the middle cerebral artery was greater than 1.3 in 49.4% of the patients. Diastolic velocity was less than 20 cm/s in 46.4% of cases, and mean velocity was greater than 150 cm/s in 8.7% of cases. In this group, low diastolic velocity indicated cerebral hypoperfusion suggestive of intracranial hypertension. When the pulsatility index was greater than 1.9, no patient had a normal diastolic velocity. Among patients with severe traumatic brain injury, 61.5% had an abnormal pulsatility index compared to 42.3% of patients with moderate traumatic brain injury. Conclusion: TCD is a simple tool for analyzing intracerebral hemodynamics.

Prevalence and Factors Associated with Violence among Primary and Secondary School Workers in Ouagadougou, Burkina Faso

Introduction: Violence is a major problem in today’s workplace. It affects workers in all sectors of activity. The aim of the study was to determine the prevalence and factors associated with violence among primary and secondary school staff in Ouagadougou. Materials and Methods: This was a cross-sectional, descriptive and analytical study that took place from April 22, 2020 to February 18, 2021 in primary and secondary schools in Ouagadougou. It focused on the educational staff of these establishments. Results: A total of 362 agents took part in the study. The average age of participants was 36.5 ± 6.89 years. The sex ratio was 1.62 men to one woman. The majority of workers were teachers, with 300 participants (82.87%). The average job tenure was 7 years ± 6.91 years. With regard to violence in the workplace, 87 workers were victims, representing a prevalence of 24.03%. Of these workers, 59 (67.81%) were men and 36 (41.38%) had less than six years’ experience. Acts of violence were predominantly pushing observed in 15 workers (50%), while shouting and aggressive tone dominated psychological violence and concerned 54 cases (36.24%). On univariate analysis, the factors associated with violence were age (p = 0.046) and type of establishment (p = 0.012). On multivariate analysis, only the nature of the establishment was the only factor associated with the occurrence of violence among teaching staff (OR = 2.3690, p = 0.0213). Conclusion: The prevalence of violence is high in these educational establishments. Age and type of establishment are factors associated with its occurrence. It is therefore necessary to develop an effective strategy for preventing this risk in the workplace.

Ultrasound in Ti-Rads Classification of Thyroid Nodules at the Marie Curie Medical Clinic

Introduction: A thyroid nodule is a localized hypertrophy within the thyroid parenchyma. The aim of our study was to study the benefit of ultrasound in the Ti-rads classification of thyroid nodules. Methodology: This was a prospective study with a descriptive aim, with prospective collection, which took place over a period of 17 months at the “Marie Curie” medical clinic. The ultrasound machine used was a Voluson E8 from 2011 and the examinations were carried out by two radiologists and two experienced sonographers. The parameters studied were sociodemographic data;clinical data and ultrasound aspects of the Ti-rads classification in the management of nodules. Results: We collected 235 patients out of 738 patients referred to the service for a cervical ultrasound, i.e. a frequency of 31.84% of cases. There was a female predominance with 95.7% of cases and a sex ratio of 0.04. The average age of our patients was 50 years. We found on cervical ultrasound: Isthmo-lobar glandular hyperplasia in 99 patients, i.e. a frequency of 42.1%. The Ti-rads 3 classification was the most represented in 69.4% of cases. The benignity criterion represented 85.6% of cases in our patients and the malignancy criterion represented 14.4% of cases. Conclusion: The precise description of a thyroid nodule provided by ultrasound (Ti-rads) is essential in the management of nodules.

