背景：扩张型心肌病（dilated cardiomyopathy，DCM）是儿童心肌病最常见的类型，也是小儿心脏移植最常见的原因。但是，目前尚不清楚儿童DCM的流行病学特点和临床过程。目的：详细描述儿童DCM的发病率、病因、预后及相关危险因素。设计及地点：本项以人群为基础的前瞻性纵向队列研究入选1996年1月至2003年2月在89个儿科心脏中心诊断为DCM的儿童以及1990年1月至1995年12月在北美大型三级治疗中心诊断的患儿。参试者：共入选1426例来自美国和加拿大的18岁以下DCM患儿。通过严格的超声心动图和／或病理学标准来诊断原发性DCM。因内分泌、免疫、药物中毒及其他原因患病者除外。主要观测指标：每年每10万儿童的发病率、死亡率和心脏移植率。结果：18岁以下儿童DCM每年的总发病率为0．57／10万，男孩的年发病率高于女孩（分别为0．66／10万和0．47／10万；P〈0．001），黑人的年发病率高于白人（分别为0．98／10万和0．46／10万；P〈0．001），婴儿（〈1岁）的年发病率高于儿童（分别为4．40／10万和0．34／10万；P〈0．001）。绝大部分属特发性，最常见的病因为心肌炎（46％）和神经肌肉性疾病（26％）。1年和5年死亡／移植率分别为31％和46％。死亡／移植的独立危险因素包括患儿诊断DCM时年龄较大、伴有充血性心力衰竭、左室收缩期Z评分较低以及DCM的病因（P均〈0．001）。结论：儿童DCM有其多样性。患儿预后主要取决于病因、年龄和心力衰竭状况。人种、性别和年龄对发病率有一定影响。大部分患儿无明确病因，限制了特异性治疗。

Although the routine use of serum prostate-specific antigen （PSA） testing has undoubtedly increased prostate cancer （PCa） detection, one of its main drawbacks is its lack of specificity. As a consequence, many men undergo unnecessary biopsies or treatments for indolent tumours. PCa-specific markers are needed for the early detection of the disease and the prediction of aggressiveness of a prostate tumour. Since PCa is a heterogeneous disease, a panel of tumour markers is fundamental for a more precise diagnosis. Several biomarkers are promising due to their specificity for the disease in tissue. However, tissue is unsuitable as a possible screening tool. Since urine can be easily obtained in a non-invasive manner, it is a promising substrate for biomarker testing. This article reviews the biomarkers for the non-invasive testing of PCa in urine.

Background: Patients commonly develop postoperative pain after total knee arthroplasty(TKA).Acupuncture-related techniques and low-level laser therapy could be beneficial for pain management for older ***: To examine the effect of low-level laser acupuncture(LA) in reducing postoperative pain, painrelated interference in daily life, morphine consumption, and morphine-related side effects in older patients with knee osteoarthritis who underwent ***, setting, participants and intervention: A single-blind randomized placebo-controlled trial was conducted. Patients(N = 82) were recruited and randomly assigned via a computer-generated list to the LA group or a placebo group. The LA group received low-level laser therapy at Sanyinjiao(SP6), Taixi(KI3),Kunlun(BL60), Fengshi(GB31), Futu(ST32) and Neiguan(PC6) after TKA, while the placebo acupuncture group received the same treatment procedure without laser energy *** outcome measures: The primary outcome was postoperative pain intensity, and it was measured at baseline and hours 2, 6, 10, 24, 48 and 72 after TKA. The secondary outcomes, including relative pain,postoperative pain-related interference in daily life and morphine consumption, were measured at hours24, 48 and 72 after ***: Generalized estimating equations revealed significant between-group differences in pain intensity(P = 0.01), and trend differences in pain intensity for the LA group starting at hours 10 to 72(P < 0.05)and morphine consumption at hours 48 and 72(P < 0.05). The changes in pain-related interference in daily life were significant(P < 0.05) at 72 h, with the exception of the parameters for worst pain, mood,and sleep. Nausea and vomiting side effects from morphine had significant between-group differences at hours 10 and 24(P < 0.05).Conclusion: Low-level LA gradually reduced older patients’ postoperative pain intensity and morphine consumption within the first 72 h after their TKA for osteoarthritis. Intention of healthcare workers to accept COVID-19 vaccination and related factors:A systematic review and meta-analysis

Considering medical and economic burden of coronavirus disease 2019(COVID-19), a high COVID-19 vaccination coverage among healthcare workers(HCWs) is an urgent need. The aim of this systematic review and meta-analysis was to evaluate the intention of HCWs to accept COVID-19 vaccination and to identify related factors. We searched Pub Med, MEDLINE, Scopus, Web of Science, Pro Quest, CINAHL and med Rxiv until July 14, 2021. The heterogeneity between results was very high;thus, we applied a random effects model to estimate pooled effects. We performed subgroup and meta-regression analysis to identify possible resources of heterogeneity. Twenty four studies, including 50 940 HCWs, met the inclusion criteria. The overall proportion of HCWs that intend to accept COVID-19 vaccination was 63.5%(95% confidence interval: 56.5%-70.2%) with a wide range among studies from 27.7% to 90.1%. The following factors were associated with increased HCWs’ willingness to get vaccinated against COVID-19: male gender, older age, white HCWs, physician profession, higher education level, comorbidity among HCWs, vaccination against flu during previous season, stronger vaccine confidence, positive attitude towards a COVID-19 vaccine, fear about COVID-19, individual perceived risk about COVID-19, and contact with suspected or confirmed COVID-19 patients. The reluctance of HCWs to vaccinate against COVID-19 could diminish the trust of individuals and trigger a ripple effect in the general public. Since vaccination is a complex behavior, understanding the way that HCWs take the decision to accept or refuse COVID-19 vaccination will give us the opportunity to develop the appropriate interventions to increase COVID-19 vaccination uptake.

