Calcifying tendinopathy is a tendon disorder with calcium deposits in the mid-substance presented with chronic activity-related pain, tenderness, local edema and various degrees of incapacitation. Most of current treatments are neither effective nor evidence-based because its underlying pathogenesis is poorly understood and treatment is usually symptomatic. Understanding the pathogenesis of calcifying tendlinopathy is essential for its effective evidence-based management. One of the key histopathological features of calcifying tendinopathy is the presence of chondrocyte phenotype which surrounds the calcific deposits, suggesting that the formation of calcific deposits was *** the origin of cells participating in the formation of chondrocyte phenotype and ossification is still unknown, many evidences have suggested that erroneous tendon cell differentiation is involved in the process. Recent studies have shown the presence of stem cells with self-renewal and multi-differentiation potential in human,horse, mouse and rat tendon tissues. We hypothesized that the erroneous differentiation of tendon-derived stem cells （TDSCs） to chondrocytes or osteoblasts leads to chondrometaplasia and ossification and hence weaker tendon, failed healing and pain, in calcifying tendinopathy. We present a hypothetical model on the pathogenesis and evidences to support this hypothesis. Understanding the key role of TDSCs in the pathogenesis of calcifying tendinopathy and the mechanisms contributing to their erroneous differentiation would provide new opportunities for the management of calcifying tendinopathy. The re-direction of the differentiation of resident TDSCs to tenogenic or supplementation of MSCsprogrammed for tenogenic differentiation may be enticing targets for the management of calcifying tendinopathy in e future.

Background Synovium-derived stem cells （SDSCs） with higher chondrogenic potential are attracting considerable attention as a cell source for cartilage regeneration. We investigated the effect of bone morphogenetic protein 2 （BMP-2） on transforming growth factor beta3 （TGF-β3）-induced chondrogenesis of SDSCs isolated from human osteoarthritic synovium in a pellet culture system. Methods The clonogenicity, stem cell marker expression and multi-differentiation potential of isolated SDSCs were determined by colony forming unit assay, flow cytometry and specific staining including alizarin red S, Oil red O and alcian blue staining, respectively. SDSCs pellet was cultured in chondrogenic medium with or without TGF-β3 or/and BMP-2. At day 21, the diameter and the weight of the pellets were measured. Chondrogenic differentiation of SDSCs was evaluated by Safranin O staining, immunohistochemical staining of collagen type Ⅱ, sulfated glycosaminoglycan （sGAG） synthesis and mRNA expression of collagen type Ⅱ, aggrecan, SOX9, link-protein, collagen type X and BMP receptor Ⅱ. Results Cells isolated under the optimized culturing density （104/60 cm2） showed clonogenicity and multi-differentiation potential. These cells were positive （〉99%） for CD44, CD90, CD105 and negative （〈10%） for CD34 and CD71. SDSCs differentiated to a chondrocytic phenotype in chondrogenic medium containing TGF-β3 with or without BMP-2. Safranin O staining of the extracellular matrix was positive and the expression of collagen type Ⅱ was detected. Cell pellets treated with TGF-β3 and BMP-2 were larger in diameter and weight, produced more sGAGs, and expressed higher levels of collagen type Ⅱ and other chondrogenic markers, except COL10A1, than medium with TGF-β3 alone. Conclusions SDSCs could be isolated from human osteoarthritic synovium. Supplementation with BMP-2 significantly promoted the in vitro TGF-β3-induced chondrogenic differentiation of SDSCs.

AIM： To identify the risk factors in predicting the out- come of acute-on-chronic hepatitis B liver failure pa- tients. METHODS： We retrospectively divided 113 patients with acute-on-chronic liver failure-hepatitis B virus （ACLF-HBV） and without concurrent hepatitis C or D virus infection and hepatocellular carcinoma into two groups according to their outcomes after anti-HBV therapy. Their demographic, clinical, and biochemical data on the day of diagnosis and after the first week of treatment were analyzed using the Mann-Whitney U test, Fisher＇s exact test, and a multiple logistic regres- sion analysis. RESULTS： The study included 113 patients （87 men and 26 women） with a mean age of 49.84 years. Fifty- two patients survived, and 61 patients died. Liver failure （85.2%）, sepsis （34.4%）, and multiple organ failure （39.3%） were the main causes of death. Mul- tivariate analyses showed that Acute Physiology and Chronic Health Evaluation （APACHE） Ⅱ scores ≥ 12 [odds ratio （OR） = 7.160, 95% CI： 2.834-18.092, P 〈 0.001] and positive blood culture （OR = 13.520, 95% CI： 2.740-66.721, P = 0.001） on the day of diagnosis and model for end-stage liver disease （MELD） scores 28 （OR = 8.182, 95% CI： 1.884-35.527, P = 0.005） after the first week of treatment were independent predictors of mortality. CONCLUSION： APACHE II scores on the day of diag- nosis and MELD scores after the first week of anti-HBV therapy are feasible predictors of outcome in ACLF- HBV patients.