lawsonia inermis is a hairless plant growing in various regions of North Africa, the Indian subcontinent, and the Middle East. It possesses many medicinal attributes, including curative properties against infectious dermatoses. This study was carried out to evaluate the phytochemical profile of the crude ethanolic extract of the plant leaves and its fractions as well as their antimicrobial activities. The phytochemical profile was performed using high-performance thin-layer chromatography (HPTlC), gas chromatography-mass spectrometry (GC-MS), and high-performance liquid chromatography (HPlC). Additionally, the phenolic and flavonoid contents were determined using the Folin-Ciocalteu spectrophotometric and the aluminum trichloride methods. Antimicrobial activity was tested using disc diffusion and microdilution methods. The presence of flavonoids, tannins, sterols, and triterpenes was revealed. GC-MS detected twelve compounds main compounds consisting of saturated and unsaturated fatty acids and phenolic and terpenoid compounds among twenty-seven components. HPlC also detected high contents of phenolic acids and flavonoids. The most abundant triterpene and sterols were ursolic acid (around 43.14 g/100g DW, 13.9 g/100g dry weight (DW), and 0.68 g/100g DW) in the crude ethanolic extract of leaves (Feli), hexane fraction (FHli) and dichloromethane fraction (FDli), respectively and, β-sitosterol in Feli (56.7 mg/100g DW), FHli (10.55 g/100g DW), FDli (106.1 mg/100g DW) and butanol fraction (FBli) (357.4 mg/100g DW). Among the flavonoids, rutin = 3.24 g/100g and quercetin = 0.63 g/100g in the ethanolic extract, rutin = 15.73 g/100g in the dichloromethane fraction, and rutin = 0.23 g/100g) in the aqueous fraction;and among phenolic compounds, caffeic acid (37.65 g/100g DW) and vanillic acid (22.70 g/100g DW) were the most important in the ethyl acetate fraction (FAeli). Background: The World Health Organization (WHO) has set a goal to eradicate or at least significantly reduce the prevalence the human immunodeficiency virus (HIV), hepatitis B Virus (HBV) and Hepatitis C Virus (HCV) by 2030. The main objective was to provide an evolving overview of the prevalence of HIV, HBV and HCV infection between 2003 and 2022 in Burkina Faso. Methods: It was a retrospective cross-sectional study based on data from 2003 to 2022. The data were collected using information available in the databases of the HOSCO and CERBA laboratories and included all individuals who underwent HIV and/or HBV and/or HCV testing. Data analysis was performed using SPSS version 20.0, EpiInfo 7, and R version 4.1.0. Results were considered statistically significant if p Results: The study recorded 7432 samples and the mean age of the subjects was 27.98 ± 8.50 years. During this period, the respective prevalence of HIV, HBV, and HCV were 4.66% (346/7432), 8.77% (582/6636) and 5.54% (322/5816). However, from 2003 to 2022, there was a significant decrease (P y=−1.75x+12.59;y=−0.24x+10.01and y=−0.11x+6.02, with “y” corresponding to prevalence and “x” to the years. Conclusion: Burkina Faso needs to rigorously apply prevention and control strategies recommended by the WHO by 2030.

Background: Since 2012, the World Health Organization has recommended intermittent preventive treatment with sulfadoxine-pyrimethamine (IPT-SP) to prevent malaria-related complications in pregnant women. Ten years following these recommendations, we conducted this study to estimate the coverage for three doses of IPT-SP (IPT3) as well as the prevalence of low birth weight (lBW), and its associated factors in lomé (Togo) in 2021. Methods: A cross-sectional study was conducted between January and March 2021. An exhaustive recruitment of women and their newborns was carried out in the maternity wards of the Sylvanus Olympio University Hospital Center. Data from antenatal consultations and clinical data of the newborns were collected. Multivariate logistic regression was carried out to determine factors associated with lBW. Results: A total of 252 mother-child pairs were included in this study. Median age of the mothers was 27 years, IQR [24-31]. More than a third (35.3%) of the mothers were primigravida. IPT3 coverage was 66.7% and 14.7% of newborns had a lBW. The prevalence of lBW was 33.3% [23.3-43.4] in women who had received fewer than 3 doses of IPT-SP and 5.4% [2.0-8.8] in those who had received at least 3 doses of IPT-SP (p Conclusion: Ten years following recommendations of the WHO on IPT-SP, malaria prevention based on IPT-SP is not optimal among pregnant women in lomé, and the proportion of lBW children remains high. Actions to strengthen the three-dose IPT-SP policy are needed to prevent malaria and its consequences among newborns in Togo.
