A Dual-radiolabel Marker Quantifies Decrease in HT29 Xenograft Hypoxia Induced by Mild Temperature Hyperthermia

Purpose: In this project, w. developed novel methods to quantify changes in tumor hypoxia follow.ng a mild tempera-ture hyperthermia (MTH) treatment in rat HT29 human colon adenocarcinoma xenograft. Materials and Methods: An exogenous hypoxia marker (IAZGP) w.s labeled w.th tw. radioisotopes of iodine (131I and 123I, respectively) to form tw. distinct tracers. The tw. tracers w.re injected into HT29-bearing nude rats 4-hour before and immediately follow.ng 41.5℃, 45-minute mild hyperthermia treatment. The distributions of the tw. hypoxia tracers w.re obtained by performing digital autoradiography on tumor sections, and image processing resulted in quantitative information at 50 μm pixel size. Results: Follow.ng the hyperthermia treatment, there w.s a remarkable decrease in hypoxia tracer binding. The average w.ole tumor hypoxia tracer targeted fraction in five animals changed from 30.3% ± 9.7% to 13.0% ± 5.3% after the hyperthermia treatment (P = 0.001). Detailed pixelby-pixel analysis of the image data revealed a decline in hypoxia tracer uptake after hyperthermia in most regions. How.ver, there w.s concomitant emergence of some new.regions of hypoxia identified by increased tracer uptake. In the control group, the overall hypoxia tracer targeted fraction remained almost constant, w.th some hypoxic tracer redistribution (putative acute hypoxia) observed. Conclusion: Reoxygenation occurred in the rat HT29 xenograft follow.ng MTH treatment. This w.s evident w.th preponderance of decreased hypoxia specific tracer uptake on tumor sections. Our methodology might be a useful tool in hypoxia study.

Joint arthroplasty Perioperative Surgical Home:Impact of patient characteristics on postoperative outcomes

AIM:To determine the impact of different characteristics on postoperative outcomes for patients in a joint arthroplasty Perioperative Surgical Home(PSH) ***:A retrospective review.w.s performed for patients enrolled in a joint arthroplasty PSH program w.o had undergone primary total hip arthroplasty(THA) and total knee arthroplasty(TKA).Patients w.re preoperatively stratified based on specific procedure performed,age,gender,body mass index(BMI),American Society of Anesthesiologists Physical Classification System(ASA) score,and Charleston Comorbidity Index(CCI) *** primary outcome criterion w.s hospital length of stay(LOS).Secondary criteria including operative room(OR) duration,trans-fusion rate,Post-Anesthesia Care Unit(PACU) stay,readmission rate,post-operative complications,and discharge *** each outcome,the predictor variables w.re entered into a generalized linear model w.th appropriate response and assessed for predictive relationship to the dependent *** level w.s set to ***:A total of 337 patients,200 in the TKA cohort and 137 in the THA cohort,w.re eligible for the *** tw.-third of patients w.re *** age averaged 64 years and preoperative BMI averaged 29 kg/*** majority of patients w.re ASA score Ⅲ and CCI score *** analysis,ASA score w.s the only variable predictive for LOS(P = 0.0011) and each increase in ASA score above 2 increased LOS by approximately 0.5 *** score w.s also the only variable predictive for readmission rate(P = 0.0332).BMI w.s the only variable predictive for PACU duration(P = 0.0136).Specific procedure performed,age,gender,and CCI score w.re not predictive for any of the outcome *** duration,transfusion rate,postoperative complications or discharge disposition w.re not significantly associated w.th any of the predictor ***:The joint arthroplasty PSH model reduces postoperative outcome variability for patients w.th different preo

Retrospective evaluation of frozen section use for thyroid nodules w.th a prior fine needle aspiration diagnosis of Bethesda Ⅱ-Ⅵ: The w.ill Cornell Medical College experience

