Evaluation of the Local Epinephrine Anesthesia without Tourniquet in the Surgical Treatment of Hand Injuries in Adults: About 27 Cases in Sub-Saharan Africa

Background: In sub-Saharan Africa, surgery for hand injuries is usually performed under axillary or general anesthesia. This is often not without consequences. The Wide Awake Local Anesthesia No Tourniquet (WALANT) is, therefore, an anesthetic asset in the surgical treatment of hand injuries. This study aims to share an experience on this technique’s effectiveness and to spark interest among African authors and practitioners. Method: This prospective study focuses on 27 surgeries of hand injuries carried out with WALANT. The study spanned over nine months (November 2021-August 2022) and included 19 men and eight women with a mean age of 35.4 years (extremes: 19 and 54). There were five (18%) flexor tendon ruptures, three of which were in zone 2, eleven (41%) metacarpal fractures, four of which were open, seven (26%) phalangeal fractures, two of which were open, and four (15%) finger springs. The operated lesions were assessed at a minimum of three months. The pain was assessed using the Visual Analog Scale. The QuickDASH score was evaluated for each patient at the end of the follow-up period. Results: Intra-operatively, the mean value of the Visual Analog Scale was 1/10, with extreme values of 1/10 and 3/10. Of the five cases of flexor tendon ruptures, three were located in zone 2 and two in zone 3. The metacarpal fractures were divided into seven closed diaphyseal fractures and four extra-articular metaphyseal fractures. Four of the seven phalangeal fractures were oblique diaphyseal, and three were transverse diaphyseal. The treatment of the protruding fingers followed the conventional technique. The assessment of the QuickDASH score in the months following surgery showed a score of 11.8 in the first month, 10.3 in the second month, and 5.0 in the third month. Conclusion: WALANT is an easy-to-use, unexpensive anesthetic technique that enables shorter operating times. It rarely presents complications. The paper explores the degree of pollution of organochlorine pesticides in fish and water in the dam at Kpassa on a tributary of Okpara River that is pumped by Benin National Water agency (SONEB) in supplying drinking water to supply the city of Parakou. Doing so, fourteen parameters of organochlorine are analyzed. Most of obtain organoclorine concentration in water and fish below is indicated critical values. However, DDT and endrine concentration in water is slightly above legal tolerable values. Hexachlorobenen and dieldrine concentrations are three times higher than legal limit value while aldrine is ten times higher. However, heptachlore is double concentrated in fishery while aldrine (endrine, dieldrine aldrine, lindane, hexachloro-benzene, DDT) is found in the dam fish and surface water three times concentrated than tolerated value. These levels of concentrations result from the intensification of organoclorine pesticide used in agriculture especially in cotton production. These are caused by the chemical application accumulated in soil and through the food value chain system. Therefore, it is very important to extract sediment from the reservoir by dredging in oder to renew the ecosystem of the dam. To persistently manage the basin water resources, it is imperatively important to observe a significant behaviour changed from all stakeholders.

This work studies the influence of mechanical and geometrical characteristics of the concrete and the soil on the stresses in a mat foundation. In this study, the soil-structure interaction is modeled by two parameters, the modulus of subgrade vertical reaction (k) and the modulus of subgrade horizontal reaction (2T). These two parameters are dependent on the geometrical and mechanical characteristics of the system. Results of this study show a sensitivity of solicitations to variations of geometrical and mechanical characteristics of the model. Although solicitations in the plate are sensitive to mechanical properties of concrete, these solicitations are strongly influenced by the mechanical and geometrical characteristics of the soil mass. However, it should be noted that the influence of Eb is denoted in the center of the plate whereas the Es feels almost in the same manner over the entire extent of the plate. This study also shows that for the same load cases, the values of the torsion moment and shear stress are not significant those of bending moments and normal stresses, respectively.

The main purpose of this work is to study doping level effects on a silicon PV cell under both moderate light concentration and normal illumination. This study also aims to compare the doping level effects under the both illumination modes. The results show for both illumination modes that diffusion parameters decrease with increasing doping level. These results are in agreement with the studies of the current and the voltage which showed for the two illumination modes that doping level increase leads to a decrease in current density and an increase in voltage. It also emerges for the two illumination modes and for the doping range 1013 cm-3 - 1016 cm-3, a decrease of maximum power and conversion efficiency. The results also show that decrease of diffusion parameters is faster under moderate concentration in comparison with normal illumination. These results predict a greater variation rate of the current, the voltage, the maximum power and the conversion efficiency under moderate concentration compared to normal illumination. Contrary to diffusion parameters study, the results show higher variation rates of parameters under normal illumination. This is explained by the fact that under moderate concentration, carriers density is close to doping level: the cell is then in high injection condition. Consequently, under moderate concentration, carriers density is less sensitive to doping level variations. The study confirms that carriers density variation with the doping level is weak under the moderate concentration compared to normal illumination.

Laryngeal tuberculosis is a rare form of extrapulmonary tuberculosis, often complicating pulmonary tuberculosis that may be unrecognized. Its clinical presentation is nonspecific, often pointing to cancer. We report the case of a 77-year-old woman, with no reported pathological history. She also has no alcohol or tobacco intoxication, who presented with chronic dysphonia evolving for 2 months, associated with an altered general condition. The examination of the larynx by direct laryngoscopy and anatomical pathology study of the biopsies led to the diagnosis of laryngeal tuberculosis. A search for secondary sites revealed a concomitant pulmonary infection. The evolution was favorable under standard anti-tuberculosis treatment, with complete voice recovery and improved performance status. Laryngeal tuberculosis should be suspected in patients living in endemic areas and suffering from chronic dysphonia, even if they are not alcoholics or smokers.

