purpose Sled towing has been shown to be an effective method to enhance the physical qualities in youth *** aim of this study was to evaluate the impact of a 6-week sled towing intervention on muscular strength,speed and power in elite youth soccer players of differing maturity *** Seventy-three male elite youth soccer players aged 12-18 years(pre-peak Height Velocity[pHV]n = 25;Circa-pHV n = 24;post-pHV n = 24)from one professional soccer academy participated in this *** assessments(10 m and 30 m),countermovement jump and isometric mid-thigh pull were undertaken before(T1)and after(T2)a 6-week *** training intervention consisted of 6 weeks(2xper week,10 sprints over 20 m distance)of resisted sled towing(linear progression 10%-30%of body mass)during the competitive *** regression models analysed differences between T1 and T2 within each maturity *** There were minimal changes in strength,speed and power(p=0.35-0.80)for each maturity group across the 6-week *** there were changes with greater certainty,they are unlikely to represent real effect due to higher regres-sion to the mean(RTM).Conclusion It appears that a 6-week sled towing training programme with loadings of 10%-30%body mass only maintains physical qualities in elite youth soccer players pre-,circa-,and *** research is required to determine the effectiveness of this training method in long-term athletic development programmes.

The Chebyshev polynomials are harnessed as functions of the one parameter of the nondimensionalized differential equation for trinomial homogeneous linear differential equations of arbitrary order n that have constant coefficients and exhibit vibration. The use of the Chebyshev polynomials allows calculation of the analytic solutions for arbitrary n in terms of the orthogonal Chebyshev polynomials to provide a more stable solution form and natural sensitivity analysis in terms of one parameter and the initial conditions in 6n + 7 arithmetic operations and one square root.

BACKGROUND:The presence of dual pathology can cause diagnostic dilemmas. We present a case of adenocarcinoma of the ampulla of Vater with concurrent amiodarone hepatotoxicity. METHODS:painless jaundice associated with a palpable gallbladder was investigated clinically,radiologically endoscopically and via liver biopsy. RESULTS:Liver biopsy showed amiodarone hepatotoxicity Endoscopic biopsy identified an ampullary adenoma However,the endoscopic ultrasound and intra-operative findings suggested a malignancy,which was confirmed postoperatively. CONCLUSIONS:While the classic findings of Courvoisier’s Law are borne out in this case,the etiology of jaundice is twofold. Although dual pathology is uncommon it should always be considered.

Restoring lifeline services to an urban neighborhood impacted by a large disaster is critical to the recovery of the city as a *** cities are comprised of many dependent lifeline systems,the pattern of the restoration of each lifeline system can have an impact on one or more *** to the often uncertain and complex interactions between dense lifeline systems and their individual operations at the urban scale,it is typically unclear how different patterns of restoration will impact the overall recovery of lifeline system functioning.A difficulty in addressing this problem is the siloed nature of the knowledge and operations of different types of ***,a city-wide,multi-lifeline restoration model and simulation are provided to address this *** approach uses the Graph Model for Operational Resilience,a data-driven discrete event simulator that can model the spatial and functional cascade of hazard effects and the pattern of restoration over time.A novel case study model of the District of North Vancouver is constructed and simulated for a reference magnitude 7.3 *** model comprises municipal water and wastewater,power distribution,and transport *** model includes 1725 entities from within these sectors,connected through 6456 dependency *** of the model shows that water distribution and wastewater treatment systems recover more quickly and with less uncertainty than electric power and road *** this uncertainty will provide the opportunity to improve data collection,modeling,and collaboration with stakeholders in the future.

AIM : To evaluate the prediction error in intraocular lens(IOL) power calculation for a rotationally asymmetric refractive multifocal IOL and the impact on this error of the optimization of the keratometric estimation of the corneal power and the prediction of the effective lens position(ELp).METHODS: Retrospective study including a total of 25 eyes of 13 patients(age, 50 to 83y) with previous cataract surgery with implantation of the Lentis Mplus LS-312 IOL(Oculentis Gmb H, Germany). In all cases, an adjusted IOL power(p IOLadj) was calculated based on Gaussian optics using a variable keratometric index value(n kadj) for the estimation of the corneal power(p kadj) and on a new value for ELp(ELp adj) obtained by multiple regression *** p IOLadj was compared with the IOL power implanted(p IOLReal) and the value proposed by three conventional formulas(Haigis, Hoffer Q and Holladay Ⅰ).RESULTS: p IOLReal was not significantly different than p IOLadj and Holladay IOL power(p >0.05). In the Bland and Altman analysis, p IOLadj showed lower mean difference(-0.07 D) and limits of agreement(of 1.47 and-1.61 D)when compared to p IOLReal than the IOL power value obtained with the Holladay formula. Furthermore, ELp adj was significantly lower than ELp calculated with other conventional formulas(p pendent on axial length, anterior chamber depth and p kadj. CONCLUSION: Refractive outcomes after cataract surgery with implantation of the multifocal IOL Lentis Mplus LS-312 can be optimized by minimizing thekeratometric error and by estimating ELp using a mathematical expression dependent on anatomical factors.

