The experimental data concerning the58Ni+48Ca reaction at Elab(Ni)=25A Mev,collected by using the CHIMERA 4π device,have been analyzed in order to investigate the competition among different reaction mechanisms for central collisions in the Fermi energy *** a main criterion for centrality selection we have chosen the flow angle(flow) method,making an event-by-event analysis that considers the shape of events,as it is determined by the eigenvectors of the experimental kinetic-energy *** the selected central events(flow >60°) some global variables,good to characterize the pattern of central collisions have been *** main features of the reaction products were explored by using different constraints on some of the relevant observables,like mass and velocity distributions and their *** emphasis was devoted,for central collisions,to the competition between fusion-evaporation processes with subsequent identification of a heavy residue and a possible multifragmentation mechanism of a well defined(if any) transient nuclear *** evolution of the system and pre-equilibrium emission were taken into account by simulating the reactions in the framework of transport *** approaches have been envisaged(dynamical stochastic BNv calculations + sequential SIMON code,QMD,CoMD,etc.).Preliminary comparison of the experimental data with BNv calculations shows reasonable agreement with the assumption of sequential multifragmentation emission in the mass region of IMFs close to the heavy *** deviations from sequential processes were found for those IMFs in the region of masses intermediate between the mass of heavy residues and the mass of light *** simulations are in *** experimental analysis will be enriched also by information obtained inspecting the IMF-IMF correlation function,in order to elucidated the nature of space-time decay property of the emitting source associated with events having

Managements of recurrent hepatocellular carcinoma after liver transplantation: A systematic review

AIM: To investigate the efficacy(survival) and safety of treatments for recurrent hepatocellular carcinoma(HCC) in liver transplantation(LT) ***: Literature search was performed on available online databases without a time limit until January 2015. Clinical studies describing survival after HCC recurrence in LT patients were retrieved for a fulltext evaluation. A total of 61 studies were selected: 13 case reports, 41 retrospective case series, and 7 retrospective comparative ***: Based on all included studies, the mean HCC recurrence rate was 16% of all LTs for HCC. A total of 1021 LT patients experienced HCC recurrence. The median time from LT to HCC recurrence was 13 mo(range 2-132 mo). The majority of patients(67%) presented with HCC extra-hepatic recurrences, involving lung, bone, adrenal gland, peritoneal lymph nodes, and rarely the brain. Overall survival after HCC recurrence was 12.97 mo. Surgical resection of localized HCC recurrence and Sorafenib for controlling systemic spread of HCC recurrence were associated with the higher survival rates(42 and 18 mo, res-pectively). However, Sorafenib, especially when combined with m TOR, was frequently associated with severe side effects that required dose reduction or discontinuation CONCLUSION: Management of recurrent HCC in LT patients is challenging and associated with poor prognosis independently of the type of treatment.

Fatigue strength of Al7075 notched plates based on the local SED averaged over a control volume

When pointed v-notches weaken structural components, local stresses are singular and their intensities are expressed in terms of the notch stress intensity factors （NSIFs）. These parameters have been widely used for fatigue assessments of welded struc- tures under high cycle fatigue and sharp notches in plates made of brittle materials subjected to static loading. Fine meshes are required to capture the asymptotic stress distributions ahead of the notch tip and evaluate the relevant NSIFs. On the other hand, when the aim is to determine the local Strain Energy density （SED） averaged in a control volume embracing the point of stress singularity, refined meshes are, not at all, necessary. The SED can be evaluated from nodal displacements and regular coarse meshes provide accurate values for the averaged local SED. In the present contribution, the link between the SED and the NSIFs is discussed by considering some typical welded joints and sharp v-notches. The procedure based on the SED has been also proofed to be useful for determining theoretical stress concentration factors of blunt notches and holes. In the second part of this work an application of the strain energy density to the fatigue assessment of A17075 notched plates is presented. The experimental data are taken from the recent literature and refer to notched specimens subjected to different shot peening treatments aimed to increase the notch fatigue strength with respect to the parent material.

