Reporting rotator cuff tears on magnetic resonance arthrography using the Snyder's arthroscopic classification

AIM To determine diagnostic performance of magnetic resonance arthrography(MRA) in evaluating rotator cuff tears(RCTs) using Snyder's classification for *** One hundred and twenty-six patients(64 males, 62 females; median age 55 years) underwent shoulder MRA and arthroscopy, which represented our reference standard. Surgical arthroscopic reports were reviewed and the reported Snyder's classification was recorded. MRA examinations were evaluated by two independent radiologists(14 and 5 years' experience) using Snyder's classification system, blinded to arthroscopy. Agreement between arthroscopy and MRA on partial-and fullthickness tears was calculated, first regardless of their extent. Then, analysis took into account also the extent of the tear. Interobserver agreement was also calculated the quadratically-weighted Cohen kappa *** On arthroscopy, 71/126 patients(56%) had a fullthickness RCT. The remaining 55/126 patients(44%) had a partial-thickness RCT. Regardless of tear extent, out of 71 patients with arthroscopically-confirmed fullthickness RCTs, 66(93%) were correctly scored by both readers. All 55 patients with arthroscopic diagnosis of partial-thickness RCT were correctly assigned as having a partial-thickness RCT at MRA by both readers. Interobserver reproducibility analysis showed total agreement between the two readers in distinguishing partial-thickness from full-thickness RCTs, regardless of tear extent(k = 1.000). With regard to tear extent, in patients in whom a complete tear was correctly diagnosed, correct tear extent was detected in 61/66 cases(92%); in the remaining 5/66 cases(8%), tear extent was underestimated. Agreement was k = 0.955. Interobserver agreement was total(k = 1.000).CONCLUSION MRA shows high diagnostic accuracy and reproducibility in evaluating RCTs using the Snyder's classification for reporting. Snyder's classification may be adopted for routine reporting of MRA.

AIM: To improve the interpretation of fecal immunochemical test (FIT) results in colorectal cancer (CRC) cases from screening and referral cohorts. METHODS: In this comparative observational study, two prospective cohorts of CRC cases were compared. The first cohort was obtained from 10 322 average risk subjects invited for CRC screening with FIT, of which, only subjects with a positive FIT were referred for colonoscopy. The second cohort was obtained from 3637 subjects scheduled for elective colonoscopy with a positive FIT result. The same FIT and positivity threshold (OC sensor; ≥ 50 ng/mL) was used in both cohorts. Colonoscopy was performed in all referral subjects and in FIT positive screening subjects. All CRC cases were selected from both cohorts. Outcome measurements were mean FIT results and FIT scores per tissue tumor stage (T stage). RESULTS: One hundred and eighteen patients with CRC were included in the present study: 28 cases obtained from the screening cohort (64% male; mean age 65 years, SD 6.5) and 90 cases obtained from the referral cohort (58% male; mean age 69 years, SD 9.8). The mean FIT results found were higher in the referral cohort (829 ± 302 ng/mLvs 613 ± 368 ng/mL,P = 0.02). Tissue tumor stage (T stage) distribution was dif-ferent between both populations [screening population: 13 (46%) T1, eight (29%) T2, six (21%) T3, one (4%) T4 carcinoma; referral population: 12 (13%) T1, 22 (24%) T2, 52 (58%) T3, four (4%) T4 carcinoma], and higher T stage was significantly associated with higher FIT results (P ved (screening vs referral: T1 498 ± 382 ng/mL vs 725 ± 374 ng/mL, P = 0.22; T2 787 ± 303 ng/mL vs 794 ± 341 ng/mL, P = 0.79; T3 563 ± 368 ng/mLvs 870 ± 258 ng/mL,P = 0.13; T4 not available). After correction for T stage in logistic regression analysis, no significant differences in mean FIT results were observed between both types of cohorts (P = 0.10). CONCLUSION

