Does needle calibre affect pain and complication rates in patients undergoing transperineal prostate biopsy？ A prospective, randomized trial

Transperineal prostate biopsy is a procedure that can be used to obtain histological samples from the prostate. To improve both the quality of the biopsy core samples and prostate cancer detection, we are currently performing a prospective, randomized trial comparing prostate biopsy samples obtained using an 18 G-needle to those obtained using a 16 G-needle. The aim of this preliminary study was to evaluate pain and complication rates in both groups in order to assess whether performing a prostate biopsy with a larger calibre needle is a feasible procedure. One hundred and eighty-seven patients undergoing transperineal prostate biopsy were prospectively evaluated and divided into two groups. The first group （94 patients, Group A） received a transperineal prostate biopsy using a 16 G-needle and the second group （93 patients, Group B） underwent transperineal prostate biopsy with an 18 G-needle. Anaesthesia was obtained with a single perineal injection at the prostatic apex in all subjects. A visual analogue scale （VAS） and facial expression scale （FES） were used to assess pain during multiple steps of the procedure in each group. A detailed questionnaire was used to obtain information about drug use because it could potentially influence the pain and complications that patients experienced. Two weeks after the procedure, early and late complications were evaluated. Statistical analysis was carried out using non-parametric tests. Prostate Specific Antigen （PSA） and drug use were similar at baseline between the two groups. Pain during prostate biopsy, which was measured with both the VAS and FES instruments, did not differ significantly between the 18- and 16 G-needle groups, and no significant differences were found in early or late complication rates between the groups. Since the very early days of clinical liver transplantation(LT),transplant surgeons and professionals have been confronted with the increased risk of failure associated with the use of fatty liver grafts(1).Notwithstanding the wide variability in steatosis assessment across different centers and pathologists(2),utilization of livers with moderate(≥30%)or severe(≥60%)macrovesicular steatosis has been consistently associated with an increased risk of primary non-function,early allograft dysfunction,acute kidney injury,as well as inferior graft and patient survival(3).The mechanisms behind the increased susceptibility of steatotic livers to ischemia-reperfusion injury(IRI)are multiple,including disturbances to microcirculation due to sinusoidal narrowing,increased oxidative stress upon reperfusion and enhanced lipid peroxidation,leading to an increased release of inflammatory mediators like IL6,IL1βand so-called damage associated molecular patterns(DAMP),like cell-free DNA and mitochondrial ***,this is reflected by hepatocyte death by necrosis rather than apoptosis,pseudopeliotic steatosis(i.e.,the expulsion of lipid droplets into the extracellular space)and lately,by tissue remodelling and fibrosis(4,5).

Is it better to be safe than sorry?This Hamletic dilemma has always stimulated medical-scientific debates in numerous fields of *** among these,the preventive-therapeutic approach to the treatment of brain trauma is one of the most striking *** brain injury(TBI)is a leading cause of brain damage among young and elderly populations with a very high hospitalization and death *** is characterized by two pathologically distinct but strictly consequential phases:a first characterized by an immediate and highly variable mechanical dysfunction of the brain tissue,which involves widespread cell death and tissue degeneration,followed by a second phase which can last from days to even years depending on the severity of the TBI and the patient’s pre-existing health *** processes,including inflammatory phenomena,oxidative stress associated with metabolic,vascular,and neuro-modulatory deficits,are very often responsible for neuro-motor and psychological deficits leading to long-term disabilities(Kaur and Sharma,2018).

Carbon dioxide capture and reduction(CCR)process emerges as an efficient catalytic strategy for CO_(2)capture and conversion to valuable chemicals.K-promoted Cu/Al_(2)O_(3)catalysts exhibited promising CO_(2)capture efficiency and highly selective conversion to syngas(CO+H_(2)).The dynamic nature of the Cu-K system at reaction conditions complicates the identification of the catalytically active phase and surface *** present work aims at more precise understanding of the roles of the potassium and copper and the contribution of the metal oxide ***γ-Al_(2)O_(3)guarantees high dispersion and destabilisation of the potassium phase,potassium and copper act synergistically to remove CO_(2)from diluted streams and promote fast regeneration of the active phase for CO_(2)capture releasing CO while passing H_(2).A temperature of 350℃is found necessary to activate H_(2)dissociation and generate the active sites for CO_(2)*** effects of synthesis parameters on the CCR activity are also described by combination of ex-situ characterisation of the materials and catalytic testing.

Estimating wood quality attributes from dense airborne LiDAR point clouds

Mapping individual tree quality parameters from high-density LiDAR point clouds is an important step towards improved forest *** present a novel machine learning-based workflow that uses individual tree point clouds from drone laser scanning to predict wood quality indicators in standing *** object reconstruction methods,our approach is based on simple metrics computed on vertical slices that summarize information on point distances,angles,and geometric attributes of the space between and around the *** models use these slice metrics as predictors and achieve high accuracy for predicting the diameter of the largest branch per log (DLBs) and stem diameter at different heights (DS) from survey-grade drone laser *** show that our models are also robust and accurate when tested on suboptimal versions of the data generated by reductions in the number of points or emulations of suboptimal single-tree segmentation *** approach provides a simple,clear,and scalable solution that can be adapted to different situations both for research and more operational mapping.

Simultaneous removal of critical rare earth elements and chalcogen oxyanions by anaerobic granular sludge

Conventional mining of economically and strategically important critical rare earth elements(REEs)(such as neodymium,lanthanum and dysprosium),and chalcogens(such as selenium and tellurium)are associated with a huge economic and environmental ***,the need to recover REEs as well as chalcogens from different waste streams including wastewaters is becoming *** assays on synthetic chalcogen/REE-laden wastewater showed that the presence of REEs significantly improved the tellurite removal rate(>80%)and enhanced selenate removal by 66%±10%.Three 3.9 L continuous upflow anaerobic granular sludge bed(UASB)reactors were operated at a hydraulic retention time of 24 h and30℃.Selenate reduction was achieved with a removal efficiency of~98%with an influent p H of 4.0 for more than 28 *** effect of REEs on tellurite removal in the UASB bioreactor could not be clearly established since a soluble tellurium removal efficiency of more than 98%was observed already before the addition of REEs at elevated tellurite *** complete REE removal in both batch assays and UASB reactors at higher pH(7.0±0.5)was attributed to precipitation,whereas chalcogen oxyanions removal was due to microbial ***,at acidic p H,biosorption was responsible for REE's removal,and the Se-enriched sludge exhibited a superior REE's removal efficiency than the non-enriched and Te-enriched sludge.
