Don’t forget emergency surgery! Lessons to learn from elective indocyanine green-guided gastrointestinal interventions

Fluorescence-based imaging has found application in several fields of elective surgery,but there is still a lack of evidence in the literature about its use in the emergency setting.Clinical trials have consistently shown that indocyanine green(ICG)-guided surgery can dramatically reduce the risk of postoperative complic-ations,length of in-hospital stay and total healthcare costs in the elective setting.It is well-known that emergency surgery has a higher complication rate than its elective counterpart,therefore an impelling need for research studies to explore,validate and develop this issue has been highlighted.The present editorial aims to provide a critical overview of currently available applications and pitfalls of ICG fluorescence in abdominal emergencies.Furthermore,we evidenced how the experience of ICG-fluorescence in elective surgery might be of great help in implementing its use in acute situations.In the first paragraph we analyzed the tips and tricks of ICG-guided cancer surgery that might be exploited in acute cases.We then deepened the two most described topics in ICG-guided emergency surgery:Acute cholecystitis and intestinal ischemia,focusing on both the advantages and limitations of green-fluorescence application in these two fields.In emergency situations,ICG fluorescence demonstrates a promising role in preventing undue intestinal resections or their entity,facilitating the detection of intestinal ischemic zones,identifying biliary tree anatomy,reducing post-operative complications,and mitigating high mortality rates.The need to improve its application still exists,therefore we strongly believe that the elective and routinary use of the dye is the best way to acquire the necessary skills for emer-gency procedures.

Intraspecific competition at the larval stage is an important ecological factor affecting life-history,adaptation and evolutionary trajectory in holometabolous insects.However,the molecular pathways underpinning these ecological processes are poorly characterized.We reared Drosophila melanogaster at three egg densities(5,60,and 300 eggs/mL)and sequenced the transcriptomes of pooled third-instar larvae.We also examined emergence time,egg-to-adult viability,adult mass,and adult sex-ratio at each density.Medium crowding had minor detrimental effects on adult phenotypes compared to low density and yielded 24 differentially expressed genes(DEGs),including several chitinase enzymes.In contrast,high crowding had substantial detrimental effects on adult phenotypes and yielded 2107 DEGs.Among these,upregulated gene sets were enriched in sugar,steroid and amino acid metabolism as well as DNA replication pathways,whereas downregulated gene sets were enriched in ABC transporters,taurine,Toll/Imd signaling,and P450 xenobiotics metabolism pathways.Overall,our findings show that larval crowding has a large consistent effect on several molecular pathways(i.e.,core responses)with few pathways displaying density-specific regulation(i.e.,idiosyncratic responses).This provides important insights into how holometabolous insects respond to intraspecific competition during development.

The rising CO2 level, population boom and increasing energy demand prompts the need of an efficient and sustainable solution to tackle the global warming issue. Reduction of greenhouse gas（GHG） emission through the conversion of detrimental CO2 into methanol is one of the most promising solutions for optimising economic and resource efficiency. The utilisation of the abundant and sustainable sunlight to replace thermal and electric energy for CO2 conversion to valuable chemicals is a highly sustainable process and attracted much research interests. Herein, we summarised the catalytic methods for CO2 conversion to methanol, reviewed the photocatalytic properties and efficient photocatalysts, as well as their performance. Carbon [78TD$IF]quantum dots（CQDs） as a new member of the carbon nanomaterials family have attracted increasing attention owing to their excellent photoluminescence property, light harvesting capability, charge recombination suppression and effective electron transport ability. This paper highlighted the multifaceted roles of CQDs in photocatalytic reactions. To this end, the challenges and future directions of CQDs-based photocatalysts have been outlined.

在建筑物的地震易损性分析中,人们常采用基于概率的抗震需求谱来进行,而云分析是常用的一种方法。在云分析方法中,统计模型用来预测建筑物在地面地震强度下的需求。这些模型的地震破坏参数(EDP s)等数据,通过建筑物在一组不同烈度(IM)地震动下的动态或静态非线性分析获得。统计模型得到了任意强度IM下的地震损伤指数(EDP),并给出不同记录间的差异。本论文通过在动力非线性分析中考虑记录间的差异及参数的不确定性,提出一种扩展的云分析方法。案例研究通过一个三跨四层的钢筋混凝土建筑,满足欧洲抗震规范的设计。采用的EDP为楼层最大层间位移,此指标可由Opensees软件计算获得。参数不确定性主要考虑了混凝土轴心抗压强度变化C30/37以及S450钢材的屈服强度变化。采用蒙特卡洛方法研究从模型参数到EDP指标间的不确定性,并采用常用的克里格替代模型来大幅减少动力非线性分析的计算成本。对比考虑这些参数不确定性及不考虑参数不确定性的云分析结果,可以得到基于性能的抗震设计中较感兴趣的易损性结果。

This position paper is the outcome of a brainstorming workshop organised by the International Society for Digital Earth(ISDE)in Beijing in March 2011.It argues that the vision of Digital Earth(DE)put forward by Vice-President Al Gore 13 years ago needs to be re-evaluated in the light of the many developments in the fields of information technology,data infrastructures and earth observation that have taken place since.The paper identifies the main policy,scientific and societal drivers for the development of DE and illustrates the multi-faceted nature of a new vision of DE grounding it with a few examples of potential applications.Because no single organisation can on its own develop all the aspects of DE,it is essential to develop a series of collaborations at the global level to turn the vision outlined in this paper into reality.