Objective: To evaluate the comparative diagnostic value of harmonic imaging(HI) in the assessment of patients with suspected infective endocarditis(IE). Setting: Tertiary referral centre. design: 139 consecutive patients were evaluated with three imaging modalities: transthoracic echocardiography with fundamental imaging(FI); HI; and transoesophageal echocardiography(TOE). Image qualitywas assessed for each modality by semiquantitative scoring(0, poor, to 3, excellent). Presence, dimension, and characteristics of vegetations were assessed separately for each imaging modality, as well as presence of abscesses. Results: 35 patients had definite IE. TOE was positive in 33 patients, HI in 28, and FI in 12(pv HI and v TOE). Mean image quality was 1.4(0.7) for FI, 2.1(0.6) for HI(pv FI), and 2.6(0.4) for TOE(pv HI). The association between FI and TOE findings was=0.35(χ2=17.57, p=0.0014) and between HI and TOE it was=0.95(χ2=125.72, pv FI). The global echo score of vegetations was 7.1(3.3) with FI, 8.5(3.4) with HI, and 11.3(3.9) with TOE(pv HI). Compared with TOE, FI identified only one of seven abscesses(sensitivity 14%) and HI identified two of seven abscesses(sensitivity 28%). Conclusions: HI provides an accurate assessment of suspected IE. TOE achieves superior definition of IE related abnormalities.

development of Global Geographical Coverage Area for Terrestrial Networks Internetworked with leo Satellite Network

Network planning, analysis and design are an iterative process aimed at ensuring that a new network service meets the needs of subscribers and operators. During the initial start-up phase, coverage is the big issue and coverage in telecommunications systems is related to the service area where a bare minimum access in the wireless network is possible. In order to guarantee visibility of at least one satellite above a certain satellite elevation, more satellites are required in the constellation to provide Global network services. Hence, the aim of this paper is to develop wide area network coverage for sparsely distributed earth stations in the world. A hybrid geometrical topology model using spherical coordinate framework was devised to provide wide area network coverage for sparsely distributed earth stations in the world. This topology model ensures Global satellite continuous network coverage for terrestrial networks. A computation of path lengths between any two satellites put in place to provide network services to selected cities in the world was carried out. A consideration of a suitable routing decision mechanism, routing protocols and algorithms were considered in the work while the shortest paths as well as the alternate paths between located nodes were computed. It was observed that a particular satellite with the central angle of 27°can provide services into the diameter of the instantaneous coverage distance of 4081.3 Km which is typical of wide area network coverage. This implies that link-state database routing scheme can be applied, continuous global geographical coverage with minimum span, minimum traffic pattern and latency are guaranteed. Traffic handover rerouting strategies need further research. Also, traffic engineering resources such as channel capacity and bandwidth utilization schemes need to be investigated. Satellite ATM network architecture will benefit and needs further study.

C3G is a GEF(guanine nucleotide exchange factor)for Rap GTPases,among which the isoform Rap1b is an essential protein in platelet *** transgenic mouse models with platelet-specific overexpression of C3G or mutant C3GΔCat,we have unveiled a new function of C3G in regulating the hemostatic function of platelets through its participation in the thrombin-PKCRap1b pathway.C3G also plays important roles in angiogenesis,tumor growth,and metastasis through its regulation of the platelet *** addition,C3G contributes to megakaryopoiesis and ***,we used a platelet-specific C3G-KO mouse model to further support the role of C3G in hemostasis.C3G-KO platelets showed a significant delay in platelet activation and aggregation as a consequence of the defective activation of Rap1,which resulted in decreased thrombus formation in ***,we explored the contribution of C3G-Rap1b to platelet signaling pathways triggered by thrombin,PMA or ADP,in the referenced transgenic mouse model,through the use of a battery of specific *** found that platelet C3G is phosphorylated at Tyr504 by a mechanism involving *** phosphorylation was shown to be positively regulated by ERKs through their inhibition of the tyrosine phosphatase ***,C3G participates in the ADP-P2Y12-PI3K-Rap1b pathway and is a mediator of thrombin-TXA2 ***,it inhibits the synthesis of TXA2 through cPLA2 *** together,our data reveal the critical role of C3G in the main pathways leading to platelet activation and aggregation through the regulation of Rap1b.