The forewing of a termite from Early Miocene lake sediments in Otago, southern New Zealand is figured and described. It exhibits the generic characters of the damp-wood termite Stolotermes Hagen, but differs from forewings of the known species in size and venation pattern and is described as Stolotermes kupe sp. nov. S. kupe represents the first confident record of fossil Stolotermitidae and extends the fossil record of the family back to the Early Miocene. It also is the first direct evidence of fossil Isoptera from New Zealand, though silicified termite faecal pellets, referable to Kalotermes brauni, have been previously described. S. kupe indicates that Stolotermitidae has been present in the Australasian region since at least the Early Miocene.

When the drivers approaching signalized intersections(onset of yellow signal),the drivers would enter into a zone,where they will be in uncertain mode assessing their capabilities to stop or cross the ***,any improper decision might lead to a right-angle or back-end *** avoid a right-angle collision,drivers apply the harsh brakes to stop just before the signalized *** this may lead to a back-end crash when the following driver encounters the former's sudden stopping *** situation gets multifaceted when the traffic is heterogeneous,containing various types of *** order to reduce this issue,this study's primary objective is to identify the driving behaviour at signalized intersections based on the driving features(parameters).The secondary objective is to classify the outcome of driving behaviour(safe stopping and unsafe stopping)at the signalized intersection using a support vector machine(SVM)*** moments are used to identify the zones and label them accordingly for further *** classification of 50 instances is identified for training and testing using a 70%-30% rule resulted in an accuracy of 85% and 86%,*** performance is further verified by random sampling using five cross-validation and 30 iterations,which gave an accuracy of 97% and 100% for training and *** results demonstrate that the proposed approach can help develop a pre-warning system to alert the drivers approaching signalized intersections,thus reducing back-end crash and accidents.

Chloroplast Proteomics and the Compartmentation of Plastidial Isoprenoid Biosynthetic Pathways

Recent advances in the proteomic field have allowed high-throughput experiments to be conducted on chloroplast samples. Many proteomic investigations have focused on either whole chloroplast or sub-plastidial fractions. To date, the Plant Protein Database （PPDB, Sun et al., 2009） presents the most exhaustive chloroplast proteome available online. However, the accurate localization of many proteins that were identified in different sub-plastidial compartments remains hypothetical. Ferro et al. （2009） went a step further into the knowledge of Arabidopsis thaliana chloroplast proteins with regards to their accurate localization within the chloroplast by using a semi-quantitative proteomic approach known as spectral counting. Their proteomic strategy was based on the accurate mass and time tags （AMT） database approach and they built up AT_CHLORO, a comprehensive chloroplast proteome database with sub-plastidial localization and curated information on envelope proteins. Comparing these two extensive databases, we focus here on about 100 enzymes involved in the synthesis of chloroplast-specific isoprenoids. Well known pathways （i.e. compartmentation of the methyl erythritol phosphate biosynthetic pathway, of tetrapyrroles and chlorophyll biosynthesis and breakdown within chloroplasts） validate the spectral counting-based strategy. The same strategy was then used to identify the precise localization of the biosynthesis of carotenoids and prenylquinones within chloroplasts （i.e. in envelope membranes, stroma, and/or thylakoids） that remains unclear until now.

To assess the impact of disease characteristics on the quality of life (QOL) in children with inflammatory bowel diseases (IBD).METHODSThis was a cross-sectional study conducted at the First Department of Pediatrics of the University of Athens at the “Aghia Sophia” Children’s Hospital. Children diagnosed with Crohn’s disease (CD) or ulcerative colitis (UC), who were followed as outpatients or during a hospitalization, participated, after informed consent was obtained from their legal representative. QOL was assessed by the IMPACT-III questionnaire. Demographic data and disease characteristics were also collected. Statistical analyses included parametric (Student’s t-test and Pearson’s r) and non-parametric (Mann-Whitney test, Fisher’s test and Spearman’s rho) ***-nine patients (UC: 37, 73.0% females, CD: 62, 51.6% females), aged 12.8 ± 2.6 years were included. Overall, as well as, sub-domain scores did not differ between UC and CD (overall score: 73.9 ± 13.3 vs 77.5 ± 11.2, respectively, P = 0.16). In the entire sample, total score was related to physician’s global assessment (PGA, patients classified as “mild/moderate” active disease had, on average, 14.8 ± 2.7 points lower total scores compared to those “in remission”, P < 0.001) and age at IMPACT completion (Pearson’s r = 0.29, P = 0.05). Disease activity assessed by the indices Pediatric Ulcerative Colitis activity index, Pediatric Crohn’s disease activity index or PGA was significantly associated with all subdomains scores. Presence of extraintestinal manifestations had a negative impact on emotional and social functioning *** activity is the main correlate of QOL in children with IBD, underlining the importance of achieving and sustaining clinical remission
