Disagreement in middle ear volume estimation between tympanometry and three-dimensional volume reconstruction in the context of tympanic membrane perforation

Introduction: Middle ear volume(MEV) is a clinically relevant parameter across middle ear diseases. MEV values between these techniques have never before been tested for agreement in ears with perforated tympanic membranes(TMs).Methods: Middle ears were identified from 36 patients ranging 18-89 years of age with TM perforations who underwent tympanometry and temporal bone computed tomography(CT) between 2005 and 2015. MEVs calculated by both tympanometry and three-dimensional volume reconstruction(3DVR) were analyzed for agreement using Bland Altman plots. The differences between tympanometric and 3DVR MEV values for each given middle ear were characterized across MEV quartiles(1= smallest; 4= largest) and across increasing states of middle ear disease using Kruskale Wallis and Wilcoxon testing with Bonferroni ***: Bland Altman plots demonstrated significant disagreement between MEV measurement techniques. Differences between tympanometric(T) and 3DVR MEV values were significantly greater with increasing average(i.e.(Tt3DVR)/2)) MEV per linear regression(p < 0.0001). Significance was demonstrated between fourth and first average MEV quartiles(p= 0.0024), fourth and second quartiles(p= 0.0024), third and first quartiles(p= 0.0048), and third and second quartiles(p= 0.048). Absolute MEV difference was not significantly different across varying states of middle ear disease(p= 0.44).Conclusion: Statistically and clinically significant disagreement was demonstrated between tympanometric and 3DVR MEV values. Studies that vary in MEV estimation techniques may be expected to demonstrate significantly different results. These preliminary results suggest that clinicians should endeavor to seek further confirmation when interpreting high tympanometric MEV values.

The anatomic determinants of conductive hearing loss secondary to tympanic membrane perforation

Objectives: Recent studies have introduced middle ear volume(MEV) as a novel determinant of perforation-induced conductive hearing loss(CHL) in a mechanism driven by trans-tympanic membrane pressure differences. The primary aims of this preliminary report are to: 1) correlate CHL with perforation size; 2) describe the relationship between CHL and MEV; and 3) compare CHL across a range of cholesteatoma ***: A retrospective pilot study was performed in 31 subjects with audiometry indicative of conductive hearing loss, temporal bone CT scans,and no prior middle ear surgery. Perforation size and MEV were analyzed with respect to CHL in a cohort of 10 perforated ears with no cholesteatoma. CHLs were compared in 3 groups defined by extent of cholesteatoma ***: Ears with large and small perforations showed mean ABG values of 32.0 ± 15.7 dB and 16.0 ± 16.4 dB, respectively. A direct relationship was observed between MEV and CHL for ears with large perforations across all frequencies, whereas this relationship for small perforations was frequency-dependent. Finally, a statistically significant increase in CHL was found across ears with increasing cholesteatoma involvement at 1000 Hz(X^2(2) = 9.786, p = 0.008),2000 Hz(x^2(2) = 8.455, p = 0.015),and 4000 Hz(x^2(2)= 8.253, p = 0.016).Conclusions: These pilot data suggest that greater perforation-induced conductive hearing losses may be associated with larger perforation sizes and cholesteatoma. The correlation between MEV and CHL may require additional study.

Karst aquifers supply a significant fraction of the world's drinking water. These types of aquifers are alsohighly susceptible to pollution from the surface with recharge usually occurring through fractures and solution openings at the bedrock surface. Thickness of the protective soil cover, macropores and openings within the soil cover, and the nature of the weathered bedrock surface all influence infiltration. Recharge openings at the bedrock surface, however, are often covered by unconsolidated sediments, resulting in the inadvertent placement of landfills, unregulated dump sites, tailing piles, waste lagoons and septic systems over recharge zones. In these settings surface geophysical surveys, calibrated by a few soil cores, could be employed to identify these recharge openings, and qualitatively assess the protection afforded by the soil cover. In a test of this hypothesis, geophysical measurements accurately predicted the thickness of unconsolidated deposits overlying karstic dolomite at a site about 100 km south of Chicago, Illinois. Zones of elevated electrical conductivity and high ground-penetrating radar (GPR) attenuation within the sediments coincided with subcropping solutionally-enlarged hydraulically active bedrock fractures. These fractures extend to over 12-m depth, as shown by 2-D inverted resistivity sections and soil coring. Anomalous electromagnetic (EM) conductivity and GPR response may be due to higher soil moisture above these enlarged fractures. An epikarstal conduit at 2.5-m depth was directly identified through a GPR survey. These results suggest that surface geophysical surveys are a viable tool for assessing the susceptibility of shallow karst aquifers to contamination.

In this paper,we investigate the impact of coronary artery dynamics on the wall shear stress(WSS)vector field topology by comparing fluid–structure interaction(FSI)and computational fluid dynamics(CFD)*** one of the most common causes of death globally,coronary artery disease(CAD)is a significant economic burden;however,novel approaches are still needed to improve our ability to predict its *** can include the unique dynamical factors present in the coronary *** investigate the impact of these dynamical factors,we study an idealized artery model with sequential *** transient simulations made use of the hyperelastic artery and lipid constitutive equations,non‐Newtonian blood viscosity,and the characteristic out‐of‐phase pressure and velocity distribution of the left anterior descending coronary *** compare changes to established metrics of time‐averaged WSS(TAWSS)and the oscillatory shear index(OSI)to changes in the emerging WSS divergence,calculated here in a modified version to handle the deforming mesh of FSI *** suggest that the motion of the artery can impact downstream patterns in both divergence and *** magnitude is also decreased by up to 57%due to motion in some *** divergence patterns varied most significantly between simulations over the systolic period,the time of the largest *** investigation highlights that coronary dynamics could impact markers of potential CAD progression and warrants further detailed investigations in more diverse geometries and patient cases.