Objective:To evaluate the efficacy of deep vein thrombosis(DVT)prevention among realworld surgical inpatients who received panax notoginseng saponins(PNS)combined with low-molecularweight heparin(LMWH).Methods:A prospective cohort study was conducted among surgical patients between January 2016 and November 2018 in Xuanwu Hospital,Capital Medical University,Beijing,*** received LMWH alone or PNS combined with LMWH for preventing *** primary outcome was incidence of lower extremity DVT,which was screened once a *** in the LMWH group were given LMWH(enoxaparin)via hypodermic injection,4000-8000 AxalU once *** in the exposure group received PNS(Xuesaitong oral tablets,100 mg,3 times daily)combined with LMWH given the same as LMWH ***:Of the 325 patients screened for the study,281 participants were included in the final *** cohort was divided into PNS+LMWH group and LMWH group with 134 and 147 participants,*** was a significant difference of DVT incidence between two groups(P=0.01),with 21(15.7%)incident DVT in the PNS+LMWH group,and 41(27.9%)incident DVT in the LMWH *** with participants without DVT,the participants diagnosed with DVT were older and had higher D-dimer *** multivariate logistic regression moclel showed a significant lower risk of incident DVT among participants in the PNS+LMWH group compared with the LMWH group(odds ratio 0.46,95%confidence interval,0.25-0.86).There were no significant differences in thromboelaslography values(including R,K,Angle,and MA)and differences in severe bleeding between two *** symptomatic pulmonary embolism occurred during the ***:Combined application of PNS and LMWH can effectively reduce the incidence of DVT among surgical inpatients compared with LMWH monotherapy,without increased risk of bleeding.

Sarawak River Flow Behaviour after Matang Bypass channel Construction during Low Tide Using InfoWorks River Simulation (RS)

Flood is occurring more frequently in Kuching nowadays due to the impact of climate change and rapid urbanization. The only discharge outlet for Sarawak River Basin currently is at Kuching Barrage and Shiplock. Sarawak State Government had decided to build Matang Bypass channel from Sarawak River’s “Oxbow” to Batang Salak River for mitigating the flooding issues within Sarawak River Basin. Matang Bypass channel had a bottom width of 250 m, 500 m reserve width and 8 Km in length. Flow behaviour with two discharge outlets during low tides are unknown yet. Therefore, this research is carried out to study Sarawak River flow behaviour after construction of Matang Bypass channel using InfoWorks River Simulation (RS). Rainfall data used is January 2018. Four scenarios investigated are 1) Open two gates at Matang Bypass channel opens and all gates at Kuching Barrage, 2) Open all gates at Matang Bypass channel and Kuching Barrage, 3) Open gates at Matang Bypass channel, but close all gates at Kuching Barrage, 4) Close all gates at Matang Bypass channel, but open all gates at Kuching Barrage. Results revealed that when water gates are opened, sea water has the potential to backflow into Sarawak River basin through Kuching Barrage since sea level at Kuching Barrage discharge outlet is always 0.5 m higher than Matang Bypass channel discharge outlet. When the gates at Matang Bypass channel are fully opened and Kuching Barrage are closed, Kuching Barrage will retain the excess water and the river water will only be discharged into ocean through Matang Bypass channel. In contrast, as the gates at Matang Bypass channel are closed and at Kuching Barrage are fully opened, Matang Bypass channel will store the excess water and river water will be discharged through Kuching Barrage alone.

Ma'an Qiao Village, a Dai and Yi ethnic minority-based community in Sichuan Province, China sustained complete infrastructure devastation during the 2008 Panzhihua earthquake. Health emergency and disaster risk management(Health-EDRM) education intervention programs were implemented in 2010 and 2011. This serial cross-sectional survey study aimed to examine the immediate and long-term impacts of the Health-EDRM interventions in this remote rural community. The findings demonstrate knowledge improvement in areas of water and sanitation, food and nutrition, and disaster preparedness immediately after the Health-EDRM education interventions. Temporal stability of knowledge retention was observed in household hygiene and waste management and smoking beliefs in 2018, 7 years after the *** important findings include knowledge uptake pattern differences of oral rehydration solution(ORS) between earthquake-prone and flood-prone communities. Usage of Internet and mobile technology for accessing disaster-related information was found to be independent of gender and income. Overall, this study demonstrated the knowledge improvement through Health-EDRM education interventions in a remote rural community. Promoting behavioral changes through interventions to raise awareness has the potential to reduce health risks in transitional post-disaster settings. Future programs should aim to identify evidence-based practices and explore how technology can support Health-EDRM education among vulnerable subgroups.
