Extrahepatic metastasis of hepatocellular carcinoma to the paravertebral muscle:A case report

Identification of extrahepatic metastases(EHM) of hepatocellular carcinoma(HCC) has been paradoxically increasing due to an increase in the survival of HCC patients. However, metastasis of HCC to the skeletal muscle tissue is extremely rare. We describe a unique case of HCC metastasizing to the paravertebral muscle. A 55-year-old man with a history of hepatitis B cirrhosis underwent partial liver resection with complete removal of HCC. Three months later, a computed tomography(CT) scan showed intrahepatic recurrence. The tumors were treated with yttrium-90 microspheres, transcatheter arterial chemoembolization, and sorafenib. Six months later, a CT scan showed an enhancing lesion of the left paravertebral muscle that on biopsy were consistent with metastatic HCC. The tumor was treated with stereotactic hypo-fractionated imageguided radiation therapy(SHFRT). A follow-up scan 3 mo post-radiotherapy revealed a stable appearance of the paravertebral muscle metastasis. Because of the progression in the intrahepatic tumors, the patient was treated with capecitabine, which was changed to dasatinib 6 mo later. The patient passed away three years after the primary surgical resection. Management of EHM poses an extreme challenge. This is the first case of HCC with EHM to the paravertebral muscle in which stability of disease was achieved using SHFRT. This case highlights the importance of early detection of hepatitis B viral infection and initiation of anti-viral therapy to decrease recurrence of HCC and prevent EHM.

Allograft loss from acute Page kidney secondary to trauma after kidney transplantation

We report a rare case of allograft loss from acute Page kidney secondary to trauma that occurred 12 years after kidney transplantation. A 67-year-old Caucasian male with a past surgical history of kidney transplant presented to the emergency department at a local hospital with left lower abdominal tenderness. He recalled that his cat, which weighs 15 lbs, jumped on his abdomen 7 d prior. On physical examination, a small tender mass was noticed at the incisional site of the kidney transplant. He was producing a normal amount of urine without hematuria. His serum creatinine level was slightly elevated from his baseline. Computer tomography revealed a large subscapular hematoma around the transplant kidney. The patient was observed to have renal trauma grade Ⅱ at the hospital over a period of three days, and he was finally transferred to a transplant center after his urine output significantly decreased. Doppler ultrasound demonstrated an extensive peri-allograft hypoechoic area and abnormal waveforms with absent arterial diastolic flow and a patent renal vein. Despite surgical decompression, the allograft failed to respond appropriately due to the delay in surgical intervention. This is the third reported case of allograft loss from acute Page kidney following kidney transplantation. This case reinforces that kidney care differs if the kidney is solitary or a transplant. Early recognition and aggressive treatments are mandatory, especially in a case with Doppler signs that are suggestive of compression.

Household air pollution and lung cancer in China:a review of studies in Xuanwei

Over half of the world's population is exposed to household air pollution from the burning of solid fuels at home. Household air pollution from solid fuel use is a leading risk factor for global disease and remains a major public health problem, especially in low- and mid-income countries. This is a particularly serious problem in China, where many people in rural areas still use coal for household heating and cooking. This review focuses on several decades of research carried out in Xuanwei County, yunnan Province, where household coal use is a major source of household air pollution and where studies have linked household air pollution exposure to high rates of lung cancer. We conducted a series of case-control and cohort studies in Xuanwei to characterize the lung cancer risk in this population and the factors associated with it. We found lung cancer risk to vary substantially between different coal types, with a higher risk associated with smoky(i.e., bituminous) coal use compared to smokeless(i.e., anthracite) coal use. The installation of a chimney in homes resulted in a substantial reduction in lung cancer incidence and mortality. Overall, our research underscores the need among existing coal users to improve ventilation, use the least toxic fuel, and eventually move toward the use of cleaner fuels, such as gas and electricity.

Advancement in materials for energy-saving lighting devices

This review provides a comprehensive account of energy efficient lighting devices, their working principles and the advancement of these materials as an underpinning to the development of technology. Particular attention has been given to solid state lighting devices and their applications since they have attracted the most interest and are the most promising. Solid state lighting devices including white light emitting diodes （LEDs）, organic LEDs （OLEDs）, quantum-dot LEDs （QLEDs） and carbon-dot LEDs （CLEDs） are promising energy efficient lighting sources for displays and general lighting. However there is no universal solution that will give better performance and efficiency for all types of applications. LEDs are replacing traditional lamp,; for both general lighting and display applications, whereas OLEDs are finding their own special application,; in various areas. QLEDs and CLEDs have advantages such as high quantum yields, narrow emission spectra, tunable emission spectra and good stability over OLEDs, so applications for these devices are being extended to new types of lighting sources. There is a great dean of research on these materials and their processing technologies and the commercial viability of these technologies appears strong.

