Absence of enhancement in a lesion does not preclude primary central nervous system T-cell lymphoma:A case report

bACKGROUND Primary central nervous system lymphoma(PCNsL)is a non-Hodgkin lymphoma that originates in the central nervous system(CNs)and is exclusively limited to the CNs.Although most PCNsLs are diffuse large b-cell lymphomas,primary CNs T-cell lymphomas(PCNsTLs)are rare.PCNsTLs typically demonstrate some degree of enhancement on contrast-enhanced magnetic resonance imaging(MRI).To the best of our knowledge,non-enhancing PCNsTL has not been reported previously.CAsE sUMMARY A 69-year-old male presented to the neurology department with complaints of mild cognitive impairment and gradual onset of left lower leg weakness over a span of two weeks.Initial MRI showed asymmetric T2-hyperintense lesions within the brain.No enhancement was observed on the contrast-enhanced T1 image.The initial diagnosis was neuro-behçet’s disease.Despite high-dose steroid therapy,no alterations in the lesions were identified on initial MRI.The patient’s symptoms deteriorated further.An MRI performed one month after the initial scan revealed an increased lesion extent.subsequently,brain biopsy confirmed the diagnosis of PCNsTL.The patient underwent definitive combined chemoradiotherapy.However,the patient developed bacteremia and died of septic shock approximately three months after diagnosis.CONCLUsION The absence of enhancement in the lesion did not rule out PCNsTL.A biopsy approach is advisable for pathological confirmation.

The ex-situ incorporation of the secondary siC reinforcement,along with the in-situ incorporation of the tertiary and quaternary Mg_(3)N_(2) and si_(3)N_(4) phases,in the primary matrix of Mg_(2)si is employed in order to provide ultimate wear resistance based on the laser-irradiation-induced inclusion of N_(2) gas during laser powder bed fusion.This is substantialized based on both the thermal diffusion-and chemical reactionbased metallurgy of the Mg_(2)si–siC/nitride hybrid composite.This study also proposes a functional platform for systematically modulating a functionally graded structure and modeling build-direction-dependent architectonics during additive manufacturing.This strategy enables the development of a compositional gradient from the center to the edge of each melt pool of the Mg_(2)si–siC/nitride hybrid composite.Consequently,the coefficient of friction of the hybrid composite exhibits a 309.3%decrease to–1.67 compared to–0.54 for the conventional nonreinforced Mg_(2)si structure,while the tensile strength exhibits a 171.3%increase to 831.5 MPa compared to 485.3 MPa for the conventional structure.This outstanding mechanical behavior is due to the(1)the complementary and synergistic reinforcement effects of the siC and nitride compounds,each of which possesses an intrinsically high hardness,and(2)the strong adhesion of these compounds to the Mg_(2)si matrix despite their small sizes and low concentrations.

bACKGROUND spontaneous pneumoperitoneum(sP) without gastrointestinal perforation rarely occurs in neonates, with most sP cases being idiopathic. Although sP usually follows a benign clinical course with favorable prognosis, it can become lifethreatening in certain situations. In these cases, urgent surgical intervention may be required. Therefore, it may be difficult to decide when or how to perform prompt interventions.AIM To demonstrate the distinct clinical features of sP to guide appropriate management by comparing characteristics between sP and typical pneumoperitoneum secondary to gastrointestinal perforation.METHODs We retrospectively reviewed electronic medical records and identified 37 neonates with radiological evidence of pneumoperitoneum who were treated at our institution. Clinical variables were compared between neonates with sP without gastrointestinal perforation(Group A) and those with pneumoperitoneum secondary to gastrointestinal perforation(Group b). Clinical variables between groups were compared using student’s t-test and the chi-square test. The risk factors related to mortality were examined using multi-logistic regression analysis.REsULTs Group A comprised 35.1%(13/37) of the patients. The frequency of persistent pulmonary hypertension(53.8%) and pneumothorax(46.2%) before the development of pneumoperitoneum was significantly higher in group A than in group b(P =0.004). Platelet count and partial pressure of arterial oxygen(PaO) were significantly lower in group A(P = 0.015 and 0.025, respectively). Overall mortality was significantly higher in group A than in group b(76.9% vs 16.7%, P = 0.001). Only preterm infants were significantly associated with high mortality(P = 0.041;odds ratio = 18.0). Accompaniment with persistent pulmonary hypertension and pneumothorax were also significantly high(P = 0.004) in group A, but these were not strongly associated with high mortality.CONCLUsION This study identified a higher mortality rate in patients with sP than that described in previous reports.

