Bouveret syndrome masquerading as a gastric mass-unmasked with endoscopic luminal laser lithotripsy:A case report

BACKGROUND Bouveret syndrome,also known as gallstone ileus,is a rare form of gastric outlet obstruction accounting for 1%-3%of *** condition is most often reported in *** diagnosis can be challenging and is often missed due to atypical presentations,which occasionally mimic gastric outlet obstruction symptoms such as nausea,vomiting,loss of appetite and *** symptoms vary with stone *** stones are managed with a surgical approach,but this carries increased morbidity and *** the past decade,the endoscopic approach has emerged as an alternative mode of treatment,but it is generally unsuccessful in the management of larger-sized stones.A literature review revealed cases of successful endoscopic treatment requiring multiple sessions for stone sizes measuring up to about 4.5 *** we present a unique case of an elderly patient with Bouveret syndrome with a 5 cm stone mimicking a gastric mass and causing gastric outlet obstruction,who was successfully managed in a single session using a complete endoscopic approach with laser *** sUMMARY An 85-year-old female patient presented with 1-month history of intermittent abdominal pain,vomiting,decreased appetite and weight *** abdominal computed tomography showed a 4.5 cm×4.7 cm partially calcified mass at the gastric pylorus causing gastric outlet *** showed an ulcerated fistulous opening and a large 5 cm impacted gallstone in the duodenal *** nets and baskets were used in an attempt to remove the stone,but this approach was *** her advanced age,poor physical condition and underlying comorbidities,she was deemed to be high-risk for ***,a minimally invasive approach using endoscopic laser lithotripsy was attempted and successfully treated the ***-procedure,the patient experienced complete resolution of her symptoms with no complications and was able to tolerate her *** was subsequently discharged home

Constipation-predominant irritable bowel syndrome: A review of current and emerging drug therapies

Irritable bowel syndrome(IBs) is a highly prevalent medical condition that adversely affects patient quality of life and constitutes a significant economic burden on healthcare resources. A large proportion of patients suffer from the constipation subtype of IBs(IBs-C), most commonly afflicting older individuals and those with a lower socioeconomic status. Conventional pharmacologic and nonpharmacologic treatment options have limited efficacies and/or significant adverse events, which lead to increased long-term health care expenditures. Failure to effectively treat IBs-C patients over the past decades has largely been due to a poor understanding of disease pathophysiology, lack of a global view of the patient, and an inappropriate selection of patients and treatment endpoints in clinical trials. In recent years, however, more effective and safer drugs have been developed for the treatment of IBs-C. The advancement in the area of pharmacologic treatment is based on new knowledge of the pathophysiologic basis of IBs-C and the development of drugs with increased selectivity within pharmacologic classes with recognized efficacies. This narrative review covers the spectrum of available drugs and their mechanisms of action, as well as the efficacy and safety profiles of each as determined in relevant clinical trials that have investigated treatment options for IBs-C and chronic constipation. A brief summary of laxative-based treatment options is presented, followed by up-to-date assessments for three classes of drugs: prokinetics, prosecretory agents, and bile acid modulators.

Relationship of functional gastrointestinal disorders and psychiatric disorders: Implications for treatment

This article revisits the links between psychopathology and functional gastrointestinal disorders such as irritable bowel syndrome （IBs）, discusses the rational use of antidepressants as well as non-pharmacological approaches to the management of IBs, and suggests guidelines for the treatment of IBs based on an interdisciplinary perspective from the present state of knowledge. Relevant published literature on psychiatric disorders, especially somatization disorder, in the context of [Bs, and literature providing direction for management is reviewed, and new directions are provided from findings in the literature. IBs is a heterogeneous syndrome with various potential mechanisms responsible for its clinical presentations. IBs is typically complicated with psychiatric issues, unexplained symptoms, and functional syndromes in other organ systems. Most IBs patients have multiple complaints without demonstrated cause, and that these symptoms can involve systems other than the intestine, e.g. bones and joints （fibromyalgia, temporomandibular joint syndrome）, heart （non-cardiac chest pain）, vascular （post-menopausal syndrome）, and brain （anxiety, depression）. Host IBs patients do not have psychiatric illness per se, but a range of psychoform （psychological complaints in the absence of psychiatric disorder） symptoms that accompany their somatoform （physical symptoms in the absence of medical disorder） complaints. It is not correct to label IBs patients as psychiatric patients （except those more difficult patients with true somatization disorder）. One mode of treatment is unlikely to be universally effective or to resolve most symptoms. The techniques of psychotherapy or cognitive-behavioral therapy can allow IBs patients to cope more readily with their illness. specific episodes of depressive or anxiety disorders can be managed as appropriate for those conditions. Medications designed to improve anxiety or depression are not uniformly useful for psychiatric complaints in IB

