Iron deficiency anemia(IDA) is associated with a number of pathological gastrointestinal conditions other than inflammatory bowel disease, and also with liver disorders. Different factors such as chronic bleeding, malabsorption and inflammation may contribute to IDA. Although patients with symptoms of anemia are frequently referred to gastroenterologists, the approach to diagnosis and selection of treatment as well as follow-up measures is not standardized and suboptimal. Iron deficiency, even without anemia, can substantially impact physical and cognitive function and reduce quality of life. Therefore, regular iron status assessment and awareness of the clinical consequences of impaired iron status are critical. While the range of options for treatment of IDA is increasing due to the availability of effective and well-tolerated parenteral iron preparations, a comprehensive overview of IDA and its therapy in patients with gastrointestinal conditions is currently lacking. Furthermore, definitions and assessment of iron status lack harmonization and there is a paucity of expert guidelines on this topic. This review summarizes current thinking concerning IDA as a common co-morbidity in specific gastrointestinal and liver disorders, and thus encourages a more unified treatment approach to anemia and iron deficiency, while offering gastroenterologists guidance on treatment options for IDA in everyday clinical practice.

目的:针对新加坡的花岗岩地质结构,研究将典型马蹄形隧道挖掘成双拱隧道时岩石支撑的设计问题。利用离散元法分析侧边导坑对隧道主体开口的影响。创新点:1.基于平面应变假设,提出一个针对性的离散元模型以分析侧边导坑的设置对主体隧道支撑的影响;2.此模型适用于解决底下长跨度开口的支撑设计问题,如大众捷运系统隧道和紧贴式双孔隧道的建造,补充了传统经验主义的设计方法。方法:1.采用离散元法对马蹄形隧道开口支撑问题进行非连续分析;2.采用二维平面应变模型简化问题。结论:1.离散元法可用于分析有限岩石覆盖层的失效机制,单隧道支撑要求以及增加侧边导坑后的支撑要求;2.模型计算结果显示,侧边导坑的挖掘使得支柱总的螺栓连接作用力增加了一倍。

System performance of solar water heaters depends upon collector and storage tank designs, solar radiation intensity and ambient temperature, amongst others. Evacuated glass tube collectors with U-tubes inside are less prone to leakages than the all-glass or the heat pipe types. U-tube solar water heaters suspended on walls and balconies could help overcome present day roof space restriction and increasing apartment-style housing. As such, their performance would depend upon its orientation when mounted in a vertical position. This paper reports the results of outdoor tests conducted on natural convection U-tube solar water heaters oriented towards different directions. Long and short term test procedures were employed to allow us to compare their performances as if they were tested simultaneously side-by-side.

Bicuculline-insensitive receptors for the inhibitory neurotransmitter γ-aminobutyric acid (GABA), GABAB receptors, are a distinct subclass of receptors that mediate depression of synaptic transmission and contribute to neu-ronal inhibition. When activated, these receptors reduce transmission at excitatory and inhibitory synapses, as a result of an increase in K+ conductance, or a decrease in voltage-dependent Ca2+ currents. They are also linked to G-proteins, or intracellular effector systems in a very complex manner. The recent development of highly specific and potent agonists and antagonists for these receptors has led to a much better understanding of their physiology and pharmacology, including their heterogeneity, as well as their molecular biology. Over the past year, expression and cloning studies have contributed to major advances in characterizing GABAB receptor structure, with the discovery of the amino acid sequences of GABABRla/R1b splice variants and GABABR2 receptors. High-density single nucleotide polymorphism （SNP） genotyping arrays are powerful tools that can measure the level of genetic polymorphism within a population. To develop a whole-genome SNP array for oil palms, SNP discovery was performed using deep resequencing of eight libraries derived from 132 Elaeis guineen- sis and Elaeis oleifera palms belonging to 59 origins, resulting in the discovery of 〉3 million putative SNPs. After SNP filtering, the Illumina OP200K custom array was built with 170 860 successful probes. Phenetic clustering analysis revealed that the array could distinguish between palms of different origins in a way consistent with pedigree records. Genome-wide linkage disequilibrium declined more slowly for the commercial populations （ranging from 120 kb at r2 = 0.43 to 146 kb at r2 = 0.50） when compared with the semi-wild populations （19.5 kb at r2 = 0.22）. Genetic fixation mapping comparing the semi-wild and commercial population identified 321 selective sweeps. A genome-wide association study （GWAS） detected a significant peak on chromosome 2 associated with the polygenic component of the shell thick- ness trait （based on the trait shell-to-fruit; S/F %） in tenera palms. Testing of a genomic selection model on the same trait resulted in good prediction accuracy （r = 0.65） with 42% of the S/F % variation explained. The first high-density SNP genotyping array for oil palm has been developed and shown to be robust for use in genetic studies and with potential for developing early trait prediction to shorten the oil palm breeding cycle.

Objective To evaluate the clinical characteristics and in-hospital outcomes of elderly South-East Asian patients undergoing primary pereutaneous coronary intervention （PPCI）. Methods From January 2009 to December 2012, 1268 patients （86.4% male, mean age of 58,4 ± 12.2 years） presented to our hospital for ST-elevation myocardial infarction （STEMI） and underwent PPCI. They were divided into two groups： elderly group defined as age _〉 70 years and non-elderly group defined as age 〈 70 years. Data were collected retrospectively on baseline clinical characteristics, door-to-balloon （D2B） time, angiographic findings, therapeutic modality and hospital course. Results The elderly group constituted 19% of the study population with mean age 76.6 ± 5.0 years. There was a higher proportion of female gender and ethnic Chinese patients in the elderly group when compared with the non-elderly group. The former was less likely to be smokers and have a significantly higher prevalence of hypertension. The mean D2B time was significantly longer in the elderly group. They also had a significantly higher incidence of triple vessel disease and obstructive left main disease. The use of radial artery access, glyeoprotein 2b/3a inhibitors and drug-eluting stents during PPCI were also significantly lower. In-hospital mortality was significantly higher in the elderly group. The rate of cardiogenic shock and inhospital complications were also significantly higher. Conclusions Our registry showed that in-hospital mortality rate in elderly South-East Asian patients undergoing PPCI for STEMI was high. Further studies into the optimal STEMI management strat- egy for these elderly patients are warranted.

xt-align:justify;font-size:medium;white-space:normal;"> Distribution patterns of selected heavy metals content in sediments from the Bay of Quiberon and Gulf of Morbihan were studied to understand the current heavy metals contamination due to urbanization and mariculture activities in the coastal area. Therefore, a survey was conducted and 196 sediments collected were characterized for heavy metals content using Inductively Coupled Plasma-Mass Spectrometry (ICP-MS) after mix acid digestion process. The distribution maps of the concentrations of the heavy metals studied were produced as an isopleth map based on data interpolation by the ArcGIS software application. The association with the adverse effects on aquatic organisms was determined by the classification of the sediment according to the sediment quality guidelines. Therefore, two approaches were employed namely: direct comparison with Sediment Quality Guidelines (SQGs) by USEPA (United States Environmental Protection Agency) and comparison with other numerical SQGs, threshold effect level/probable effect level, and effect range low/effect range medium. In order to estimate the effect of multiple contaminations of heavy metals, the mean-ERM-quotient was calculated at each sampling point.