This study aimed to develop a pH-responsive folic acid-grafted organic/inorganic hybrid nanocomposite system for site-selective oral delivery of therapeutic antibodies. A folic acid-grafted aminoclay(FA-AC) was prepared via an in situ sol-gel method. Then, a drug-loaded nanocomplex was prepared via the electrostatic interaction of FA-AC with infliximab(IFX), a model antibody, and coated with Eudragit? s100(EFA-AC-IFX). FA-AC exhibited favorable profiles as a drug carrier including low cytotoxicity, good target selectivity, and capability to form a nanocomplex with negatively charged macromolecules. A pH-responsive FA-AC-based nanocomplex containing IFX(EFA-AC-IFX) was also obtained in a narrow size distribution with high entrapment efficiency(>87%). The conformational stability of IFX entrapped in EFA-AC-IFX was well maintained in the presence of proteolytic enzymes. EFA-ACIFX exhibited pH-dependent drug release, minimizing premature drug release in gastric conditions and the upper intestine. Accordingly, oral administration of EFA-AC-IFX to colitis-induced mice was effective in alleviating the progression of ulcerative colitis, while oral IFX solution had no efficacy. These results suggest that a pH-responsive FA-AC-based nanocomposite system can be a new platform for the site-selective oral delivery of therapeutic antibodies.

Epoxy-based nano-composites can be ideal electromagnetic interference(EMI)-shielding materials owing to their lightness,chemical inertness,and mechanical durability.However,poor conductivity and brittleness of the epoxy resin are challenges for fast-growing portable and flexible EMI-shielding applications,such as smart wristband,medical cloth,aerospace,and military equipment.In this study,we explored hybrid nanofillers of single-walled carbon nanotubes(sWCNT)/reduced graphene oxide(rGO)as conductive inks and polyester fabrics(PFs)as a substrate for flexible EMI-shielding composites.The highest electrical conductivity and fracture toughness of the sWCNT/rGO/PF/epoxy composites were 30.2 s m^(−1)and 38.5 MPa m^(1/2),which are~270 and 65%enhancement over those of the composites without sWCNTs,respectively.Excellent mechanical durability was demonstrated by stable electrical conductivity retention during 1000 cycles of bending test.An EMI-shielding effectiveness of~41 db in the X-band frequency of 8.2-12.4 GHz with a thickness of 0.6 mm was obtained with an EM absorption-dominant behavior over a 0.7 absorption coefficient.These results are attributed to the hierarchical architecture of the macroscale PF skeleton and nanoscale sWCNT/rGO networks,leading to superior EMI-shielding performance.We believe that this approach provides highly flexible and robust EMI-shielding composites for next-generation wearable electronic devices.

RAV1(Related to AbI3/VP1)is a plant-specific b3 and AP2 domain-containing transcription factor that acts as a negative regulator of growth in many plant species.The expression of RAV1 is downregulated by brassinosteroids(bRs);large-scale transcriptome analyses have shown that the expression of RAV1 was previously targeted by bRI1-EMs-sUPPREsOR1(bEs1)and bRAssINAZOLE-REsIsTANT1(bZR1),which are critical transcription factors for the bR-signaling process.Using RAV1-overexpressing transgenic plants,we showed that RAV1 overexpression reduced the bR signaling capacity,resulting in the downregulation of bR biosynthetic genes and bEs1 expression.Furthermore,we demonstrated that bEs1,not bZR1,is directly bound to the RAV1 promoter and repressed RAV1 expression,and vice versa;RAV1 is also bound to the bEs1 promoter and repressed bEs1 expression.This mutual inhibition was specific to RAV1 and bEs1 because RAV1 exhibited binding activity to the bZR1 promoter but did not repress bZR1 expression.We observed that constitutively activated bR signaling phenotypes in bes1-D were attenuated by the repression of endogenous bEs1 expression in transgenic bes1-D plants overexpressing RAV1.RNA-sequencing analysis of RAV1-overexpressing transgenic plants and bes1-D mutant plants revealed differentially expressed genes by RAV1 and bEs1 and genes that were oppositely co-regulated by RAV1 and bEs1.RAV1 and bEs1 regulated different transcriptomes but co-regulated a specific set of genes responsible for the balance between growth and defense.These results suggested that the mutual inhibitory transcriptional activities of RAV1 and bEs1 provide fine regulatory mechanisms for plant growth and development.

