Shortness of breath in clinical practice: A case for left atrial function and exercise stress testing for a comprehensive diastolic heart failure workup

The symptom cluster of shortness of breath(SOB) contributes significantly to the outpatient workload of cardiology services. The workup of these patients includes blood chemistry and biomarkers, imaging and functional testing of the heart and lungs. A diagnosis of diastolic heart failure is inferred through the exclusion of systolic abnormalities, a normal pulmonary function test and normal hemoglobin, coupled with diastolic abnormalities on echocardiography. Differentiating confounders such as obesity or deconditioning in a patient with diastolic abnormalities is difficult. While the most recent guidelines provide more avenues for diagnosis, such as incorporating the left atrial size, little emphasis is given to understanding left atrial function, which contributes to at least 25% of diastolic left ventricular filling; additionally, exercise stress testing to elicit symptoms and test the dynamics of diastolic parameters, especially when access to the "gold standard" invasive tests is lacking, presents clinical translational gaps. It is thus important in diastolic heart failure work up to understand left atrial mechanics and the role of exercise testing to build a comprehensive argument for the diagnosis of diastolic heart failure in a patient presenting with SOB.

This paper presents a survey of image processing techniques proposed in the literature forextracting key cereal crop growth metrics from high spatial resolution, typically proximalimages. The descriptive crop growth metrics considered are: crop canopy cover, aboveground biomass, leaf area index (including green area index), chlorophyll content, andgrowth stage. The paper includes an overview of relevant fundamental image processingtechniques including camera types, colour spaces, colour indexes, and image segmentation. The descriptive crop growth metrics are defined. Reference methods for groundtruth measurement are described. Image processing methods for metric estimation aredescribed in detail. The performance of the methods is reviewed and compared. The surveyreveals limitations in image processing techniques for cereal crop monitoring such as lackof robustness to lighting conditions, camera position, and self-obstruction. Directions forfuture research to improve performance are identified.

The Balfour Formation has a pronounced lithological variation that is characterized by alternating sandstone- and mudstone-dominated members. The sandstone-dominated Oudeberg and Barberskrans Members are composed of lithofacies that range from intraformationai conglomerates to fine-grained sediments, whereas the mudstone-dominated members （Daggaboersnek, Elandsberg, and Palingkloof） are dominated by the facies Fm and FI. Petrography, geochemistry, and a paleocurrent analysis indicated that the source rock of the Balfour Formation was to south east and the rocks had a transitional/dissected magmatic arc signature. The sandstones-rich members were deposited by seasonal and ephemeral high-energy, low-sinuous streams, and the fine-grained-rich members were formed by ephemeral meandering streams. The paleoclimates have been equated to present temperate climates; they were semiarid becoming arid towards the top of the Balfour Formation. This has been determined by reconstructing the paleolatitude of the Karoo Basin, geochemistry, paleontology, sedimentary structures, and other rock properties, like color.

AIM: To investigate the performance of Gadofluorine P-enhanced magnetic resonance imaging(MRI) on the diagnosis of diabetes in a streptozotocin(STZ)-induced diabetic rat ***: Fischer 344 rats were treated with *** not treated with STZ served as controls. T1-weighted MRI was performed using a 3T scanner before and after the injection of Gd-DOTA or Gadofluorine P(6 diabetic rats, 5 controls). The normalized signal intensity(SI) and the enhancement ratio(ER) of the pancreas were measured at each time point, and the values were compared between the normal and diabetic rats using the Mann-Whitney test. In addition,the values were correlated with the mean islet *** cut-off values were calculated using a positive test based on receiver operating *** Gd concentration after the injection of each contrast media was measured using laser ablation-inductively coupled plasma-mass spectrometry in a separate set of rats(4 diabetic rats, 4 controls for Gadofluorine P; 2, 2 for Gd-DOTA).RESUlTS: The normalized SI and ER of the pancreas using Gd-DOTA were not significantly different between diabetic rats and controls. With Gadofluorine P, the values were significantly higher in the diabetic rats than in the control rats 30 min after injection(P l group was greater for Gadofluorine P than for Gd-DOTA(0.967 vs 0.667, P = 0.085). An increase in normalized SI 30 min after Gadofluorine P was correlated with a decrease in the mean number of islets(r 2 = 0.510, P = 0.014). In vitro assessment of the synergism between extracts of Cocos nucifera husk and some standard antibiotics

Objective: To evaluate the interactions between the crude extracts of Cocos nucifera(***) and six front line antibiotics(ampicillin sodium salt, penicillin G sodium,amoxicillin, chloramphenicol, ciprofloxacin and tetracycline hydrochloride), against some bacterial pathogens linked with human ***: The pulverized husk of *** was dissolved in 95% n-hexane and extracted using Soxhlet extraction method and sterile distilled water(aqueous).The antibacterial susceptibility of the crude extracts of *** was tested against environmental and clinical strains(6) obtained from the South African Bureau of Standards(SABS), Vibrio(6) and listeria pathogens(6).The agar-well diffusion method was used for screening the extracts for their antibacterial *** minimum inhibitory concentration and minimum bactericidal concentration of the extracts were ***-kill assay was used to evaluate bactericidal and/or bacteriostatic *** synergistic effect of the crude extracts and antibiotics was assessed and evaluated by adopting the checkerboard ***: With the time-kill assay, the highest bactericidal activity was observed on Vibrio fluvialis El041 with a-5.6 ± 0.2 log_(10)CFU/ml decrease in cell density as a result of the combination of the extracts and chloramphenicol at two-fold minimum inhibitory *** using the time-kill assay constituted about 72%, while indifference constituted about 28%.The checkerboard method revealed synergistic interaction in 67% of the combinations, and indifference in 33%.There was no specificity in the observed synergy to a particular class of ***: This investigation suggests the crude extracts of *** to be a potential broad spectrum antimicrobial ***, further study is needed to isolate the pure compounds from these crude extracts.