AIM: To check the safety and efficacy of boceprevir/telaprevir with peginterferon/ribavirin for hepatitis C virus(HCv) genotype 1 in the real-world settings. METHODS: This study was a non-randomized, observational, prospective, multicenter. This study involved 47 centers in Italy. A database was prepared for the homogenous collection of the data, was used by all of the centers for data collection, and was updated continuously. All of the patients enrolled in this study were older than 18 years of age and were diagnosed with chronic infection due to HCv genotype 1. The HCv RNA testing was performed using COBAS-Taq Man2.0(Roche, LLQ 25 IU/m L). RESULTS: All consecutively treated patients were included. Forty-seven centers enrolled 834 patients as follows: Male 64%; median age 57(range 18-78), of whom 18.3% were over 65; mean body mass index 25.6(range 16-39); genotype 1b(79.4%); diagnosis of cirrhosis(38.2%); and fibrosis F3/4(71.2%). The following drugs were used: Telaprevir(66.2%) and PEG-IFN-alpha2a(67.6%). Patients were na?ve(24.4%), relapsers(30.5%), partial responders(14.8%) and null responders(30.3%). Overall, adverse events(AEs) occurred in 617 patients(73.9%) during the treatment. Anemia was the most frequent AE(52.9% of cases), especially in cirrhotic. The therapy was stopped for 14.6% of the patients because of adverse events or virological failure(15%). Sustained virological response was achieved in 62.7% of the cases, but was 43.8% in cirrhotic patients over 65 years of age. CONCLUSION: In everyday practice, triple therapy is safe but has moderate efficacy, especially for patients over 65 years of age, with advanced fibrosis, nonresponders to peginterferon + ribavirin.

为了解ＨＩＶ抗体阳性血浆中的ＨＩＶ１病毒基因亚型的情况，应用逆转录ＰＣＲ和ＤＮＡ序列测定技术，对６份获自高危人群的抗ＨＩＶ１阳性血浆进行序列分析和基因亚型分型的研究，结果表明均属ＨＩＶ１Ｂ亚型。Ｖ３环氨基酸序列分析指出这些ＨＩＶ１Ｂ亚型病毒株与泰国ＨＩＶ１Ｂ亚型病毒株核苷酸和氨基酸序列相似；同时发现ＨＩＶ１ｃＤＮＡ和氨基酸序列均相同，推测这６份标本可能来自同时感染同一株ＨＩＶ病毒的感染者。本研究对了解高危人群中ＨＩＶ１流行的遗传变异和ＨＩＶ１亚型病毒株的分子流行病分析具有一定的意义。

Synthesis and Structural Characterization of Monocrystalline α-v₂O₅ Nanowires

A stable one-dimensional system in an orthorhombic α-v2O5 nanowires monocrystalline structure was obtained by a solvothermal method from a polymorphic v2O5 structure. The starting material was firstly submitted to acid hydrolysis in H2O2 followed by a solvothermal treatment. The outcome of this procedure, a metastable phase of the one-dimensional system v10O24·12H2O/v3O7·H2O, was subsequently reoxidized by controlled heating in an open air system. The final product was an orange crystalline solid mainly formed by monocrystalline nanowires of α-v2O5 having lengths of tens of micrometers and widths of about 75 nm with a preferential [200] growth direction. It was found that the pH value of the initial solution plays an important role in the formation of the crystalline phase in the final products. Characterization was performed by X-ray diffraction (XRD), scanning electron microscopy (SEM) and high-resolution transmission electron microscopy (HRTEM). This study offers an alternate route for the synthesis of vanadium oxides and related compounds.

Background: The US National Psoriasis Foundation recently recommended that PASI 50 and PASI 75 response rates be used in clinical trials to enable comparisons across studies of different psoriasis therapies. To date, these response rates have not been reported for the two-compound ointment containing calcipotriol and betamethasone dipropionate (Daivobet/Dovobet; leo Pharma, Ballerup, denmark). Further, in order to compare Daivobetwith other therapeutics recently presented to the European regulatory authorities and the FDA, comparison with the biologicals, efalizumab, etanercept and alefacept, were also made. Objectives: To present the PASI 50 and PASI 75 results for the two-compound ointment containing calcipotriol and betamethasone dipropionate. Methods: Data from six phase III studies conducted with the two-compound ointment were pooled and the PASI 50 and PASI 75 response rates calculated for patients with severe (PASI≥17) or less severe disease (PASI vant published phase III studies. Results: PASI 50 and PASI 75 were achieved by more patients treated with the two-compound ointment than with the individual components. In patients with severe disease, the PASI 50 response rate after 4 weeks’treatment was 88.8%with the two-compound ointment, 69.2%with betamethasone dipropionate, 53.8%with calcipotriol, and 30.0%with ointment vehicle. In comparison, 12 weeks’treatment with the biologicals resulted in PASI 50 response rates of 59%with efalizumab, 74%with etanercept, and 56%with alefacept. Conclusions: The two-compound ointment is effective, producing a PASI 50 and PASI 75 response in greater than 80%and 50%of patients, respectively, regardless of psoriasis severity.