Endoscopic ablation of Barrett's esophagus using high power setting argon plasma coagulation: A prospective study

AIM: This prospective study evaluated the effectiveness of 90 W argon plasma coagulation (APC) for the ablation of Barrett's esophagus (BE) that is considered to be the main risk factor for the development of esophageal ***: The results from 25 patients, observed at the First department of General Surgery, University of verona, Italy, from October 2000 to October 2003, who underwent APC for histologically proven BE were prospectively ***: The ablation treatment was completed in all the patients but one (96%). The mean number of APC sessions needed to complete ablation was 1.6 (total number: 40). The eradication was obtained in the majority of cases by one session only (60%), two sessions were required in 24% of the cases and three or more in 16%.About 43% of the sessions were complicated. Retrosternal pain (22.5%) and fever (17.5%) were the most frequent symptoms. Only one major complication occurred, it was an hemorrhage due to ulcer formation on the treated esophagus that required urgent endoscopic sclerosis and admission. The follow-up was accomplished in all the patients with a mean period of 26.3 mo and 20 patients (84%) with a follow-up period longer than 24 mo. Only one patient showed a relapse of metaplastic mucosa 12 mo after the completion of ablation. The patient was hence re-treated and now is free from recurrence 33 mo ***: High power setting (90 W) APC showed to be safe and effective. The effects persist at a mean follow-up period of two years with a comparable cost in term of complications with respect to standard power settings. Further studies with greater number of patients are required to confirm these results and to assess if ablation reduces the incidence of malignant progression.

Role of MGMT as biomarker in colorectal cancer

O6-methylguanine DNA methyltransferase(MGMT) gene promoter methylation plays an important role in colorectal carcinogenesis, occurring in about 30%-40% of metastatic colorectal cancer. Its prognostic role has not been defined yet, but loss of expression of MGMT, which is secondary to gene promoter methylation, results in an interesting high response to alkylating agents such as dacarbazine and temozolomide. In a phase 2 study on heavily pre-treated patients with MGMT methylated metastatic colorectal cancer, temozolomide achieved about 30% of disease control rate. Activating mutations of RAS or BRAF genes as well as mismatch repair deficiency may represent mechanisms of resistance to alkylating agents, but a dose-dense schedule of temozolomide may potentially restore sensitivity in RAS-mutant patients. Further development of temozolomide in MGMT methylated colorectal cancer includes investigation of synergic combinations with other agents such as fluoropyrimidines and research for additional biomarkers, in order to better define the role of temozolomide in the treatment of individual patients.

Experimental infection with Yersinia pseudotuberculosis in European brown hare(Lepus europaeus,Pallas)

Small bowel diverticulitis with severe anemia and abdominal pain

The current case report is related to a male patient with diabetes, obesity [body mass index(BMI) 33], hypertension and recurrence of anemia associated to melena and deep asthenia. M.P., a 60-year-old obese individual, was referred to our department by the primary care unit(PCU) of our hospital for severe anemia(Hemoglobin 6.5 g/d L) associated to episodes of melena and abdominal pain. In the past 5 mo the patient referred to the local hospital 3 times for episodes of melena(hemoglobin levels showed anemia 9.8 g/d L) but the main gastroenterological exams were completely negative(colonoscopy and gastroscopy). The PCU of our Hospital, after stabilization of the main parameters and blood transfusion for the low levels of hemoglobin, referred the patient to gastroenterologists: the patient was subjected to both colonoscopy and gastroscopy that were negative. Due to the condition of acute severe hemorrhage the patient, during the first 3 h from the access to the PCU, was subjected to arteriography that did not reveal any hemorrhagic foci or vascular alterations. The video capsule for the study of the small bowel showed the presence of blood beginning from the third portion of duodenum but deep gastroscopy did not reveal it. The patient was then subjected to double balloon endoscopy that revealed a severe diverticulosis of the small bowel with blood from the diverticula. The entero-tomografia computerizzata confirmed the diagnosis and revealed an extension of the diverticula for almost the entire small bowel(no diverticula in the colon). The patient was subjected to wide spectrum antibiotic therapy with resolution of the symptoms and stabilization of hemoglobin levels. The surgeon suggests no indication to surgery for the wide area involved from the disease and potential high risk of complication due to the high BMI. At home, the patient started a monthly therapy with rifaximin and probiotics associated to mesalazine. At present, after 12 mo from the last episode of hemo