The below-cloud aerosol scavenging process by precipitation is one of the most important mechanisms to remove aerosols from the atmosphere.Due to its complexity and dependence on both aerosol and raindrop sizes,wet scavenging process has been poorly treated,especially during the removal of fine particles.This makes the numerical simulation of below-cloud scavenging in large-scale aerosolmodels unrealistic.To consider the slip effects of submicron particles,a simplified expression for the diffusion scavenging was developed by approximating the Cunningham slip correction factor.The derived analytic solution was parameterized as a simple power function of rain intensity under the assumption of the lognormal size distribution of particles.The resultant approximated expression was compared to the observed data and the results of previous studies including a 3D atmospheric chemical transport model simulation.Compared with the default GEOs-Chem coefficient of 0.00106R0.61 and the observation-based coefficient of 0.0144R0.9268,the coefficient of a and b in∧m=aRb spread in the range of 0.0002-0.1959 for a and 0.3261-0.525 for b over a size distribution of GsD of 1.3–2.5 and a geometric mean diameter of 0.01-2.5μm.Overall,this study showed that the scavenging coefficient varies widely by orders of magnitude according to the size distribution of particles and rain intensity.This study also demonstrated that the obtained simplified expression could consider the theoretical approach of aerosol polydispersity.Our proposed analytic approach showed that results can be effectively applied for reduced computational burden in atmospheric modeling.

The constitutively-expressed cyclooxygenase 1（COX-1） and the inducible COX-2 are both involved in the conversion of arachidonic acid（AA） to prostaglandins（PGs）.However,the functional roles of COX-1 at the cellular level remain unclear.We hypothesized that by comparing differential gene expression and eicosanoid metabolism in lung fibroblasts from wild-type（WT） mice and COX-2^（-/-） or COX-1^（-/-） mice may help address the functional roles of COX-1 in inflammation and other cellular functions.Compared to WT,the number of specifically-induced transcripts were altered descendingly as follows：COX-2^（-/-）〉 COX-1^（-/-）〉 WT ＋ IL-1β.COX-1^（-/-） or COX-2^（-/-） cells shared about 50%of the induced transcripts with WT cells treated with IL-1β,respectively.An interactive ＂anti-inflammatory,proinflammatory,and redox-activated＂ signature in the protein-protein interactome map was observed in COX-2^（-/-） cells.The augmented COX-1mRNA（in COX-2^（-/-） cells） was associated with the upregulation of mRNAs for glutathione s-transferase（GsT）,superoxide dismutase（sOD）,NAD（P）H dehydrogenase quinone 1（NQO1）,aryl hydrocarbon receptor（AhR）,peroxiredoxin,phospholipase,prostacyclin synthase,and prostaglandin E synthase,resulting in a significant increase in the levels of PGE2,PGD2,leukotriene b4（LTb4）,PGF（1α）,thromboxane b2（TXb2）,and PGF（2α）.The COX-1 plays a dominant role in shifting AA toward the LTb4 pathway and anti-inflammatory activities.Compared to WT,the upregulated COX-1 mRNA in COX-2^（-/-） cells generated an ＂eicosanoid storm＂.The genomic characteristics of COX-2^（-/-） is similar to that of proinflammatory cells as observed in IL-1β induced WT cells.COX-1^（-/-） and COX-2^（-/-） cells exhibited compensation of various eicosanoids at the genomic and metabolic levels.
