Objective:The purpose of the study was to identify the best sequence of therapy beginning with a tyrosine kinase inhibitor(TKI)as the first-line therapy for patients with metastatic renal cell carcinoma(mRCC)in terms of overall survival(OS),progression-free survival(PFS),and rates of discontinuation and adverse effects during the treatment ***:This is a retrospective,nationwide multicenter study of patients with mRCC after diagnosis at 10 different tertiary medical centers in Korea from January 1992 to December *** focused on patients at either“favorable”or“intermediate”risk according to the International mRCC Database Consortium criteria,and they were followed up(median 335 days).Finally,a total of 1409 patients were selected as the study *** generated a Cox proportional hazard model adjusted for covariates,and the different therapy schemes were statistically tested in terms of OS as well as *** addition,frequencies of discontinuation and adverse events were compared among the therapy ***:Of the primary patterns of treatment sequences(24 sequences),“sunitinib epazopanib”and“sunitinibeeverolimuseimmunotherapy”showed the most beneficial results in both OS and PFS with significantly lower hazards than“sunitinib”,which is the most commonly treated agent in *** that the“TKIeTKI”structure showed relatively higher discontinuation rates with higher adverse effects,the overall beneficial sequence would be“sunitinibeeverolimuseimmunotherapy”.Conclusion:Among several sequential therapy starting with TKIs,“sunitinibeeverolimuse immunotherapy”was found to be the best scheme for mRCC patients with“favorable”or“intermediate”risks.

1) Background: Osteoarthritis (OA) is defined as a degenerative joint disease that mainly affects the bone. This study aims to evaluate the effect of low-intensity continuous ultrasound (LICUS) treatment on the knee of osteoarthritis patients through home-based intervention using the LICUS medical device. 2) Methods: The clinical trials were designed in a single-arm, open-label, and intervention study. Thirty-five participants, including those who dropped out (12%), were screened and enrolled. The patients received LICUS (1.1 MHz, 1.5 w/cm2, collimated beams) on the knee by the instructions of the investigator at home (5 min/session, 3 times/day, for four-weeks). Outcome measures were assessed using the Visual Analog Scale (VAS) as a primary endpoint and the western Ontario and McMaster Universities Osteoarthritis Index (wOMAC) as a secondary endpoint to evaluate pain relief and functional recovery of the knee between pre-treatment (baseline) and post-treatment (four-weeks). 3) Results: Knee pain scores measured using the VAS and wOMAC indices were significantly reduced after a four-week treatment with LICUS compared to baseline. Knee stiffness and functional capacity were significantly reduced after the LICUS application. In addition, there were no reports of adverse effects during the study period. 4) Conclusion: Long-term and home-based application of LICUS can be recommended as an alternative option for the treatment of OA patients, as evidenced by the effect of pain relief and knee function recovery.

Segregation of voiced and unvoiced components from residual of speech signal

In conventional source-filter models, voiced and unvoiced components were considered independently. However, in practice it was difficult to separate the source into two parts. An actual source consists of a mixture of two sources and the ratio varies according to the content or the intention of speaker. It had been investigated to separate the voiced and unvoiced components for different source models. Source signals were modeled based on the residual signal measured from inverse filtering. Three different source models were assumed. The parameters of each model were optimized for the original speech signal using a genetic algorithm. The resulting parameters were compared in terms of the mel-cepstral distance to the original signal, the spectrogram and the spectral envelope from the synthesized signal. The optimization method achieves an improvement of 15% for the Klatt model, but there is little improvement in the modified residual case.

Background: This study investigated the validity of the DNA-marker based test to determine susceptibility to ETECF4 diarrhoea by comparing the results of two DNA sequencing techniques in weaner pigs following experimental infection with F4 enterotoxigenic Escherichia coli(ETEC-F4). The effects of diet and genetic susceptibility were assessed by measuring the incidence of piglet post-weaning diarrhoea(PwD), faecal E. coli shedding and the diarrhoea ***: A DNA marker-based test targeting the mucin 4 gene(MUC4) that encodes F4 fimbria receptor identified pigs as either fully susceptible(SS), partially or mildly susceptible(SR), and resistant(RR) to developing ETEC-F4 diarrhoea. To further analyse this, DNA sequencing was undertaken, and a significantly higher proportion of C nucleotides was observed for RR and SR at the Xba I cleavage site genotypes when compared to SS. However, no significant difference was found between SR and RR genotypes. Therefore, results obtained from Sanger sequencing retrospectively allocated pigs into a resistant genotype(MUC4–), in the case of a C nucleotide, and a susceptible genotype(MUC4+), in the case of a G nucleotide, at the single nucleotide polymorphism site. A total of 72 weaner pigs(age ~ 21 days), weighing 6.1 ± 1.2 kg(mean ± SEM), were fed 3 different diets:(i) positive control(PC) group supplemented with 3 g/kg zinc oxide(Zn O),(ii) negative control(NC) group(no Zn O or HAMSA),and(iii) a diet containing a 50 g/kg high-amylose maize starch product(HAMSA) esterified with acetate. At days five and six after weaning, all pigs were orally infected with ETEC(serotype O149:F4;toxins LT1, ST1, ST2 and EAST). The percentage of pigs that developed diarrhoea following infection was higher(P = 0.05) in MUC4+ pigs compared to MUC4– pigs(50% vs. 26.8%, respectively). AIM:To identify possible predictive factors for rebleeding after angiographically negative findings in patients with acute non-variceal gastrointestinal ***: From January 2000 to July 2007, 128 patients with acute non-variceal gastrointestinal bleeding had negative f indings after initial angiography. Clinical and laboratory parameters were analyzed ***: Among 128 patients, 62 had no recurrent gastrointestinal bleeding and 66 had recurrent gastrointestinal bleeding within 30 d. As determined by the use of multivariate analysis, an underlying malignancy, liver cirrhosis and hematemesis were significant factors related to recurrent gastrointestinal ***: Clinical factors including underlying malignancy, liver cirrhosis, and hematemesis are important predictors for rebleeding after angiographically negative findings in patients with acute non-variceal gastrointestinal bleeding.
