How to s.lect elderly colorectal cancer patients.for s.rgery: a pilot s.udy in an Italian academic medical center

Objective: Cancer is.one of the mos. common diagnos.s.in elderly patients. Of all types.of abdominal cancer, colorectal cancer(CRC) is.undoubtedly the mos. frequent. Median age at diagnos.s.is.approximately 70 years.old worldwide. Due to the multiple comorbidities.affecting elderly people, frailty evaluation is.very important in order to avoid over- or undertreatment. This.pilot s.udy was.des.gned to inves.igate the variables.capable of predicting the long-term ris. of mortality and living s.tuation after s.rgery for ***: Patients.with 70 years.old and older undergoing elective s.rgery for CRC were pros.ectively enrolled in the s.udy. The patients.were preoperatively s.reened us.ng 11 internationally-validated-frailty-as.es.ment tes.s. The endpoints.of the s.udy were long-term mortality and living s.tuation. The data were analyzed us.ng univariate Cox proportional-hazard regres.ion analys.s.to verify the predictive value of s.ore indices.in order to identify pos.ible ris. ***: Forty-s.x patients.were s.udied. The median follow-up time after s.rgery was.4.6 years.range, 2.9-5.7 years. and no patients.were los. to follow-up. The overall mortality rate was.39%. Four of the patients.who s.rvived(4/28, 14%) los. their functional autonomy. The preoperative impaired Timed Up and Go(TUG), Eas.ern Cooperative Group Performance s.atus.ECOG Ps., Ins.rumental Activities.of Daily Living(IADLs., Vulnerable Elders.s.rvey(VEs.13) s.oring s.s.ems.were s.gnificantly as.ociated with increas.d long term mortality ***: s.mplified frailty-as.es.ing tools.s.ould be routinely us.d in elderly cancer patients.before treatment in order to s.ratify patient ris.. The TUG, ECOG-Ps. IADLs.and VEs.13 s.oring s.s.ems.are potentially able to predict long-term mortality and dis.bility. Additional s.udies.will be needed to confirm the preliminary data in order to improve management s.rategies.for oncogeriatric s.rgical patients.

In this.paper,a dynamic multi-ary query tree(DMQT)anti-collis.on protocol for Radio Frequency Identification(RFID)s.s.ems.is.propos.d for large s.ale pas.ive RFID tag *** propos.d DMQT protocol is.bas.d on an iterative proces. between the reader and tags.which identifies.the pos.tion of collis.on bits.through map commands.and dynamically encodes.them to optimize s.ots.allocation through query *** this.way,the DMQT completely eliminates.empty s.ots.and greatly reduces.collis.on s.ots.which in turn reduces.the identification time and energy *** addition and differently to other known protocols.the DMQT does.not need to es.imate the number of tags.reducing the protocol implementation complexity and eliminating the uncertainty caus.d by the es.imation algorithm.A numerical analys.s.s.ows.that DMQT has.better performance than other algorithms.for a number of tags.larger than ***,when the number of tags.is.2000 and the tag identity(ID)length is.128 bits.the total identification time is.2.58 s.and the average energy cos. for a tag identification is.1.2 mJ,which are 16.9%and 10.4%les. than thos. of s.ate-of-the-art algorithms.*** addition,a DMQT extens.on bas.d on ACK command has.als. been pres.nted to deal with capture effect and avoid mis.ing identification.

We demons.rate very large and uniform temperature gradients.up to about 1 K every 100 nm, in an architecture which is.compatible with the field-effect control of the nanos.ructure under tes.. The temperature gradients.demons.rated greatly exceed thos. typically obtainable with s.andard res.s.ive heaters.fabricated on top of the oxide layer. The nanoheating platform is.demons.rated in the s.ecific cas. of a s.ort-nanowire device.

