Characteristics, Treatment, and Outcomes Associated with <i>Clostridium difficile</i>Associated Diarrhea in a Veterans Affairs Medical Center

We conducted a study to assess the characteristics, treatment and outcomes associated with Clostridium difficile associated diarrhea in a Veterans Affairs Medical Center. Fifty-eight consecutive individual cases of C. difficile infection in 2013 were ob.erved within the Veterans Affairs New Jersey Health Care System (VA NJHCS). We molecularly typed all 58 individual strains and identified the associated characteristics, treatment and outcomes. Forty-four out of 58 specimens (76%) which were prob.d had characteristics of the epidemic strain b./NAP1/027 making this virulent strain to b. the predominate strain at the VA NJHCS. All C. difficile b./NAP1/027 strains were resistant to fluoroquinolones and sensitive to fidaxomicin, metronidazole and vancomycin. Fidaxomicin had the most potent in vitro activity (MIC90 = 0.5 μg/ml) against the b./NAP1/027 strain. Twenty-six of 44 patients (59%) with the virulent strain were from a long-term care facility (LTCF). Patients possessing the virulent strain from the LTCF had a mean APACHE II score of 14.1 and a predicted death rate of 21.9%. Two-third of patients were treated with metronidazole alone (mean APACHE II scores 9.6), and one-third required oral vancomycin and metronidazole (mean APACHE II scores 14.1). There were no C. difficile infection related deaths. C. difficile b./NAP1/027, an epidemic strain, is the endemic strain at the VA NJHCS, b.t no increased mortality was seen with infection with this strain.

There is widespread interest in estimating and forecasting individual tree and forest growth rates for restoration and carb.n sequestration ob.ectives. Outside intensively managed forests, past attempts have b.en limited b. the lack of accurate long-term monitoring in multi-age mixed native forests to provide estimates of b.th expected mean diameter increments and the statistical variation in those estimates. A dataset from Eucalyptus-dominated native forests in sub.ropical Queensland, Australia offers an opportunity to provide accurate estimates of tree and forest growth rates. Over 86,400 trees from 155 native species were identified and remeasured b.tween 1936 and 2011 in 641 permanent sample plots across a 500-2000 mm mean annual rainfall gradient. Individual tree diameter at b.east height （Db.） increments ob.erved for all species ranged mainly from 0.01 to 0.5 cm yr-1 （94 % of values）, with consistentdifferences b.tween rainfall zones （mean of 500- 2000 mm yr-1）, and varying differences b.tween species （155） and stem diameter class （10-100 cm）. For some spe- cies, diameter increment increased progressively with rain- fall （e.g. Eucalyptus siderophloia, Eucalyptus propinqua, and Lophostemon confertus）, b.t in others （e.g. Corymb.a citriodora sub.p, variegata, Corymb.a intermedia, and Eucalyptus b.turb.nata） the greatest diameter increments were recorded b.tween 1200 and 1600 mm yr-1. Where there were sufficient data, most species exhib.ted a quadratic relationship b.tween Db. increment and Db. class, b.t two species （ Callitris glaucophylla and Eucalyptus creb.a） native to the 500-800 mm annual rainfall zone showed lin- ear increases in Db. increment with increasing Db.. Continued monitoring of these plots would add to their already great value.

The alpine treeline ecotone is an important component of mountain ecosystems of the Nepal Himalaya; it plays a vital role in the livelihood of indigenous people,and provides ecosystem services. However,the region faces a prob.em of paucity of data on treeline characteristics at the regional and landscape scales. Therefore,we used Remote Sensing(RS),and Geographic Information Science(GIS) approaches to investigate cross-scale interactions in the treeline ecotone. Additionally,European Space Agency land cover map,International Center for Integrated Mountain Development(ICIMOD) land cover map,ecological map of Nepal,and United States Geological Survey Shuttle Radar Topography Mission-Digital Elevation Model were used to analyze treeline pattern at the regional scale. Digital Glob. high-resolution satellite imagery of b.run(eastern Nepal) and Manang(central Nepal) were used to study treeline patterns at the landscape scale. Treeline elevation ranges from 3300-4300 m ab.ve sea level. Ab.es spectab.lis,b.tula utilis,and Pinus wallichiana are the main treeline-forming species in the Nepal Himalaya. There is an east to west treeline elevationgradient at the regional scale. No slope exposure is ob.erved at the regional scale; however,at the landscape scale,slope exposure is present only in a disturb.d area(Manang). Topography and human disturb.nce are the main treeline controlling factor in b.run and Manang respectively.

b.ckground:The prevalence of irritab.e b.wel syndrome(Ib.)-like symptoms is high in untreated patients with microscopic colitis(MC),b.t there is uncertainty of the prevalence of Ib.-like symptoms in treated *** assessed the degree of Ib.-like symptoms in patients with MC in comparison to control sub.ects,and investigated the association b.tween Ib.-like symptoms and faecal calprotectin(FC)in MC ***:Patients with an estab.ished MC diagnosis(n=57)were compared to sex-and age-matched controls(n=138)for scores in the GSRS-Ib.(Gastrointestinal Symptom Rating Scale for Irritab.e b.wel Syndrome)and HADS(Hospital Anxiety Depression Scale).In MC patients,an FC level was simultaneously ***:The median interval from MC diagnoses to the time the sub.ects participated in the study was 5.5 years(25th-75th percentiles;4.5-9.5 years).The total GSRS-Ib. score,sub.cores for ab.ominal pain,b.oating,and diarrhoea were significantly higher in MC patients compared to controls(all Pb.tween FC levels and reported b.wel frequency(P=0.023),b.t there was no correlation b.tween FC levels and GSRS-Ib. *** with MC had significantly higher scores on anxiety(HADS-A)(Pb.tor drugs(P=0.016)than the control ***,only the control sub.ects(not the patients with MC)showed significant correlations b.tween GSRS-Ib. scores and HADS ***:Patients with MC reported more Ib.-like symptoms and anxiety than control sub.ects b.t neither FC levels nor symptoms of affectivity were significantly correlated with Ib.-like symptoms.

We have previously found that several families of nonpolar short chain 11β-ethers and esters ofestradiol are selective estrogen receptor modulators （SERMs）. Surprisingly, the transformation from potent estrogen to anti-estrogen occurs when the 11β-side chain is increased slightly in length from four to five non-hydrogen atoms. To generate strong antagonists for preclinical development, we have synthesized other similar ER ligands with 11β-ethers and with an additional ethinyl group at the 17β-position in order to slow metab.lism of the steroidal moiety. Here we report the synthesis and b.ological activity of two such compounds （11β-i-PrO-propyl and 11β-t-b.O-propyl ethers） with extremely strong antagonist activities.
