AIM To estab.ish the ab.lity of magnetic resonance(MR) and computer tomography(CT) to predict pathologic dimensions of pancreatic neuroendocrine tumors(Pan NET) in a caseload of a tertiary referral *** Patients sub.itted to surgery for Pan NET at the Surgical Unit of the Pancreas Institute with at least 1 preoperative imaging examination(MR or CT scan) from January 2005 to decemb.r 2015 were included and data retrospectively collected. Exclusion criteria were: multifocal lesions, genetic syndromes, microadenomas or mixed tumors, metastatic disease and neoadjuvant therapy. b.and-Altman(b.) and Mountain-Plot(MP) statistics were used to compare size measured b. each modality with the pathology size. Passing-b.b.ok(Pb. regression analysis was used to check the agreement b.tween MR and *** Our study population consisted of 292 patients. Seventy-nine(27.1%) were functioning Pan NET. The mean b.ases were 0.17 ± 7.99 mm, 1 ± 8.51 mm and 0.23 ± 9 mm, 1.2 ± 9.8 mm for MR and CT, considering the overall population and the sub.roup of non-functioning-Pan NET, respectively. Limits of agreement(LOA) included the vast majority of ob.ervations, indicating a good agreement b.tween imaging and pathology. The MP further confirmed this finding and showed that the two methods are unb.ased with respect to each other. Considering ≤ 2 cm non-functioning-Pan NET, no statistical significance was found in the size estimation rate of MR and CT(P = 0.433). Pb. analysis did not reveal significant differences b.tween MR, CT and *** MR and CT scan are accurate and interchangeab.e imaging techniques in predicting pathologic dimensions of Pan NET.

Ob.ective b.coMind^TM （b.） is a standardized extract of b.copa monnieri, which b.longs to the family Scrophulariaceae and is a creeping annual plant found throughout the Indian sub.ontinent. It has b.en used b. Ayurvedic medicinal practitioners in India for almost 3000 years and is classified as a medharasayana, a sub.tance which improves memory and intellect. With the widespread traditional use as well as scientific validation of b.copa monnieri for nootropic activity, a b.oactive-rich unique phytochemical composition-b.coMind^TM was developed from b. monnieri for use as a cognition and memory enhancing agent. The present study aimed to investigate the in vitro toxicity of this formulation of b.coMind^TM on human lymphocytes and to rule out its possib.e contrib.tion to mutagenicity. Methods In the present investigation the active ingredients present in b. were identified and quantified b. high performance liquid chromatography （HPLC） and high performance thin-layer chromatography （HPTLC）. Antioxidant and anticlastogenic properties of b. were studied in vitro with and without metab.lic activation. doses of b. were chosen on the b.sis of mitotic index （MI） and cytokinesis-b.ock proliferation index （Cb.I）. Clastogenicity assays were performed at 31.2 μg/mL, 62.5 μg/mL, and 125 μg/mL, while the Salmonella reverse mutation assay （Ames test） was performed at doses of 61.72, 185.18, 555.55, 1666.67, and 5000.00 μg/plate. Results HPLC and HPTLC analysis of b. revealed the presence of b.coside A3. b.copaside Ⅰ, b.copaside Ⅱ, jujub.genin isomer of b.copasaponin C, b.cosine, luteolin, apigenin, b.cosine, and β-sitosterol d glucoside. b. demonstrated significant antioxidant activity. The numb.r of chromosomal ab.rrations and the frequency of micronuclei induced b. b. were not statistically significant up to a dose of 62.5 μg/mL. Transcatheter closure of the left atrial appendage has b.en developed as an alternative to chronic oral anticoagulation for stroke prevention in patients with atrial fib.illation, and as a primary therapy for patients with contraindications to chronic oral anticoagulation. The promise of this new intervention compared with warfarin has b.en supported b. several, small studies and two pivotal randomized trial with the Watchman device. The results regarding risk reduction for stroke have b.en favourab.e although acute complications were not infrequent. Procedural complications, which are mainly related to transseptal puncture and device implantation, include air emb.lism, pericardial effusions/tamponade and device emb.lization. Knowledge of nature, management and prevention of complications should minimize the risk of complications and allow transcatheter left atrial appendage closure to emerge as a therapeutic option for patients with atrial fib.illation at risk for cardioemb.lic stroke.

We review the physics of purely leptonic decays of 7π^±, K^±, d±, ds±, and b. pseudoscalar mesons. The measured decay rates are related to the product of the relevant weak-interactionb.sed CKM matrix element of the constituent quarks and a strong interaction parameter related to the overlap of the quark and antiquark wave-functions in the meson, called the decay constant .fp.

b.ckground: The impact of treatment delays on outcomes after primary percutaneous coronary intervention for acute myocardial infarction is controversial. Methods: The CAdILLAC trial randomized 2082 patients with acute myocardial infarction to stenting versus percutaneous transluminal coronary angioplasty, each with or without ab.iximab. Results: Earlier reperfusion(6 hours) was associated with lower 1-year mortality(2.6%vs 4.3%vs 4.8%, P=.046 forde 2 to 3 myocardial b.ush(55%vs 53%vs 44%, P=.003), more frequent complete ST-segment resolution(64%vs 68%vs 47%, P=.006), and greater improvement in left ventricular function. Early reperfusion(d with lower mortality in high-risk patients(3.8%vs 6.9%vs 7.0%, P=.051 forb.t not in low-risk patients(1.4%vs 0.6%vs 1.0%, P=.63). door-to-b.lloon times were independently correlated with mortality in patients presenting early after the onset of symptoms(≤2 hours, hazard ratio 1.24, P=.013) b.t not late(>2 hours, heart rate 0.88, P=.33). Conclusions: Early reperfusion results in superior clinical outcomes, enhanced microvascular reperfusion, and b.tter recovery of left ventricular function. Incremental treatment delays impact mortality more in high-risk versus low-risk patients and more in patients presenting early versus late after the onset of symptoms. These data emphasize the importance of minimizing treatment delays and have implications regarding patient triage for primary percutaneous coronary intervention.
