Objective: To evaluate the synergistic effect of a Selective Serotonin Reuptake Inhibitor (SSRI) and a Selective Cyclooxygenase 2 Inhibitor (ISCOx-2) in the management of After Rachianesthesia Headache in Obstetric Care. Patients and Methods: This was an experimental double-blind, randomized, multi-center clinical trial of non-inferiority clinical trials over a 12-month period (May 1, 2018 to April 30, 2019) in six hospitals in southeastern DR Congo, including all patients undergoing surgery or analgesia on spinal anesthesia for obstetrical indication and with headache characteristic of a dural breccia, with no known causes, contraindications to experimental products and informed consent to been obtained. The patients were divided into four different groups depending on the protocol used, namely: Group A = Rehydration + Paracetamol, Group B = Rehydration + Celecoxib, Group C = Rehydration + Nefopam and Group 4 = Rehydration + Celecoxib + Nefopam (Study Group). Using the Statview II software, the statistical analyzes of data obtained from the different tests were made using the ANOVA test for comparing the averages of different samples using the Fisher test. Background: In March 2020, the World Health Organization declared the coronavirus disease 2019 a pandemic, this was caused by the Severe Acute Respiratory Syndrome Coronavirus 2. The overall mortality from this remains high, yet there are limited studies assessing the associated factors in Africa. Objective: To identify the factors associated with mortality in hospitalized COVID-19 patients in Douala, Cameroon. Method: We conducted a single-centre retrospective cross-sectional study by reviewing records of patients managed for COVID-19 between March and June 2020. Diagnosis was confirmed by real-time RT-PCR. Outcome of interest was mortality during hospitalization. We inputted and analyzed data using SPSS version 25.0, compared mortality between groups using the Chi’s squared test and the fisher’s exact test where appropriately investigated for associations using the Logistic regression in a stepwise approach and alpha-value set at P = 0.05. Results: We analyzed 282 case files, 68% were males (M:F = 2:1) and a mean age of 52 (±15) years. We had an overall mortality of 31.5% (89/282 patients) and 54% (50/92 patients) admitted in the ICU died. Patients aged 0 - 39 years had a significantly lesser odds of mortality compared to those > 70 years (OR: 0.2 (0.1 - 0.5) P Conclusion: The in-hospital mortality in COVID-19 patients is very high and hospitalisation into the ICU is associated with even higher mortality. Advanced age, diffused lung involvement (particularly with alveolar consolidation), hemodynamic instability at presentation and altered level of consciousness favoured hospitalisation in the ICU. Once in the ICU, placement on mechanical ventilation did not reduce death discernibly.

Epidemiological and Histopathological Aspects of Chronic Gastritis at the Departmental University Teaching Hospital of Borgou Alibori (CHUD-BA) from 2011 to 2021

Background: Gastritis is better evaluated nowadays since the advent of upper gastrointestinal endoscopy. Despite the availability of this workup in Benin Republic, there is a scarcity of data on chronic gastritis. Objective: This work aimed to study the epidemiological and histopathological aspects of chronic gastritis in the Departmental University Teaching Hospital of Borgou Alibori (CHUD-B/A) from 2011 to 2021. Method: This was a retrospective cross-sectional study with descriptive and analytical purposes, carried out from February 23, 2022 to May 23, 2022, among patients who had undergone gastric biopsy and/or gastrectomy from January 2011 to January 2022 (10 year), and whose specimens were sent into the histopathology section of the CHUD-B/A. Non-probability sampling was performed with exhaustive recruitment. A survey form has been used to collect data from patients’ medical records. Pearson’s chi-square and fisher’s exact tests have been used as appropriate to determine correlations between variables. Result: A total of 310 cases of chronic gastritis were diagnosed in the histopathology section of the CHUD-B/A from 2011 to 2021. This represented a chronic gastritis frequency of 91.45% for all gastric lesions diagnosed throughout the study period. Erythematous gastropathy was the most frequent (70.35%). Helicobacter pylori was present in 36.77% and dysplasia was observed in 12.26% of cases. Erythematous gastropathy was a predictive factor for the absence of gastric dysplasia in histopathology check up (p-value = 0.042). In contrast, intestinal metaplasia was predictive of the presence of gastric dysplasia in histopathology check up (p-value < 0.001). Conclusion: Chronic gastritis is very common in our setting. Systematic biopsy performance in front of an evocative clinical situation followed by histopathological examination may be encouraged.

目的从医院领导角度了解并比较我国肿瘤专科医院和三级医院的缓和医疗服务现况。方法对肿瘤医院(206家)和三级医院(443家)领导进行缓和医疗服务的电子问卷调查并比较两类医院是否存在差异,调查内容包括服务形式和内容、未来计划以及阻碍学科发展的因素等。采用SPSS 16.0软件进行数据分析。根据数据类型,组间比较分别采用t检验、fisher确切概率检验及χ2检验。结果全国共88家(42.72%)肿瘤医院和174家(39.28%)三级医院领导完成调研,肿瘤医院参加调研的比例明显高于三级医院(P<0.01)。过去10年,78.74%(137/174)的三级医院和85.23%(75/88)肿瘤医院都能提供缓和医疗服务;目前仍能提供这一服务的比例分别为87.59%(120/137)和90.67%(68/75),差异均无统计学意义。48.53%(33/68)的肿瘤医院以及40.83%(49/120)的三级医院有缓和医疗专科医师(P=0.31)。肿瘤医院的专科门诊率为27.94%(19/68),三级医院为17.50%(21/120),差异无统计学意义;肿瘤医院在会诊[29家(42.65%)和33家(27.50%)]、住院床位[28家(41.18%)和32家(26.67%)]、专属安宁疗护机构[20家(29.41%)和20家(16.67%)]情况方面,均较三级医院有显著优势(P<0.05)。在缓和医疗服务和镇痛治疗上,两类医院都比5年前有显著进步(P<0.01);对缓和医疗与肿瘤治疗整合的观点都很支持,肿瘤医院均值为(7.06±3.19)分,三级医院为(6.29±3.64)分,差异无统计学意义;肿瘤医院对增加科研项目的赞同度显著高于三级医院[(7.24±3.12)和(5.91±3.71)分,P<0.01]。阻碍缓和医疗服务的前三位因素依次为缺乏报销支持、专业人员和预算。结论尽管全国85%以上的肿瘤医院和三级医院提供服务,但缓和医疗专科医师的比例不足50%。两类医院的缓和医疗及镇痛治疗都有显著进步,而肿瘤医院在专科会诊、住院床位、安宁疗护机构上比三级医院有优势。缺乏报销支持、专业人才和预算为阻碍缓和医疗服务的三大因素。

The identification of jets originating from quarks and gluons,often referred to as quark/gluon tagging,plays an important role in various analyses performed at the Large Hadron Collider,as Standard Model measurements and searches for new particles decaying to quarks often rely on suppressing a large gluon-induced *** paper describes the measurement of the efficiencies of quark/gluon taggers developed within the ATLAS Collaboration,using √s=13 TeV proton–proton collision data with an integrated luminosity of 140 fb^(-1) collected by the ATLAS *** taggers with high performances in rejecting jets from gluon over jets from quarks are studied:one tagger is based on requirements on the number of inner-detector tracks associated with the jet,and the other combines several jet substructure observables using a boosted decision tree.A method is established to determine the quark/gluon fraction in data,by using quark/gluon-enriched subsamples defined by the jet *** in tagging efficiency between data and simulation are provided for jets with transverse momentum between 500 GeV and 2 TeV and for multiple tagger working points.