Endoscopic esophageal stent placement is widely used in the treatment of a variety of benign and malignant esophageal *** expanding metal stents(sEMs)are associated with significantly reduced stent related mortality and morbidity compared to plastic stents for treatment of esophageal conditions;however they have known complications of stent migration,stent occlusion,tumor ingrowth,stricture formation,reflux,bleeding and perforation amongst others.A rare and infrequently reported complication of sEMs is stent fracture and subsequent migration of the broken *** have only been a handful of published case reports describing this *** this report we describe a case of a spontaneously fractured nitinol esophageal sEMs,and review the available literature on the unusual occurrence of sEMs fracture placed for benign or malignant obstruction in the *** fracture could be a potentially dangerous event and should be considered in a patient having recurrent dysphagia despite successful placement of an esopha-geal *** usually requires endoscopic therapy and may unfortunately require surgery for retrieval of a distally migrated *** recognition and prompt management may be able to prevent further problems.

The formation of fine,non-dendritic equiaxed grains throughout a casting without the addition of refiners(*** of alloy chemistry),is made possible by using ultrasonic,magnetic or pulsed magnetic and electric current pulse *** dominant mechanisms proposed for the grain refinement produced during the application of an external field are cavitation phenomena assisted nucleation or fragmentation of dendrites(ultrasonic field),wall crystals arising from the cold surface of the mould(electric current pulse,magnetic and pulsed magnetic fields).In all these cases fluid flow provides an additional contribution(*** temperature gradients,growth rate and remelting of dendrites)to maintaining an equiaxed grain *** origin of equiaxed grains under an external field also depends on the casting conditions(volume and shape of casting)and the type of alloy other than the mechanisms specific to a particular *** current work aims to provide a detailed understanding of the various factors and mechanisms that influence the grain refinement achieved during the solidification of pure metals(magnesium and zinc)subjected to Ultra sonic Treatment(UsT).The role of the temperature range of UsT application,time duration and an unpreheated sonotrode are examined with respect to the origin,evolution of equiaxed grain structure,morphology and the columnar to equiaxed *** origin of grains was analysed from three fundamental aspects that contribute to refinement(i)heterogeneous nucleation(ii)fragmentation of existing dendrites and(iii)grains produced from the colder surfaces(arising from mould walls or vibrating surfaces as wall crystals).A comparison of UsT refinement with mechanical,low-frequency vibration,electric current pulse and magnetic field solidification of pure metals has also been provided to highlight the importance of the cold surfaces(sonotrode and mould wall)in influencing grain refinement.

A 3-D numerical study of solitary wave interaction with vertical cylinders using a parallelised particle-in-cell solver

This paper aims to provide a better understanding of the interaction between solitary waves and vertical circular cylinders. This is achieved via process based numerical modelling using the parallel particle-in-cell based incompressible flow solver PICIN. The numerical model solves the Navier-stokes equations for free-surface flows and incorporates a Cartesian cut cell method for fluid-structure interaction. solitary waves are generated using a piston-type wave paddle. The PICIN model is first validated using a test case that involves solitary wave scattering by a single vertical cylinder. Comparisons between the present results and experimental data show good agreement for the free surface elevations around the cylinder and the horizontal wave force on the cylinder. The model is then employed to investigate solitary wave interaction with a group of eleven vertical cylinders. The wave run-up and wave forces on the cylinders are discussed.

Personal thermal management is emerging as a promising strategy to provide thermal comfort for the human body while conserving *** improving control over the heat dissipating from the human body,personal thermal management can provide effective personal cooling and ***,we propose a facile surface modification approach to tailor the thermal conduction and radiation properties based on commercially available fabrics,to realize better management of the whole heat transport pathway from the human body to the ambient.A bifunctional asymmetric fabric(BAF)offering both a cooling and a warming effect is *** to the advantages of roughness asymmetry and surface modification,the BAF demonstrates an effective cooling effect through enhanced heat conduction and radiation in the cooling mode;in the warming mode,heat dissipation along both routes is reduced for personal *** a result,a 4.6℃ skin temperature difference is measured between the cooling and warming BAF modes,indicating that the thermal comfort zone of the human body can be enlarged with one piece of BAF *** expect this work to present new insights for the design of personal thermal management textiles as well as a novel solution for the facile modification of available fabrics for both personal cooling and warming.
