Epithelial cell identity in hyperplastic precursors of breast cancer

Introduction:In the adult human breast,hyperplastic enlarged lobular unit(HELU) and atypical ductal hyperplasia(ADH) are two common abnormalities that frequently coexist with ductal carcinoma in situ(DCIS).For this reason,they have been proposed as the early steps in a biological continuum toward breast ***:We investigated in silico the expression of 369 genes experimentally recognized as involved in establishing and maintaining epithelial cell identity and mammary gland remodeling,in HELUs or ADHs with respect to the corresponding patient-matched normal ***:Despite the common luminal origin,HELUs and ADHs proved to be characterized by distinct gene profiles that overlap for 5 genes *** HELUs were associated with the overexpression of progesterone receptor(PGR),ADHs were characterized by the overexpression of estrogen receptor 1(ESR1) coupled with the overexpression of some proliferation-associated ***:This unexpected finding contradicts the notion that in differentiated luminal cells the expression of estrogen receptor(ER) is dissociated from cell proliferation and suggests that the establishing of an ER-dependent signaling is able to sustain cell proliferation in an autocrine manner as an early event in tumor *** clinical evidence indicates that only a fraction of HELUs and ADHs evolve to invasive cancer,present findings warn that exposure to synthetic progestins,frequently administered as hormone-replacement therapy,and estrogens,when abnormally produced by adipose cells and persistently present in the stroma surrounding the mammary gland,may cause these hyperplastic lesions.

All non-immortalized mesenchymal stem cells have a limited proliferative potential,that is,replicative senescence(RS)is an integral characteristic of the life of all mesenchymal stem cells(MSCs).It is known that one of the important signs of RS is a decrease of cell motility,and that violations of migration processes contribute to the deterioration of tissue ***,the characterization of the properties of the cell line associated with RS is a prerequisite for the effective use of MSCs in restorative *** of the key proteins regulating cell motility is the small GTPase *** main purpose of this work was to study the nuclear-cytoplasmic redistribution of the RhoA protein during RS in MSC lines recently obtained and characterized in our *** study found that a comparative analysis of the intracellular localization of RhoA in three cell lines(MSCWJ-1,FetMSC,DF2)showed a decrease in the nuclear localization of RhoA during RS.

Effect of infliximab on small bowel stenoses in patients with Crohn's disease

AIM: To assess prospectively small bowel stenoses in Crohn's disease (CD) patients treated with infliximab using Small Intestine Contrast Ultrasonography (SICUS). METHODS: Twenty patients (M 12, age, 42.7 ± 11.8 years), 15 of whom showed obstructive symptoms indicating the presence of small bowel stenosis, and 5 without stenosis, were treated with infliximab (5 mg/kg at wk 0, 2, 6 and 5 mg/kg every 8 wk thereafter) for steroid refractoriness, fistulizing disease, or to avoid high-risk surgery. SICUS was performed at the induction phase and at regular time intervals during the follow-up period of 34.7 ± 16.1 mo (range 7-58). Small bowel stenoses were detected by SICUS, endoscopy and MRI. RESULTS: In no case was progression of stenoses or the appearance of new ones seen. Of the 15 patients with stenosis, 5 stopped treatment after the induction phase (2 for no response, 3 for drug intolerance, one of whom showed complete regression of one stenosis). Among the remaining 10 patients, a complete regression of 8 stenoses (1 stenosis in 5 patients and 3 stenoses in one patient) was observed after 6-22 infliximab infusions. CONCLUSION: In patients with CD treated with infliximab we observed: (a) No progression of small bowel stenosis and no appearance of new ones, (b) Complete regression of 1/22 stenosis after the induction phase and of 8/15 (53.3%) stenosis after 6-22 infusionsduring maintenance therapy.

AIM:To find out if by combining 2 ultrasound based elastographic methods:acoustic radiation force impulse(ARFI)elastography and transient elastography(TE),we can improve the prediction of fibrosis in patients with chronic hepatitis ***:Our study included 197 patients with chronic hepatitis *** each patient,we performed,in the same session,liver stiffness(LS)measurements by means of TE and ARFI,respectively,and liver biopsy(LB),assessed according to the Metavir score.10 LS measurements were performed both by TE and ARFI;median values were calculated and expressed in kilopascals(kPa)and meters/second(m/s),*** TE and ARFI measurements with IQR<30%andSR≥60%were considered ***:On LB 13(6.6%)patients had F0,32(16.2%) had F1,52(26.4%)had F2,47(23.9%)had F3,and 53(26.9%)had F4.A direct,strong correlation was found between TE measurements and fibrosis(r=0.741),between ARFI and fibrosis(r=0.730)and also between TE and ARFI(r=0.675).For predicting significant fibrosis(F≥2),for a cutoff of 6.7 kPa,TE had 77.5% sensitivity(Se)and 86.5%specificity(Sp)[area under the receiver operating characteristic curve(AUROC)0.87] and for a cutoff of 1.2 m/s,ARFI had 76.9%Se and 86.7%Sp(AUROC 0.84).For predicting cirrhosis(F=4),for a cutoff of 12.2 kPa,TE had 96.2%Se and 89.6% Sp(AUROC 0.97)and for a cutoff of 1.8 m/s,ARFI had 90.4%Se and 85.6%Sp(AUROC 0.91).When both elastographic methods were taken into consideration,for predicting significant fibrosis(F≥2),(TE≥6.7 kPa and ARFI≥1.2 m/s)we obtained 60.5%Se,93.3% Sp,96.8%positive predictive value(PPV),41.4%negative predictive value(NPV)and 68%accuracy,while for predicting cirrhosis(TE≥12.2 kPa and ARFI≥1.8 m/s) we obtained 84.9%Se,94.4%Sp,84.9%PPV,94.4% NPV and 91.8%***:TE used in combination with ARFI is highly specific for predicting significant fibrosis;therefore when the two methods are concordant,liver biopsy can be avoided.