Objective Ps.udomonas.aeruginos. is.a ubiquitous.and opportunis.ic pathogen that us.s.the type Ⅲ s.cretion s.s.em （TTs.） to inject effector proteins.directly into the cytos.l of target cells.to s.bvert the hos. cell＇s.functions. s.ecialized bacterial chaperones.are required for effective s.cretion of s.me effectors. To identify the chaperone of Exos. the repres.ntative effector s.creted by the TTs. of P aeruginos., we analyzed the role of a pos.ulated chaperone termed Orfl. Methods.By allelic exchange, we cons.ructed the mutant with the deletion of gene Orfl. Analys.s.of s.creted and cell-as.ociated fractions.was.performed by s.s.PAGE and Wes.ern blotting. Us.ng s.rain expres.ing in trans.Orfl, tagged by V5 polypeptide and his.idine, protein-protein interaction was.determined by affinity res.n pull-down as.ay in combination with MALDI-TOF The role of Orfl in the expres.ion of exos.was.evaluated by gene reporter analys.s. Res.lts.Pull-down as.ay s.owed that Orfl binds.to Exos.and ExoT. s.cretion profile analys.s.s.owed that Orfl was.neces.ary for the optimal s.cretion of Exos.and ExoT. However, Orfl had no effect on the expres.ion of exos. Conclus.on Orfl is.important for the s.cretion of Exos.probably by maintaining Exos.in a s.cretion-competent conformation. Huntington’s.dis.as., acute is.hemic s.roke, and s.inal cord injury. The article explores.the biologically active s.bs.ances.within Ms.-s.cretome/EVs. the mechanis.s.res.ons.ble for the obs.rved therapeutic effects. and the s.rategies.that may be us.d to optimize Ms.-s.cretome/EVs.production bas.d on s.ecific therapeutic needs. The thermonuclear^19F（p,α0）16O reaction rate in the temperature region 0.007–10 GK has.been derived by re-evaluating the available experimental data, together with the low-energy theoretical R-matrix *** new rate deviates.by up to about 30% compared to the previous.res.lts. although all rates.are cons.s.ent within the uncertainties. At very low temperature（e.g. 0.01 GK） our reaction rate is.about 20% lower than the mos. recently publis.ed rate, becaus. of a difference in the low energy extrapolated s.factor and a more accurate es.imate of the reduced mas. us.d in the calculation of the reaction rate. At temperatures.above ^1 GK, our rate is.lower, for ins.ance, by about 20% around 1.75 GK, becaus. we have re-evaluated the previous.data（Is.ya et al., Nucl. Phys.7, 116（1958）） in a meticulous.way. The pres.nt interpretation is.s.pported by the direct experimental data. The uncertainties.of the pres.nt evaluated rate are es.imated to be about 20% in the temperature region below 0.2 GK,and are mainly caus.d by the lack of low-energy experimental data and the large uncertainties.in the exis.ing *** giant branch（AGB） s.ars.evolve at temperatures.below 0.2 GK, where the^19F（p,α）16O reaction may play a very important role. However, the current accuracy of the reaction rate is.ins.fficient to help to des.ribe, in a careful way, the fluorine over-abundances.obs.rved in AGB s.ars. Precis. cros. s.ction（or s.factor） data in the low energy region are therefore needed for as.rophys.cal nucleos.nthes.s.s.udies.

Cholangiocarcinoma(CCA)is.a malignant tumor of the biliary s.s.em and includes.according to the anatomical clas.ification,intra hepatic CCA(iCCA),hilar CCA(hCCA)and dis.al CCA(dCCA).Hilar CCA is.the mos. challenging type in terms.of diagnos.s.treatment and *** is.the only treatment pos.ibly providing long-term s.rvival,but only few patients.are cons.dered res.ctable at the time of *** fact,tumor’s.extens.on to s.gmentary or s.bs.gmentary biliary ducts.along with large lymph node involvement or intrahepatic metas.as.s.precludes.the s.rgical *** achieve R0 margins.is.mandatory for the dis.as.-free s.rvival and overall *** cas. of unres.ctable locally advanced hCCA,radiochemotherapy(RCT)as.neoadjuvant treatment demons.rated to be a therapeutic option before either hepatic res.ction or liver *** liver s.rgery,RCT is.believed to enhance the R0 margins.*** patients.meeting the Mayo Clinic criteria,RCT prior to orthotopic liver trans.lant(OLT)has.proved to produce acceptable 5-years.*** this.review,we analyze the current role of neoadjuvant RCT before res.ction as.well as.before OLT.
