AIM To investigate and summarise the current evidence surrounding management of Bouveret's syndrome(BS).METHODS A MEDLINE search was performed for the BS. The search was conducted independently by two clinicians(Yahya ALHabbal and Matthew ng) in April 2016. A case of BS is also *** A total of 315 articles, published from 1967 to 2016, were found. For a clinically meaningful clinical review, articles published before 01/01/1990 and were excluded, leaving 235 unique articles to review. Twenty-seven articles were not available(neither by direct communication nor through inter-library transfer). These were also excluded. The final number of articles reviewed was 208. There were 161 case reports, 13 reviews, 23 images(radiological and clinical images), and 11 letters to editor. Female to male ratio was 1.82. Mean age was 74 years. Treatment modalities included laparotomy in the majority of cases, laparoscopic surgery, endoscopic surgery and shockwave *** There is limited evidence in the literature about the appropriate approach. We suggest an algorithm for management of BS.

BACKGROUND Colorectal cancer(CRC) is a major health problem. There is minimal consensus of the appropriate approach to manage patients with positive immunochemical fecal occult blood test(iFOBT), following a recent *** To determine the prevalence of advanced neoplasia in patients with a positive iFOBT after a recent colonoscopy, and clinical and endoscopic predictors for advanced *** The study recruited i FOBT positive patients who underwent colonoscopy between July 2015 to March 2020. Data collected included demographics, clinical characteristics, previous and current colonoscopy findings. Primary outcome was the prevalence of CRC and advanced neoplasia in a patient with positive iFOBT and previous colonoscopy. Secondary outcomes included identifying any clinical and endoscopic predictors for advanced *** The study included 1051 patients(male 53.6%;median age 63). Forty-two(4.0%) patients were diagnosed with CRC, 513(48.8%) with adenoma/sessile serrated lesion(A-SSL) and 257(24.5%) with advanced A-SSL(AA-SSL). A previous colonoscopy had been performed in 319(30.3%). In this cohort, four(1.3%) were diagnosed with CRC, 146(45.8%) with A-SSL and 56(17.6%) with AA-SSL. Among those who had a colonoscopy within 4 years, none had CRC and 7 had AA-SSL. Of the 732 patients with no prior colonoscopy, there were 38 CRCs(5.2%). Independent predictors for advanced neoplasia were male [odds ratio(OR) = 1.80;95% confidence interval(CI): 1.35-2.40;P < 0.001), age(OR = 1.04;95%CI: 1.02-1.06;P < 0.001) and no previous colonoscopy(OR = 2.07;95%CI: 1.49-2.87;P < 0.001).CONCLUSION A previous colonoscopy, irrespective of its result, was associated with low prevalence of advanced neoplasia, and if performed within four years of a positive iFOBT result, was protective against CRC.

目的:针对新加坡的花岗岩地质结构,研究将典型马蹄形隧道挖掘成双拱隧道时岩石支撑的设计问题。利用离散元法分析侧边导坑对隧道主体开口的影响。创新点:1.基于平面应变假设,提出一个针对性的离散元模型以分析侧边导坑的设置对主体隧道支撑的影响;2.此模型适用于解决底下长跨度开口的支撑设计问题,如大众捷运系统隧道和紧贴式双孔隧道的建造,补充了传统经验主义的设计方法。方法:1.采用离散元法对马蹄形隧道开口支撑问题进行非连续分析;2.采用二维平面应变模型简化问题。结论:1.离散元法可用于分析有限岩石覆盖层的失效机制,单隧道支撑要求以及增加侧边导坑后的支撑要求;2.模型计算结果显示,侧边导坑的挖掘使得支柱总的螺栓连接作用力增加了一倍。

This paper presents a 591×438-DPI ultrasonic fingerprint *** sensor is based on a piezoelectric micromachined ultrasonic transducer(PMUT)array that is bonded at wafer-level to complementary metal oxide semiconductor(CMOS)signal processing electronics to produce a pulse-echo ultrasonic imager on a *** meet the 500-DPI standard for consumer fingerprint sensors,the PMUT pitch was reduced by approximately a factor of two relative to an earlier *** conducted a systematic design study of the individual PMUT and array to achieve this scaling while maintaining a high *** resulting 110×56-PMUT array,composed of 30×43-μm^(2) rectangular PMUTs,achieved a 51.7% fill-factor,three times greater than that of the previous *** with the custom CMOS ASIC,the sensor achieves 2 mV kPa^(−1) sensitivity,15 kPa pressure output,75μm lateral resolution,and 150μm axial resolution in a 4.6 mm×3.2 mm *** the best of our knowledge,we have demonstrated the first MEMS ultrasonic fingerprint sensor capable of imaging epidermis and sub-surface layer fingerprints.

Mental illness is recognized as a significant social and public health problem in China, with approximately 16 million people affected with mental illness according to a report in 2002.^1 In addition, approximately 30 million children and young adolescents are suffering from mood and/or behavioral disorders. Drug and alcohol problems and other psychosocial disorders affecting women, elders and survivors of disasters are also important problems adding to the overall national mental health burden. Research from both China and abroad indicate that mental and behavioral problems will continue to increase in the future. The World Health Organization （WHO） has warned that the financial burden from mental disorders in China will constitute 1/4 of total burden from various diseases in 2020.^1