Mathematical Modeling of Landfill Gas (MSW)—Production of Gas with Methane Gas Content from Landfills (MSW)

The municipal solid waste (msw) is a source of landfill gas (msw)—with methane gas content. Preoccupations for landfill gas (msw) management date back since 1976 when, at a landfill (msw) in California (USA), it turned out practically that the landfill gas (msw) with methane gas content contains a gas with high caloric value that can be collected and used for economic purposes. The landfill gas (msw) contains methane gas (30% - 60% volume), carbon dioxide (45% - 50% volume), hydrogen sulfide and other gases. Methane gas, carbon dioxide, nitrous oxide and other gases are listed in Kyoto Protocol as high greenhouse gases. Their ecological-rational management is both a national and global preoccupation. In terms of greenhouse gases, especially methane gas, the landfill (msw) is held responsible for 3.5% - 5% of the total global greenhouse gases. Practically, the quantitative estimation of the methane gas in a municipal solid waste landfill can be done by measuring the landfill gas (msw) flow in an extraction-collection well. In Romania, a quantitative estimation relationship of methane gas from deposits (msw) was made, approaching the problem in a different way. This paper presents the calculation formula, the working algorithm, the municipal waste landfill equation and the NOMOGRAMA of a municipal solid waste landfill (msw). The NOMOGRAMA allows us to define the values for parameter -m- (number of months needed for an amount of municipal solid waste (msw) to degrade, starting with the year from which the landfill gas (msw) emission with methane gas content is calculated). Taking into account the environmental conditions for each location of municipal solid waste landfill, the calculation uses various indexes and approximations, while the fundamental parameter remains -m- defined by the NOMOGRAMA of the municipal solid waste landfill (msw). NOMOGRAMA of a Landfill (msw)—Setting <i>m</i>Parameter Values

MSW wastes depositing on a landfill in accordance with the national environment protection legislation means a (msw) waste conglomerate which generates greenhouse effect gas, especially CH4 on an active lifetime of the deposit, but, in the same time, after the waste depositing quit too. As a consequence it is absolutely necessary to estimate gas emission quantities, mainly CH4 annually. This thing can be done by quantitative waste degraded estimation, annually also. This paper describes the adequate technique to be used for the -m- parameters values establishing having in mind that -m- represents the number of months when a certain quantity of msw is degraded, annually, during the lifetime of the msw deposit (landfill). Nevertheless the establishing of the -m- values means, in fact, set up of the msw landfill NOMOGRAMA itself. The wastes agglomeration on a landfill can be expressed through a mathematical equation, whose analyze lead to the -m- values establishing. The waste (msw) will be degraded in time under the specific environmental factors of every region.

A New Approach Method of CH<sub>4</sub>Emission Estimation from Landfills Municipal Solid Waste (MSW)

The CH4 is one of the six Greenhouse Effect Gases (GEG) that is mentioned in the Kyoto Protocol. The GEG is generated by the anthropic activities which are conducive to climate changes if their management is not conducted in a proper way. The main purpose of the environment policy is the reduction of the GEG emission. It is well-known that the CH4 gas emission from municipal solid waste MSW landfills is responsible for 4 ÷ 5% of the total Greenhouse Effect. It is necessary to have a practical method to calculate the quantitative CH4 gas emission, in order to apply an efficient management of the CH4 gas emission from MSW landfills, conforming or non-conforming. This method has to be transparent, credible, coherent, and applicable both for conforming and non-conforming MSW deposits. This paper proposes a new estimation calculation method of the CH4 gas emission from all MSW deposits in Romania. The IPCC group of experts has made recommendations related to the estimation of CH4 but the European Union (EU) admits that the environmental conditions are not the same in every Member State. The annual evolution of CO2 for the CH4 gas emission at every MSW location is valuable information for the Environment Authority with a view to realistic environmental planning and for an efficient policy to be applied in order to reduce the greenhouse effect of MSW landfills.

Circulating adhesion molecules in patients with virus-related chronic diseases of the liver

AIM: In the inflammatory state, intercellular adhesion molecule-1 (ICAM-1) and vascular cellular adhesion molecule-1 (VCAM-1) play a key role in promoting migration of immunological cells from the circulation to target *** of our study was to investigate soluble forms of these molecules in patients with virus-related chronic liver diseases, to assess their behavior in different pathologies and correlation with severity of liver ***: Circulating ICAM-1 and VCAM-1 were assayed by EIA commercial kits (R&D System Co.,Abington, UK) in 23 patients with chronic active hepatitis (CH), 50 subjects affected by liver cirrhosis (LC) and 15 healthy controls comparable for sex and age. In patients, serum alanine aminotransferase (ALT) and aspartate aminotransferase (AST) were also detected by ***: LC patients had significantly higher ICAM-1 values than CH patients (38.56±7.4 ng/mL vs 20.89±6.42 ng/mL; P＜0.001) and these ones had significantly higher values than controls (12.92±1.08 ng/mL; P＜0.001). In CH group, ICAM-1 levels were significantly related to inflammatory activity (P= 0.041) and ALT values (r= 0.77;P＜0.05). VCAM-1 values were significantly increased only in LC patients (P＜0.001) and related to severity of liver ***: These findings suggest that the determination of serum ICAM-1 can be considered as an additional useful marker of hepatocellular necrosis and inflammatory activity in chronic hepatitis, while serum VCAM-1 is an indicator of liver fibrogenesis and severity of disease in cirrhosis.