Background:previous studies have demonstrated the prognostic significance of pathologic tumor response in pancreatic adenocarcinoma following neoadjuvant therapy(NAT).The aim of this study was to determine the incidence of significant pathologic response to NAT in borderline resectable pancreatic cancer(BRpC),and association of NAT regimen and other clinico-pathologic characteristics with pathologic ***:patients with BRpC who underwent NAT and pancreatic resection between January 2012 and June 2017 were *** response was assessed on a qualitative scale based on the College of American pathologists grading *** and baseline characteristics,oncologic treatment,pathology,and survival outcomes were ***:Seventy-one patients were included for *** patients had complete pathologic responses(tumor regression score 0),12 patients had marked responses(score 1),42 had moderate responses(score 2),and 13 had minimal responses(score 3).patients with complete or marked responses were more likely to have received neoadjuvant gemcitabine chemoradiation(62.5%,38.1%,and 23.1%of the complete/marked,moderate,and minimal response groups,respectively;p=0.04).Of the complete/marked,moderate,and minimal response groups,margins were negative in 75.0%,78.6%,and 46.2%(p=0.16);node negative disease was observed in 87.5%,54.8%,and 15.4%(pp=0.563).Of the four patients with pathologic complete responses,three were disease-free at 66.1,41.7 and 31.4 months,and one was deceased with metastatic liver disease at 16.9 ***:A more pronounced pathologic tumor response to NAT in BRpC is correlated with node negative disease,but was not associated with a statistically significant survival benefit in this study.

Virtual Reality as an Adjunct to Ketamine Infusion Therapy Increases patient Satisfaction in the Management of Chronic pain and Depression: A Retrospective pilot Study

The management of patients with concomitant chronic pain (Cp) and Major Depressive Disorder (MDD) remains challenging for clinicians. Current chronic pharmacologic management is often unsuccessful, or has intolerable side effects to the patients. While not restricted to patients with chronic pain, these patients are often diagnosed with depression, presenting with symptoms such as poor mood, anhedonia, and altered cognitive processes. It is estimated that a substantial proportion of treated patients do not derive a substantive benefit from traditional pharmacological treatments for depression. The present study involved a retrospective review of cases, exploring the patient-reported satisfaction with and tolerability of a novel use of virtual reality (VR), coined KVR, as an adjunct to intravenous ketamine infusion therapies. Specifically, the ketamine-virtual reality protocol was employed as a potential adjunctive intervention for patients suffering from chronic pain and depression. Visual Analog Scores (VAS) associated with pain were significantly lower on the third than on the first assessment day. Montgomery-?sberg Depression Rating Scale (MADRS) scores improved following infusion and across days (i.e., sessions). Lastly, 2/3 of patients preferred the use of VR with their ketamine infusion. The results are considered in terms of implementing prospective studies to examine whether the combination therapies have a synergistic benefit and the nature and magnitude of clinically meaningful treatment effects, if any.

Astyanax mexicanus has repeatedly colonized cave environments,displaying evolutionary parallelisms in many troglobitic *** being a model system for the study of adaptation to life in perpetual darkness,the parasites that infect cavefish are practically *** this study,we investigated the macroparasite communities in 18 cavefish populations from independent lineages and compared them with the parasite diversity found in their sister surface fish populations,with the aim of better understanding the role that parasites play in the colonization of new *** the cavefish populations,we identified 13 parasite taxa,including a subset of 10 of the 27 parasite taxa known for the surface *** infecting the cavefish belong to five taxonomic groups,including trematodes,monogeneans,nematodes,copepods,and *** are the most dominant group,found in 14 *** macroparasites include species with direct life cycles and trophic transmission,including invasive ***,paired comparisons indicate higher parasite richness in the cavefish than in the surface *** variation in parasite composition across the caves suggests historical and geographical contingencies in the host-parasite colonization process and potential evolution of local *** base-line data on parasite diversity in cavefish populations of *** provides a foundation to explore the role of divergent parasite infections under contrasting ecological pressures(cave *** environments)in the evolution of cave adaptive traits.
