Impact of type 2 diabetes on the plasma levels of vascular endothelial growth factor and its soluble receptors type 1 and type 2 in patients with peripheral arterial disease

Objective： Type 2 diabetes coexistent with lower extremity artery disease （peripheral arterial disease （PAD）） can be observed in numerous patients. The mechanism compensating for ischemia and contributing to healing is angiogenesis-the process of forming new blood vessels. The purpose of this study was to assess the likely impact of type 2 diabetes on the plasma levels of proangiogenic factor （vascular endothelial growth factor A （VEGF-A）） and angiogenesis inhibitors （soluble VEGF receptors type 1 and type 2 （sVEGFR-1 and sVEGFR-2）） in patients with PAD. Methods： Among 46 patients with PAD under pharmacological therapy （non-invasive）, we identified, based on medical history, a subgroup with coexistent type 2 diabetes （PAD-DM2＋, n=15） and without diabetes （PAD-DM2-, n=31）. The control group consisted of 30 healthy subjects. Plasma levels of VEGF-A, sVEGFR-1, and sVEGFR-2 were measured using the enzyme-linked immunosorbent assay （ELISA） method. Results： The subgroups of PAD-DM2＋ and PAD-DM2- revealed significantly higher concentrations of VEGF-A （P=-0.000007 and P=0.0000001, respectively） and significantly lower sVEGFR-2 levels （P=-0.02 and P=-0.00001, respectively）, when compared with the control group. Patients with PAD and coexistent diabetes tended to have a lower level of VEGF-A and higher levels of sVEGFR-1 and sVEGFR-2 comparable with non-diabetic patients. Conclusions： The coexistence of type 2 diabetes and PAD is demonstrated by a tendency to a lower plasma level of proangiogenic factor （VEGF-A） and higher levels of anglogenesis inhibitors （sVEGFR-1 and sVEGFR-2） at the same time. Regardless of the coexistence of type 2 diabetes, hypoxia appears to be a crucial factor stimulating the processes of angiogenesis in PAD patients comparable with healthy individuals, whereas hyperglycemia may have a negative impact on angiogenesis in lower limbs.

A preliminary evaluation of VEGF-A, VEGFR1 and VEGFR2 in patients with well-controlled type 2 diabetes mellitus

Objective： Decompensated chronic hyperglycemia often leads to late microvascular complications such as retinopathy, diabetic foot syndrome, and diabetic kidney disease. The aim of this study was to determine the concentration of vascular endothelial growth factor A （VEGF-A） and its receptors in patients with well-controlled diabetes. Methods： The study was conducted on 31 patients with well-controlled type 2 diabetes without microor macroangiopathy. Thirty healthy volunteers were enrolled in a control group. Serum concentrations of VEGF-A, VEGF receptors 1 and 2 （VEGFR1 and VEGFR2）, fasting glucose, and lipid profiles were measured, and the plasma concentration of glycated hemoglobin （HbAlc） was determined. Results： No significant differences were observed between the concentration of VEGF-A, VEGFR1 or VEGFR2 in the subject group and that in the control group. Positive correlations were noted between the levels of VEGF-A, VEGFR2, and triglyceride, and there was a negative correlation between the levels of VEGFR2 and high-density lipoprotein （HDL）-cholesterel in the study group. Conclusions： The concentrations of VEGF-A and its receptors 1 and 2 in patients with well-controlled diabetes are comparable to those of healthy individuals, which may indicate that appropriate control of glucose levels delays the occurrence of vascular complications. A negative correlation between VEGFR2 and HDL-cholesterol levels, and positive correlations between VEGF-A, VEGFR2, and triglyceride levels, suggest that lipid abnormalities occurring in diabetes may be involved in the modulation of angiogenesis.

Robotic Foregut Surgery: One Surgeon’s Experience with Nissen Fundoplication, Esophagomyotomy, and Hiatal Hernia Repair

Purpose: The purpose of the study was to review a single surgeon, seven-year experience, using the Da Vinci Telerobotic system performing 124 foregut operations. Methods: Data review from 2002-2009 including 71 Nissen fundoplications, 26 esophagomyotomies, and 27 hiatal hernia repairs was performed. Parameters collected included gender, age, body mass index (BMI), estimated blood loss (EBL), port set up time (PST), robot operating time (ROT), total case time (TCT), length of stay (LOS), complications, conversions, and resident involvement. Statistical analysis was con-ducted. Results: 124 foregut operations: 45 males, 79 females, mean age of 54.8 ± 16.7 (18 - 85) years, mean TCT 174.4 ± 45.0 (102 - 321) min. Nissen fundoplication: mean BMI of 30.8 ± 3.9 (22.4 - 46.8) kg/m2, EBL 30.2 ± 21.8 (5 - 100) ml, PST 32.3 ± 9 (14 - 63) min, ROT 111.4 ± 37.3 (51 - 229) min, TCT 175.0 ± 46.4 (102 - 321) min, median LOS 1 (0 - 9) day, complication rate 7.0% (5/71), conversion rate 5.6%, resident involvement 69.0% (49/71). Esophagomyotomy: mean BMI of 26.5 ± 6.1 (15.4 - 36.6) kg/m2, EBL 39.1 ± 41.7 (10 - 200) ml, PST 28.0 ± 8.6 (16 - 47) min, ROT 122.9 ± 45 (31 - 217) min, and TCT 178.0 ± 40.5 (105 - 262) min, median LOS 1 (0 - 6) day, complication rate 15.4% (4/26), conversion rate 0%, resident involvement 69.2% (18/26). Hiatal hernia repair: mean BMI of 28.4 ± 4.2 (21.9 - 36.8) kg/m2, EBL 38.4 ± 32.7 (10 - 150) ml, PST 28.8 ± 8.0 (17 - 52) min, ROT 109.0 ± 44.5 (49 - 250) min, and TCT 169.2 ± 46.5 (102 - 299) min, median LOS 1 (1 - 14) day, complication rate 11.1% (3/27), conversion rate 3.7%, resident involvement 66.7% (18/27). Conclusion: Robotic-assisted foregut surgery is safe and effective. This series compares favorably with other robotic studies in length of hospital stay, total case time, and complication and conver-sion rates. Foregut surgery is an excellent robotic training ground for residents.

