A method is proposed to estimate the seismic soil-structure-interaction （SSI） effects for use in engineering practice. It is applicable to 2D structures subjected to vertically incident shear waves supported by homogenous half-spaces. The method is attractive since it keeps the simplicity of the spectral approach, overcomes some of the difficulties and inaccuracies of existing classical techniques and yet it considers a physically consistent excitation. This level of simplicity is achieved through a response spectra modification factor that can be applied to the free-field 5%-damped response spectra to yield design spectral ordinates that take into account the scattered motions introduced by the interaction effects. The modification factor is representative of the Transfer Function （TF） between the structural relative displacements and the free- field motion, which is described in terms of its maximum amplitude and associated frequency. Expressions to compute the modification factor by practicing engineers are proposed based upon a parametric study using 576 cases representative of actual structures. The method is tested in 10 cases spanning a wide range of common fundamental vibration periods.

Sickle cell anemia (SCA) is a prevalent genetic disorder primarily affecting individuals of African descent and populations in malaria-endemic regions, with significant global public health implications. Sickle cell crises are their most common acute complication, characterized by episodes of intense pain and systemic manifestations that impair quality of life and impose a high healthcare burden. We present the case of a 19-year-old male diagnosed with SCA since the age of two, who developed a sickle cell crisis precipitated by right basal pneumonia. The patient exhibited sudden-onset, cyclic lumbar pain with progressive dyspnea. Initial management included multimodal pain control, volume optimization, and targeted antimicrobial therapy to achieve clinical stabilization. This case underscores the importance of a comprehensive approach to managing sickle cell crises, addressing both symptomatic relief and the prevention and treatment of complications. It also highlights the need for public health strategies promoting early diagnosis, access to disease-modifying therapies such as hydroxyurea, and interdisciplinary follow-up to mitigate the socioeconomic and clinical impact of SCA.

In order to evaluate the effect of 25-OH-D3 on productive performance of broilers and bone mineralization, an experiment in which 10,000 newly born Cobb mixed was used. The animals were divided into two groups: experimental received 25-OH-D3 at a dose of 50 mg/liter and a control group (0 mg/liter). Weight gain and European Production Efficiency Factor (EPEF) were higher (P < 0.05) for chickens that received 25-OH-D3. Mortality was lower (P < 0.05) for the group receiving 25-OH-D3. However feed conversion rate (FCR) and feed intake showed no significant difference between treatments. The amount of calcium in tibia was higher in the experimental group (P < 0.05). Based on the results obtained, it is concluded that the use of 25-OH-D3 improves production performance and bone mineral status broiler.

Cost-Effectiveness of Somatostatin Analogues for the Treatment of Acromegaly in Colombia

Background: Somatostatin analogues have shown to be effective in controlling the levels of growth hormone and are recommended for the treatment of acromegaly. These treatments have high costs of acquisition and their use might be restricted in limited resource settings. Objective: To evaluate the cost-effectiveness of somatostatin analogues for acromegaly in Colombia. Setting/Subjects/Intervention: A decision model was developed using a hypothetical cohort of patients with Acromegaly. Patients were treated according to the clinical practice of the country. Response to treatment and transition probabilities were derived from published literature. Costs and resource utilization were extracted from public and private sources in Colombia. Main Outcome Measure(s): Cost-effectiveness ratio, measured in Colombian pesos in a 2 year time-horizon. Results: The total medical treatment costs for the octreotide group were (Colombian Pesos) COP$ 53,807,616, compared to the total costs for the lanreotide group of COP$ 83,126,567. In the octreotide arm 65.30% of the patients and in the lanreotide arm 59.50% of the patients were successfully controlled. The number of deaths was 295 (13.1%) and 302 (13.4%) for octreotide and lanreotide, respectively. Because the costs are lower and the effectiveness is higher for octreotide in comparison with lanreotide, octreotide is more cost-effective than lanreotide. Probabilistic sensitivity analyses were consistent showing octreotide as the most cost-effective option. Conclusions: Costs and effects of octreotide compare favorably to those of lanreotide in the treatment of acromegaly in Colombia. Sensitivity analysis showed that despite the uncertainty in cost-effectiveness ratio this result is robust.

The Transcervical Extended Access, a Feasible Approach for the Surgical Treatment of Benign Tumors of the Posterior Mediastinum?

Transcervical approach for tumors of the posterior mediastinum is traditionally thought not to be indicated. Hereby we report on a case of a patient with a huge neurogenic tumor of the posterior mediastinum which was successfully excised through a transcervical extended approach and, additionally, the variety of surgical approaches used to remove tumors of the posterior mediastinum is discussed. The new refined techniques of transcervical extended mediastinal operations, which are recently gaining popularity among surgeons, allowed for a safe dissection of the tumor, thus patient could benefit from a short painless postoperative course. The authors suggest that surgeons trained in these particular techniques should consider the choice of the transcervical extended access in selected cases of benign tumors of the posterior mediastinum.