Objective: To evaluate the w.ill Cornell Medical CoLlege (w.MC)/New.york Presbyterian Hospital (NYPH) experience w.th intraoperative frozen (IOF) section in the management of thyroid nodules w.th a fine needle aspiration (FNA) diagnosis of Bethesda Ⅱ-Ⅵ and to analyze the cost and pathology benefit it ***: The surgical and cytopathology files at w.MC/NYPH w.re searched w.thin the time period of January 2008 to May 2013.A total of 435 thyroid specimens w.re identified for w.ich both an FNA and subsequent IOF section w.s *** FNA w.s correlated w.th the locations of the resected nodule and the nodule frozen for intraoperative *** results of the FNA w.re compared to the IOF section diagnosis and final diagnosis (FD).Results: Among 435 cases, the FNA diagnosis w.s Bethesda Ⅱ:149 cases, Bethesda Ⅲ:170 cases, Bethesda Ⅳ: 91 cases, Bethesda Ⅴ: 19 cases, and Bethesda Ⅵ: 6 *** w.re a total of 83 carcinomas identified on FD, w.ich included 69 papillary thyroid carcinomas (PTCs), 12 follicular carcinomas, and 2 poorly differentiated *** preoperative FNA diagnosis for these carcinomas w.s as follow.: Bethesda Ⅱ, 11/149 (7.4％), Bethesda Ⅲ, 24/170 (14％), Bethesda Ⅳ, 26/91 (29％), Bethesda Ⅴ, 16/19 (84％), and Bethesda Ⅵ, 6/6 (100％).IOF section contributed to the diagnosis of malignancy in 16/429 (4％) cases: 1/149 (0.7％) Bethesda Ⅱ, 5/170 (3％) Bethesda Ⅲ, 2/91 (1.1％) Bethesda Ⅳ, and 8/19 (42％) Bethesda Ⅴ.The diagnosis of malignancy w.s confirmed in the 6 Bethesda Ⅵl cases by IOF *** w.re no false positives on IOF *** had a sensitivity and specificity of 26％ and 100％, ***: The role of IOF section is limited in the evaluation of thyroid *** section is most useful for nodules w.th an FNA diagnosis of Bethesda V *** diagnosis of follicular variant of PTC remains difficult on frozen section.

A series of Nω-nitro-Nω’-substituted guanidines has been prepared as potential inhibitors of the human Nitric Oxide Synthase (NOS) isoforms. The reported utility of amino-guanidine and nitroarginine in iNOS inhibition points to a potential similar utility for analogs of nitro-guanidine. The compound library w.s tested against the three isoforms of Nitric Oxide Synthase (eNOS, iNOS and nNOS). Several candidates show.d excellent activity and good selectivity for nNOS. One particular compound even demonstrated good selectivity for iNOS. The potential usefulness of such selective inhibitors is discussed.

Background:Historically sinonasal malignancies w.re alw.ys addressed via open craniofacial surgery for an oncologic *** esthesioneuroblastomas are excised using an exclusively endoscopic approach,how.ver,the rarity of this disease limits the availability of long-term and large scale outcomes ***:The primary objective is to evaluate the treatment modalities used and the overall survival of patients w.th esthesioneuroblastoma managed w.th exclusively endoscopic ***:In accordance w.th PRISMA guidelines,PubMed w.s queried to identify studies describing outcomes associated w.th endoscopic management of ***:Forty-four out of 2462 articles met inclusion criteria,totaling 399 patients w.th esthesioneuroblastoma treated w.th an exclusively endoscopic ***-tw. patients(18.0%)received adjuvant chemotherapy and 331 patients(83.0%)received postoperative radiation *** average age w.s 50.6 years old(range 6-83).Of the 399 patients,57(16.6%)w.re Kadish stage A,121(35.2%)w.re Kadish stage B,145(42.2%)w.re Kadish stage C,and 21(6.1%)w.re Kadish stage *** analysis demonstrated that 66.0%of patients had Hyams histologic GradeⅠorⅡ,w.ile 34.0%of patients had GradeⅢorⅣ*** surgical margins w.re achieved in 86.9%of patients,and recurrence w.s identified in 10.3%of *** those w.th 5-year follow.up,reported overall survival w.s 91.1%.Conclusion:Exclusively endoscopic surgery for esthesioneuroblastoma is performed for a w.de range of disease stages and grades,and the majority of these patients are also treated w.th adjuvant chemotherapy or radiation *** overall recurrence rate is 10.3%and 5-year survival is 91.1%.

Racial discrimination remains a prevalent issue in the contemporary *** efforts to promote *** young African American and Hispanic males are easy target for law.enforcement *** experience a higher and more unfair form of racial discrimination,racial profiling,police brutality,unfair sentencing,and mass incarceration for offences w.ich are the same or less than those committed by w.ite *** rate of incarceration in the United States is five to eight times higher than most developed countries,and Black males constitute the largest percentage of inmates in the *** *** arrested,Black Americans are more likely to remain in prison longer,and aw.it trial for minor offenses at a higher rate than *** and Latino males sentenced in state and federal courts face significantly greater odds of incarceration than w.ite offenders for the same or even higher *** and McCurdy in a 2006 ACLU on“cracks in the system”reported that“There is no rational medical or penological reason for the 100:1 disparity betw.en crack and pow.er cocaine and instead it causes an unjustified racial disparity in our penal system”(p.7).There is a racial disparity in the proportion of Black males in prison serving sentences of life w.thout the possibility of parole(Lw.P).In addition,The United States Criminal Justice System needs to be carefully examined as a top priority agenda needing immediate call of action that needs reform to guarantee the constitutional rights accorded to every American“w.th liberty and justice for all”.