A complex Sol-Gel process has been used for synthesis of silica glasses designed to contain high-level nuclear wastes. Cs, Sr, Co, and Nd (generically denoted Me) were used, the last as surrogate for actinides. Gels in the form of powders and sintered compacts were prepared by hydrolysis and polycondensation of tetraethoxide/Me nitrate solutions, which contained ascorbic acid as a catalyst. Thermal treatment studies were conducted on the resulting gels. Transformation to final products was studied by thermogravimetric analysis, infrared spectroscopy, and X-ray diffraction. Cell therapy of a patient with type Ⅲ Osteogenesis imperfecta caused by mutation in COL1A2 gene and unstable collagen type I

The allogenic bone marrow derived mesenchymal stem cells transplantation was given to the newborn girl diagnosed with osteogenesis imperfecta type III, with multiple bone fractures, extreme shortness and limbs deformities. The treatment was performed at the age of 4 and 6 weeks. The clinical diagnosis was supported by biochemical analysis of collagen type I recovered from culture medium of cultivated patient’s skin fibroblast, which revealed its triple helix instability at temperature about 2?C lower than normal. Sequencing of both genes encoding procollagen type I revealed heterozygous substitution G23569Ain COL1A2 gene causing change of glycine at position 517 to aspartate. The donor of mesenchymal stem cells was the girl’s father. She received two intravenous infusions of suspended cultured mesenchymal cells in 16 days apart without any side effects. An analysis of procollagen type I secreted to the culture medium by bone marrow-derived mesenchymal stem cells obtained from the patient, 3 months following transplantation revealed its normal triple helix stability. During the subsequent two years of follow up two new bone fractures were noted. Currently a two-year-old girl’s presents extreme growth and weight deficiency. The motoric development is also retarded, but the patient constantly improves and makes progresses.

The sexual maturation in all mammals is the period in which the quiescent gonads are activated by gonadotropins from anterior pituitary, increasing the secretion of sexual hormones. Sexual maturation it is also related with the development of several other body features such as body mass and maturation of the circulatory, skeletal and hematopoietic systems. The aim of the present study was to evaluate the function of neutrophils submitted to in vivo lower and higher concentration of testosterone (sexually immature: 60 days and sexually mature: 90 days). Using different approaches we evaluated cell viability and function and gene expression in rat neutrophils from 60 and 90 days-old animals. Neutrophils from 90 days-old rats showed a decrease in phagocytic and fungicidal capacity, without change in cellular viability. Additionally, we verified that sexual maturation induced increase in production of reactive oxygen and nitrogen species (RONS) and also in TNF-α, IL-6 and IL-10 cytokines. In conclusion, our data suggest that increase in testosterone levels induced significant alteration in neutrophil function, impairing phagocytic capacity.

目的：在既往研究中．从未停止过关于修复和正畸治疗中进行咬合重建时．应该建立何种形式的咬合接触的讨论。对天然恒牙列的动态咬合的流行病学调查将有助于走出这种持续讨论的困境。本研究的目的就是要通过调查研究．总结年青群体和老年群体天然牙列的特征性动态咬合接触形式，并且分析动态咬合接触形式与性别和年龄之间的相关性。材料和方法：本研究共调查了834人。所有的调查对象都具有天然牙列．并且没有修复体存在。调查对象的年龄范围是20--63岁。在调查对象进行牙诱导的侧方和前伸运动过程中，检查天然牙的动态咬合接触形式。结果：结果发现存在以下的动态咬合接触形式：双侧和单侧的尖牙保护[牙合]，组牙功能[牙合]．平衡[牙合]以及“其他[牙合]型”。男性和女性之间不同类型的动态咬合接触形式的出现概率相同。但是．一些咬合接触形式的出现率与年龄有相关性。随着年龄的增加．双侧和单侧尖牙保护耠的出现率出现下降趋势，而组牙功能[牙合]的出现率上升。平衡[牙合]以及“其他[牙合]型”的出现率很低．并且没有呈现与年龄相关的变化趋势。结论：双侧尖牙保护[牙合]和组牙功能[牙合]是年青患者的特征性咬合接触形式．而老年患者中组牙功能[牙合]的接触形式更加普遍。当年青患者接受修复和正畸手段的咬合重建时．尖牙保护[牙合]是一种最适合的咬合接触形式，而对于老年患者来说．组牙功能[牙合]则更加适合。需要进一步的研究来探讨[牙合]型和牙周炎、磨牙症或者颞下颌关节紊乱病之间的关系。