Nitrogen (N) and phosphorus (P) wastes produced by Nile tilapia Oreochromis niloticus L. fed Azolla, an aquatic atmospheric nitrogen fixing fern, was evaluated for 90 days in pond experiment. Six isonitrogenous (29.2% crude protein) and isoenergetic (16.9 Kj·g–1) diets A0, A10, A20, A30, A40 and A50, containing 0%, 10%, 20%, 30%, 40% and 50% of Azolla meal (AM) respectively, as partial fishmeal (FM) substitutes, was provided to experimental fish. The Azolla-free diet A0 served as a control. Fish specific growth rate (SGR) was higher with the control diet, the lower values being obtained in A50-fed fish (P Azolla could be used in diet to sustain Nile tilapia growth and as “environmentally-friendly” ingredient to limit P loss, while providing N to the field, beneficially in tropical marshland pond where this nutrient is already limiting.

Physico-Chemical Characteristics of Gushing Water Aquifers in the Coastal Sedimentary Basin of Benin (West Africa)

The coastal sedimentary basin in southern Benin consists of monoclinal layers divided into two plateau zones (North and South) which are separated by a longitudinal depression ESE-WNW. The valleys of the main N-S-oriented rivers (Ouémé, Couffo and Mono) set the bondaries of the different plateau of the BSC (Coastal Sedimentary Basin). The present study, based on geology, hydrochemistry, temperature and log data available on boreholes, makes a physico-chemical characterization of the waters of the gushing aquifers of the coastal sedimentary basin of Benin. The gushing water boreholes are shared between the valleys of the main rivers of the BSC. Some of these boreholes are thermal with a water temperature between 38 and 69 degrees Celsius. The hydrogeological correlations established in the BSC in accordance to the North- South direction in the valleys of the main streams (Couffo and Ouémé) reveal that the captured aquifers are sands, marls and limestones that respond either in major discordance (northern zone) or above the sedimentation gaps (southern zone). Chemical analyses have shown that gushing thermal waters are mineralized in the south with a neutral to basic pH and are highly concentrated with bicarbonate, calcium and magnesium ions. In the North, on the other hand, thermal waters are acidic with a pH ranging from 4.8 to 5.9. The acidic nature of the northern waters is influenced by the crystalline base while the southern neutral to basic waters are influenced by the lithological nature (limestone and marl) of the aquifer. Influence of Salinity on Duckweed Growth and Duckweed Based Wastewater Treatment System

The growth of duckweed (Spirodela polyrrhiza) was investigated in an outdoor batch system under 15 different conductivities ranging from 200 μS/cm to 3000 μS/cm with average 110 μmol/m2 daylight intensity. The growth was performed in an anaerobically treated domestic wastewater using an initial plants population of 50 fronds. Increase in Relative Growth Rate—RGR—(in terms of fresh weight and number of fronds) had a significant non-linear (polynomial) relationship with the conductivity. The maximum RGR related to fresh weight of 0.176 was observed at the conductiviity of 1200 μS/cm while the maximum RGR related to fronds number was 0.193 at the conductivity of 800 μS/cm. The optimum range for duckweeds growth was found to be between conductivities of 600 μS/cm and 1400 μS/cm. The maximal rates of removal of COD of 14.5 mg/day, of 0.65 mg/day, NTK of 15.68 mg/day, faecal coliforms of 100% and turbidity of 80.8% were observed in this range. The COD and PO42- removal rates were highly correlated to the growth rate, with the correlation factor of up to 0.783 and, then to the conductivity. The NTK reduction was positively related to the conductivity.

Evaluation and Prediction of Groundwater Quality in the Municipality of Za-Kpota (South Benin) Using Machine Learning and Remote Sensing

Accessing drinking water is a global issue. This study aims to contribute to the assessment of groundwater quality in the municipality of Za-Kpota (southern Benin) using remote sensing and Machine Learning. The methodological approach used consisted in linking groundwater physico-chemical parameter data collected in the field and in the laboratory using AFNOR 1994 standardized methods to satellite data (Landsat) in order to sketch out a groundwater quality prediction model. The data was processed using QGis (Semi-Automatic Plugin: SCP) and Python (Jupyter Netebook: Prediction) softwares. The results of water analysis from the sampled wells and boreholes indicated that most of the water is acidic (pH varying between 5.59 and 7.83). The water was moderately mineralized, with conductivity values of less than 1500 μs/cm overall (59 µS/cm to 1344 µS/cm), with high concentrations of nitrates and phosphates in places. The dynamics of groundwater quality in the municipality of Za-Kpota between 2008 and 2022 are also marked by a regression in land use units (a regression in vegetation and marshland formation in favor of built-up areas, bare soil, crops and fallow land) revealed by the diachronic analysis of satellite images from 2008, 2013, 2018 and 2022. Surveys of local residents revealed the use of herbicides and pesticides in agricultural fields, which are the main drivers contributing to the groundwater quality deterioration observed in the study area. The results of the groundwater quality prediction models (ANN, RF and LR) developed led to the conclusion that the model based on Artificial Neural Networks (ANN: R2 = 0.97 and RMSE = 0) is the best for groundwater quality changes modelling in the Za-Kpota municipality.
