BACKGROUND Statins have an important and well-established role in the prevention of atherosclerotic cardiovascular disease(ASCVD).However,several studies have reported widespread underuse of statins in various practice settings and *** of relevant literature reveals opportunities for improvement in the implementation of guideline-directed statin therapy(GDST).AIM To examine the impact of cardiologist intervention on the use of GDST in the ambulatory *** Patients with at least one encounter at the adult Internal Medicine Clinic(IMC)and/or Cardiology Clinic(CC),who had an available serum cholesterol test performed,were *** 2 comparison groups were defined as:(1)Patients only seen by IMC;and(2)Patients seen by both IMC and *** were excluded if variables needed for calculation of ASCVD risk scores were lacking,and if demographic information lacked guideline-directed treatment *** were analyzed using student t-tests or χ^2,as *** of Variance was used to compare rates of adherence to *** A total of 268 patients met the inclusion criteria for this study;211 in the IMC group and 57 in the IMC-CC ***,56%of patients were female,mean age 56 years(±10.65,SD),22%Black or African American,56%Hispanic/Latino,14%had clinical ASCVD,13%current smokers,66%diabetic and 63%*** use was observed in 55%(n=147/268)of the entire patient *** the IMC-CC group,73.6%(n=42/57)of patients were prescribed statin therapy compared to 50.7%(n=107/211)of patients in the IMC group(P=0.002).In terms of appropriate statin use based on guidelines,there was no statistical difference between groups[IMC-CC group 61.4%(n=35/57)vs IMC group,55.5%(n=117/211),P=0.421].Patients in the IMC-CC group were older,had more cardiac risk factors and had higher proportions of non-white patients compared to the IMC group(P<0.02,all).CONCLUSION Although overall use of GDST was suboptimal,there was no statis

The impact of direct-acting antivirals on hepatitis C associated hepatocellular carcinoma

The increased incidence of hepatocellular carcinoma(HCC)in the last several decades in the United States and worldwide has partly resulted from an increase in hepatitis C virus(HCV)*** carcinogenesis is speculated to be indirectly related to multiple steps from inflammation to fibrosis and advanced fibrosis/cirrhosis over 20 or more ***,the direct carcinogenic potential from HCV may explain HCC occurring in non-cirrhotic HCV *** potent direct-acting antivirals(DAAs)in recent years have changed hepatitis C treatment significantly and have resulted in the sustained virologic response(SVR)rate exceeding 90%.Although initial reports concerned the increase in de novo and recurrent HCC associated with DAAs,more recent studies showed that DAA-induced SVR on the contrary reduced risk of HCV-associated HCC without increasing its *** International Consortium of Hepatitis C Therapeutic Registry and Research Network(HCV-TARGET)database and other resources of HCV patients treated with DAA collectively in the near future most likely will be able to show definitive evidence on the risk of HCC occurrence and recurrence after DAA with *** long-term risk of HCC in chronic hepatitis C patients with advanced fibrosis or cirrhosis remains high after DAAs with ***,HCC surveillance on this sub-group of patients is important for early detection and intervention of HCC.

Despite the many submarine telecommunications and power cables laid world-wide there are fewer than ten published studies of their environmental effects in the refereed *** paper describes an investigation into the effects of laying and operating the basslink High Voltage Direct Current(HVDC)cable and its associated metallic return cable across bass Strait in South East *** more than 95%of its length the cable was directly laid into a wet jetted trench given the predominantly soft sediments *** remote video investigations found that within two years all visible evidence of the cable and trench was gone at over a third of the transects at six deep water sites(32-72 m deep).At other deep water transects the residual trench trapped drift material providing habitat for the generally sparsely distributed benthic *** surveys at both of the near shore sites(<15 m deep)on the northern side of the Strait also found the cable route was undetectable after a *** the southern side,where the cable traversed hard basalt rock near shore,it was encased in a protective cast iron half *** studies by divers over 3.5 years demonstrated the colonization of the hard shell by similar species occupying hard substrates elsewhere on the basalt *** field strengths associated with the operating cable were found to be within 0.8%of those predicted from theory with strength dropping rapidly with distance from the *** 20 m the field was indistinguishable from background.

Evaluation of Human Impacts on Bartlett Pond Ecosystem, Laredo, Southern Texas, USA, through Empirical Modeling

The trace elements chemistry of Bartlett Pond, a small shallow wetland pond in Laredo, Southern Texas, was sampled to evaluate the dynamics of trace elements impacts on water quality and ecosystems ecology of the pond. Two types of fish (bass and tilapia) were also sampled to see the trace element accumulation in different parts of their body. The concentrations of trace elements in water samples were found in the following order: Fe ≫Sb > Pb > As ≫Co > Tl > Cr > Cd within Bartlett Pond. Overall, the water quality of the pond is unacceptable for drinking and any other purposes as trace element concentrations (e.g. As, Cd, Co, Cr, Pb, Fe, Sb and Tl) are exceedingly higher (several fold) than the WHO and US EPA guidelines. Predictive and correlation analysis shows that most trace elements exhibit a strong positive correlation among them indicating the same anthropogenic sources and biogeochemical processes regulate these trace elements within the pond. Distributions of the trace elements in water exhibit different shapes mostly as positively skewed distribution for As, Cd, Co, Cr, and Tl, symmetrical distribution for Fe and almost symmetrical distribution for Pb and Sb. Concentrations of As, Co and Tl accumulated much higher in different parts of the bass than Tilapia fish. The concentrations of As, Tl, Co, and Sb appeared significantly higher in different parts of the body of both bass and Tilapia than the maximum SRM certified values. Accumulation of these contaminants in fish tissues pose increased health risks to humans who consume these contaminated fish although fishing is prohibited. Anthropogenic activities in the region primarily degrade the whole pond ecosystem ecology of the Bartlett Pond and waters of this pond to be not recommended for any use. These findings may be useful for the scientific community and concerned authorities to improve understanding about these precious natural resources and conservation of the ecosystem ecology.