Heifer Body Weight Gain and Reproductive Achievement in Response to Protein and Energy Supplementation While Grazing Dormant Range Forage

Heifers grazing winter range require supplemental nutrients to complement dormant forage to achieve optimal growth and performance. A study was conducted to evaluate nutritional environment and effect of different supplementation strategies for developing heifers grazing dormant winter range. Eighty-four Angus crossbred heifers were stratified by body weight at weaning, allocated to one of six replicated pastures, and randomly assigned one of three supplemental treatments: 1) 908 g/d of a control supplement providing 340 g·hd-1·d-1 of CP with 130 g of rumen undegradable protein (RUP) and 614 MJ of ME (LRUP);2) 908 g/d of a RUP supplement providing 340 g·hd-1·d-1 of CP with 170 g of RUP and 567 MJ of ME (HRUP);or 3) 1814 g/d of a protein and energy supplement providing 340 g·hd-1·d-1 of CP with 120 g of RUP + 100 g of propionate salt (NutroCalTM, Kemin Industries, Inc.) and 1222 MJ of ME (LRUP + E). Body weights were taken in November, with monthly 12 h shrunk BW from January thru April, and again in September (at time of pregnancy diagnosis). Heifer average daily gain was similar throughout the developmental period except from d125 to d159 where LRUP + E supplemented heifers had greater gains

Pathological findings following radical prostatectomy in patients who are candidates for active surveillance： impact of varying PSA levels

Active surveillance is an acceptable treatment option in men with a low-risk prostate cancer. In the present study, we have retrospectively reviewed the outcomes of 509 men who fit the criteria for active surveillance but selected radical prostatectomy. Then, the impact of varying prostate-specific antigen （PSA） levels on the risk of upstaging and upgrading in these patients was assessed. Pathological characteristics of patients who fulfilled the inclusion criteria under three active surveillance criteria--those of the University of California-San Francisco, the National Cancer Institute and the European Association of Urology--were examined. The proportion of men who were deemed candidates for active surveillance but were subsequently upstaged or upgraded was determined. Of 509 patients, 186 （36.5%）, 132 （25.9%） and 88 （17.3%） men fulfilled the active surveillance criteria, respectively. Upgrading （Gleason scores 7-10） ranged from 32.8% to 38.6%, while upstaging （≥ pT3） ranged from 10.2% to 12.5%, depending on the three active surveillance criteria. After a median follow-up of 24 months, three patients developed a biochemical recurrence. When the impact of varying PSA levels was examined using a test for trend analysis in the context of PSA for each protocol, rates of upstaging were lower in men with PSA 〈4 ng m1-1. However, there was no impact of varying PSA levels on upgrading. In conclusion, commonly used active surveillance protocols carry the risks of upgrading and upstaging. More reliable and accurate markers are needed to better stratify the risks of men who are appropriate candidates for active surveillance.

Mental health conditions in children and adolescents can be improved by slow mindful nature connection known as forest therapyor *** therapy has recently received growing attention as an enabler of relaxation and preventive health care withdemonstrated clinical ***,it is not well-known that forest therapy also decreases mental health issues amongindividuals with mental health *** study explored the psychological and physiological health benefits of structuredforest therapy programs for children and adolescents with mental health disorders.A one-group pre-test-posttest design wasemployed for our study *** participants(aged 9–14 years)engaged in two one-hour guided standard sequenceforest therapy experiences.A Mindful Attention Awareness Scale(MAAS),Connectedness to Nature Scale(CNS),Profile ofMood States(POMS),place meanings(e.g.,functional,emotional,and cognitive attachment to the forest)questionnaire,andphysiological health assessment were administered to the *** results showed that negative mood states weresignificantly reduced and that a positive mood state was significantly improved after the structured forest therapy ***,mindfulness,nature connection,place meanings,and physiological health were significantly boosted after *** results demonstrate substantial psychological and physiological health and well-being outcomes ofstructured forest therapy for similar individuals.