Temporal bone anatomy characteristics in superior semicircular canal dehiscence

Introduction: Superior semicircular canal dehiscence(SCD) remains difficult to diagnose despite advances in high-resolution computed tomography(HRCT) imaging. We hypothesize possible associations between gross temporal bone anatomy and sub-millimeter pathology of the semicircular canals, which may supplement imaging and clinical suspicion. This pilot study investigates differences in gross temporal bone anatomic parameters between temporal bones with and without ***: Records were reviewed for 18 patients referred to an otology clinic complaining of dizziness with normal caloric stimulation results indicative of non-vestibular findings. Eleven patients had normal temporal bone anatomy while seven had SCD. Three-dimensional reconstruction of every patient's temporal bone anatomy was created from patient-specific computational tomography images. Surface area(SA),volume(V), and SA to V ratios(SA:V) were computed across temporal bone anatomical ***: SCD temporal bones have significantly smaller V, and larger temporal bone SA. Mean(±SD) V was 21,484 ± 3,921 mm^3 in temporal bones without SCD and 16,343 ± 34,471 mm^3 for those with SCD. Their respective SA were 13,733 ± 1,603 mm^2 and 18,073 ± 3,002 mm^*** bone airspaces and lateral semicircular canals did not demonstrate significant differences where SCD was and was not present. Plots of MV_(warm)response against computed SCD temporal bone anatomic parameters(SA, V and SA:V) showed moderate to strong correlations:temporal bone SA:V(r= 0.64), temporal bone airspace V(r= 0.60), temporal bone airspace SA(r= 0.55), LSCC SA(r= 0.51), and LSCC-toTM Distance(r= 0.65).Conclusions: This analysis demonstrated that SCD is associated with decreased temporal bone volume and density. The defect in SCD does not appear to influence caloric responses.

Apparent Formation Factor for Leachate-Saturated Waste and Sediments: Examples from the USA and China

The formation factor relates bulk resistivity to pore fluid resistivity in porous materials. Understanding the formation factor is essential in using electrical and electromagnetic methods to monitor ieachate accumulations and movements both within and around landfills. Specifically, the formation factor allows leachate resistivity, the degree of saturation, and, possibly, even the hydraulic conductivity of the waste to be estimated from non-invasive surface measurements. In this study, apparent formation factors are computed for three landfills with different types of waste as well as sediments contaminated by landfill leachate. Resistivity soundings at the closed Mallard North landfill in suburban Chicago （Illinois, USA） mapped leachate surfaces that were confirmed by monitoring wells. The resistivity of leachate-saturated waste from resistivity sounding inversions was then divided by the leachate resistivity values measured in-situ to compute apparent formation factors （Fa） ranging from 1.6 to 4.9. A global Fa of 3.0±1.9 was computed for the entire monitored portion of this landfill. At a nearby mixed laboratory waste landfill, a 2D inverted resistivity section was used to compute an Fa of 2.9. Finally, a distinctly different Fa value of 10.6±2.8 was computed for leachate-saturated retorted oil shale wastes north of Maoming （茂名）, Guangdong （广东） Province, China. Shallow aquifers in the Laohuling （老虎岭） Formation near this landfill are polluted by acidic leachate containing heavy metals and organic compounds. The Fa for aquifers containing contaminated groundwater fall in the same range as aquifers with normal groundwater, 1.7-3.9. However, models from inverted sounding curves over these contaminated areas exhibit unusually low resistivity layers, which may be diagnostic of contamination.

High-power,electronically controlled source of user-defined vortex and vector light beams based on a few-mode fiber amplifier

Optical angular momentum(OAM)-based structured light beams provide an additional degree of freedom for practical applications ranging from optical communication to laser-based material processing. Many techniques exist for generating such beams within laser sources and these primarily rely upon the use of specially designed optical components that limit laser power scaling and ready tunability of the topological charge and polarization of the output OAM beams. Here we show that some of these limitations can be overcome by employing a computer controlled reflective phase-only spatial light modulator to adaptively tailor the input(and subsequent output) beam wavefront and polarization in a few-mode fiber amplifier. In this way modal-coupling-induced beam distortion within the fiber amplifier can be mitigated and we are able to generate at will any desired supported spatial mode guided in the fiber, including conventional linearly polarized(LP) modes, scalar OAM modes, and cylindrical vector modes, at average powers >10 W and with a peak power of >11 kW. Our results pave the way to the realization of practical high-power structured laser sources with tunable chirality and polarization.

Maintaining nano-lamellar microstructure in friction stir welding (FSW) of accumulative roll bonded (ARB) Cu-Nb nano-lamellar composites (NLC)

Accumulative roll bonded （ARB） Copper Niobium （Cu-Nb） nano-lamellar composite （NLC） panels were friction stir welded （FSWed） to evaluate the ability to join panels while retaining the nano-lamellar structure. During a single pass of the friction stir welding （FSW） process, the nano-lamellar structure of the parent material （PM） was retained but was observed to fragment into equiaxed grains during the second pass. FSW has been modeled as a severe deformation process in which the material is subjected to an instantaneous high shear strain rate followed by extreme shear strains. The loss of the nano-lamellar layers was attributed to the increased strain and longer time at temperature resulting from the second pass of the FSW process. Kinematic modeling was used to predict the global average shear strain and shear strain rates experienced by the ARB material during the FSW process. The results of this study indicate that through careful selection of FSW parameters, the nano-lamellar structure and its associated higher strength can be maintained using FSW to join ARB NLC panels.

Predictors of clinical outcomes after non-operative management of symptomatic full-thickness rotator cuff tears
