Preoperative Feeding in Single Ventricle Neonates is Predictive of Shorter Time to Goal Feed

Background:Patients with single ventricle anatomy are at increased risk of growth failure and *** cardiac centers,there is little standardization of feeding practices in this complex *** hypothesized that initiation of our center’s preoperative feeding protocol would result in decreased gastrostomy tube(G-tube)use,decreased length of stay and would not result in increased Necrotizing Enterocolitis(NEC)***:A single institution review of 52 patients who had undergone stage I single ventricle palliative repair was *** diagnoses were hypoplastic left heart syndrome(39%),atrioventricular canal(15%),and other(46%).Postoperative parameters such as time to goal feed and need for gastrostomy tube(G-tube)were compared among preoperatively fed and non-preoperatively fed *** to goal feed was calculated as time from first postoperative enteral feed to goal volume of 100 mL/***:Of the 26 patients who met inclusion criteria for preoperative feeding,22 patients(85%)were fed prior to *** proportional hazard ratio revealed that age at surgery(p=0.047)and being preoperatively fed(p=0.001)were associated with reaching goal feed *** analysis revealed that being preoperatively fed made a patient twice as likely to reach goal feed sooner(p=0.047).Univariable logistic regression revealed that days on total parenteral nutrition(p=0.018),length of hospitalization(p=0.008),and time to 1st postoperative feed(p=0.020)were significantly associated with higher odds of needing a G-tube *** logistic regression did not show any predictors of postoperative G-tube ***,there was a trend towards lower G-tube usage in the preoperatively fed ***:Implementing a standardized preoperative feeding protocol in single ventricle neonates can result in significantly shorter time to goal feed in preoperatively fed *** is beneficial for institutions to begin implementi

A pentaploid-based linkage map of the ancestral octoploid strawberry Fragaria virginiana reveals instances of sporadic hyper-recombination

The first high-resolution genetic linkage map of the ancestral octoploid(2n=8x=56)strawberry species,Fragaria virginiana,was constructed using segregation data obtained from a pentaploid progeny *** novel mapping population of size 178 was generated by crossing highly heterozygous *** hybrid“LB48”as a paternal parent with diploid(2n=2x=14)Fragaria vesca“Hawaii 4”.The LB48 linkage map comprises 6055 markers genotyped on the Axiom®IStraw90 strawberry SNP *** map consists of 28 linkage groups(LGs)organized into seven homoeology groups of four LGs each,and excludes a small 29th LG of undefined *** member of each homoeology group was assignable to an“A”subgenome associated with ancestral diploid Fragaria vesca,while no other subgenomes were *** an intriguing discrepancy within homoeology group VI,synteny comparisons with the previously published Fragaria×ananassa DA×MO linkage map revealed substantial *** initial map construction,examination of crossover distributions revealed that six of the total 5162(=29 chromosomes/individual×178 individuals)chromosomes making up the data set exhibited abnormally high crossover counts,ranging from 15 to 48 crossovers per chromosome,as compared with the overall mean of 0.66 crossovers per *** of these six hyper-recombinant(HypR)chromosomes occurred in a different LG and in a different *** calculated upon exclusion of the six HypR chromosomes,the canonical(i.e.,broadly representative)LB48 map had 1851 loci distributed over a total map length of 1873 cM,while their inclusion increased the number of loci by 130,and the overall map length by 91 *** of these hyper-recombinant chromosomes points to the existence of a sporadically acting mechanism that,if identified and manipulable,could be usefully harnessed for multiple purposes by geneticists and breeders.

Is pork trade good for China's greenhouse gas emissions?

China produces the largest amount of pork in the world, which emits the largest amount of greenhouse gases （GHGs）. This paper calculates GHG emissions from China＇s hog production at the provincial level using newly published emission factors~ Empirical results show that GHG emissions from China＇s hog production mainly respond to the scale intensity~ Capital abundance and income contribute positively to GI-IG emissions from hog production. Pork trade increases GHG emissions from China＇s hog produc- tion with a significantly direct effect, reduces GHG emissions through indirect technique effects.

Medicine as Profession： An Overlooked Approach to Medical Ethics

This article begins with three problems of ＂dual loyalties＂ in medicine, the supposed fact that military physicians are, as medical officers, sometimes required to do what violates ordinary medical ethics--for example, ignore medical need in order to treat their own wounded before civilians or wounded enemy, help make chemical or biological weapons more deadly, or assist at a rough interrogation. These problems are analyzed as special cases of a problem that could arise in any profession, a problem easily resolved using a theory of professional ethics （more or less） absent from medical ethics until now though common outside. Employing a physician--rather than an ordinary officer, some other kind of healer, or scientist--is to enter a sort of ＂Ulysses contract＂ requiring the physician＇s professional standards to preempt obligations otherwise applying to an employee. In this way, the article also illustrates the benefits that might accrue to medical ethics from drawing （more than is now common） on other fields of practical ethics.

Nanoscale inhibition of polymorphic and ambidextrous lAPP amyloid aggregation with small molecules

Understanding how small molecules interface with amyloid fibrils at the nanoscale is of importance for developing therapeutic treatments against amyloid-based diseases. Here, we show for the first time that human islet amyloid polypeptides （LAPP） in the fibrillar form are polymorphic, ambidextrous, and possess multiple periodicities. Upon interfacing with the small molecule epigallocatechin gallate （EGCG）, IAPP aggregation was rendered off-pathway and assumed a form with soft and disordered clusters, while mature IAPP fibrils displayed kinks and branching but conserved the twisted fibril morphology. These nanoscale phenomena resulted from competitive interactions between EGCG and the IAPP amyloidogenic region, as well as end capping of the fibrils by the small molecule. This information is crucial in delineating IAPP toxicity implicated in type 2 diabetes and for developing new inhibitors against amyloidogenesis.
