Background:Much has been written about the loss to follow-up in the transition between pediatric and adult Congenital Heart Disease(CHD)care *** less is understood about the loss to follow-up(LTF)after a successful *** is critical too,as patients lost to specialised care are more likely to experience mor-bidity and premature ***:To understand the prevalence and reasons for loss to follow-up(LTF)at a large Australian Adult Congenital Heart Disease(ACHD)***:Patients with moderate or highly complex CHD and gaps in care of>3 years(deﬁned as LTF)were identiﬁed from a comprehensive ACHD *** telephone interviews examined current care and barriers to clinic ***:Overall,407(22%)of ACHD patients(n=1842)were *** mean age at LTF was 31(SD 11.5)years and 54%were male;311(76%)were *** to adults seen regularly,lost patients were younger,with a greater socio-economic disadvantage,and had less complex CHD(p<0.05 for all).We interviewed 59 patients(14%).The top 3 responses for care absences were“feeling well”(61%),losing track of time(36%),and not needing fol-low-up care(25%).Conclusions:A large proportion of the ACHD population becomes lost to specialised cardiac care,even after a successful *** Australian study reports younger age,moderate complexity defects,and socio-economic disadvantage as predictive of loss to *** study highlights the need for novel approaches to patient-centered service delivery even beyond the age of transition and resources to maintain patient engagement within the ACHD service.

Impact of cancer diagnosis on life expectancy by area-level socioeconomic groups in New South Wales, Australia: a population-based study

Objective: Improvement in cancer survival over recent decades has not been accompanied by a narrowing of socioeconomic disparities. This study aimed to quantify the loss of life expectancy(LOLE) resulting from a cancer diagnosis and examine disparities in LOLE based on area-level socioeconomic status(SES).Methods: Data were collected for all people between 50 and 89 years of age who were diagnosed with cancer, registered in the NSW Cancer Registry between 2001 and 2019, and underwent mortality follow-up evaluations until December 2020. Flexible parametric survival models were fitted to estimate the LOLE by gender and area-level SES for 12 common ***: Of 422,680 people with cancer, 24% and 18% lived in the most and least disadvantaged areas, respectively. Patients from the most disadvantaged areas had a significantly greater average LOLE than patients from the least disadvantaged areas for cancers with high survival rates, including prostate [2.9 years(95% CI: 2.5±3.2 years) vs. 1.6 years(95% CI: 1.3±1.9 years)] and breast cancer [1.6 years(95% CI: 1.4±1.8 years) vs. 1.2 years(95% CI: 1.0±1.4 years)]. The highest average LOLE occurred in males residing in the most disadvantaged areas with pancreatic [16.5 years(95% CI: 16.1±16.8 years) vs. 16.2 years(95% CI: 15.7±16.7 years)] and liver cancer [15.5 years(95% CI: 15.0±16.0 years) vs. 14.7 years(95% CI: 14.0±15.5 years)]. Star clusters were historically considered simple stellar populations,with all stars sharing the same age and initial chemical ***,the presence of chemical anomalies in globular clusters(GCs),called multiple stellar populations(MPs),has challenged star formation theories in dense *** studies show that mass,metallicity,and age are likely controlling parameters for the manifestation of *** the limit between clusters with/without MPs in physical parameter space is crucial to reveal the driving mechanism behind their *** this study,we look for MP signals in Whiting 1,which is traditionally considered a young *** the Magellan telescope,we obtained low-resolution spectra withinλλ=3850-5500?for eight giants of Whiting *** measured the C and N abundances from the CN and CH spectral indices.C and N abundances have variations comparable with their measurement errors(~0.1 dex),suggesting that MPs are absent from Whiting *** these findings with literature studies,we propose a limit in the metallicity *** compactness index parameter space,which relatively clearly separates star clusters with/without MPs(GCs/open clusters).This limit is physically *** a larger scale,the galactic environment determines cluster compactness and metallicity,leading to metal-rich,diffuse,old clusters formed ex *** proposed limit also impacts our understanding of the formation of the Sagittarius dwarf galaxy:star clusters formed after the first starburst(age≤8-10 Gyr).These clusters are simple stellar populations because the enriched galactic environment is no longer suitable for MP formation.
