Genetic association of interleukin-6 polymorphism (-174 G/C) with chronic liver diseases and hepatocellular carcinoma

Interleukin-6 (IL-6) is a pleiotropic cytokine which is expressed in many inflammatory cells in response to different types of stimuli, regulating a number of biological processes. The IL-6 gene is polymorphic in both the 5' and 3' flanking regions and more than 150 single nucleotide polymorphisms have been identified so far. Genetic polymorphisms of IL-6 may affect the outcomes of several diseases, where the presence of high levels of circulating IL-6 have been correlated to the stage and/or the progression of the disease itself. The -174 G/C polymorphism is a frequent polymorphism, that is located in the upstream regulatory region of the IL-6 gene and affects IL-6 production. However, the data in the literature on the genetic association between the -174 G/C polymorphism and some specific liver diseases characterized by different etiologies are still controversial. In particular, most of the studies are quite unanimous in describing a correlation between the presence of the high-producer genotype and a worse evolution of the chronic liver disease. This is valid for patients with hepatitis C virus (HCV)-related chronic hepatitis and liver cirrhosis and hepatocellu-lar carcinoma (HCC) whatever the etiology. Studies in hepatitis B virus-related chronic liver diseases are not conclusive, while specific populations like non alcoholic fatty liver disease/non-alcoholic steatohepatitis, autoimmune and human immunodeficiency virus/HCV coinfected patients show a higher prevalence of the lowproducer genotype, probably due to the complexity of these clinical pictures. In this direction, a systematic revision of these data should shed more light on the role of this polymorphism in chronic liver diseases and HCC.

Improved Lattice Boltzmann Without Parasitic Currents for Rayleigh-Taylor Instability

Over the last decade the Lattice Boltzmann Method (LBM) has gained significant interest as a numerical solver for multiphase flows. However most of the LBvariants proposed to date are still faced with discreteness artifacts in the form of spurious currents around fluid-fluid interfaces. In the recent past, Lee et al. have proposeda new LB scheme, based on a higher order differencing of the non-ideal forces, whichappears to virtually free of spurious currents for a number of representative *** this paper, we analyze the Lee method and show that, although strictly speaking, itlacks exact mass conservation, in actual simulations, the mass-breaking terms exhibita self-stabilizing dynamics which leads to their disappearance in the long-term evolution. This property is specifically demonstrated for the case of a moving droplet atlow-Weber number, and contrasted with the behaviour of the Shan-Chen model. Furthermore, the Lee scheme is for the first time applied to the problem of gravity-drivenRayleigh-Taylor instability. Direct comparison with literature data for different values of the Reynolds number, shows again satisfactory agreement. A grid-sensitivitystudy shows that, while large grids are required to converge the fine-scale details, thelarge-scale features of the flow settle-down at relatively low resolution. We concludethat the Lee method provides a viable technique for the simulation of Rayleigh-Taylorinstabilities on a significant parameter range of Reynolds and Weber numbers.

An evaluation of intraoperative and postoperative outcomes of torsional mode versus longitudinal ultrasound mode phacoemulsification:a Meta-analysis

AIM： To evaluate and compare the intraoperative parameters and postoperative outcomes of torsional mode and longitudinal mode of phacoemulsification. METHODS： Pertinent studies were identified by a computerized MEDLINE search from January 2002 to September 2013. The Meta-anaiysis is composed of two parts. In the first part the intraoperative parameters were considered： ultrasound time （UST） and cumulative dissipated energy （CDE）. The intraoperative values were also distinctly considered for two categories （moderate and hard cataract group） depending on the nuclear opacity grade. In the second part of the study the postoperative outcomes as the best corrected visual acuity （BCVA） and the endothelial cell loss （ECL） were taken in consideration. RESULTS： The UST and CDE values proved statistically significant in support of torsional mode for both moderate and hard cataract group. The analysis of BCVA did not present statistically significant difference between the two surgical modalities. The ECL count was statistically significant in support of torsional mode （P〈0.001）. CONCLUSION： The Meta -analysis shows the superiority of the torsional mode for intraoperative parameters （UST, CDE） and postoperative ECL outcomes.

Colorectal cancer (CRC) is a common neoplasia in the Western countries, with considerable morbidity and mortality. Every fifth patient with CRC presents with metastatic disease, which is not curable with radical intent in roughly 80% of cases. Traditionally approached surgically, by resection of the primitive tumor or stoma, the management to incurable stage IV CRC patients has significantly changed over the last three decades and is nowadays multidisciplinary, with a pivotal role played by chemotherapy (CHT). This latter have allowed for a dramatic increase in survival, whereas the role of colonic and liver surgery is nowadays matter of debate. Although any generalization is difficult, two main situations are considered, asymptomatic (or minimally symptomatic) and severely symptomatic patients needing aggressive management, including emergency cases. In asymptomatic patients, new CHT regimens allow today long survival in selected patients, also exceeding two years. The role of colonic resection in this group has been challenged in recent years, as it is not clear whether the resection of primary CRC may imply a further increase in survival, thus justifying surgery-related morbidity/mortality in such a class of short-living patients. Secondary surgery of liver metastasis is gaining acceptance since, under new generation CHT regimens, an increasing amount of patients with distant metastasis initially considered non resectable become resectable, with a significant increase in long term survival. The management of CRC emergency patients still represents a major issue in Western countries, and is associated to high morbidity/mortality. Obstruction is traditionally approached surgically by colonic resection, stoma or internal by-pass, although nowadays CRC stenting is a feasible option. Reconstructing hourly residential electrical load profiles for Renewable Energy Communities using non-intrusive machine learning techniques

The successful implementation of Renewable Energy Communities(RECs)involves maximizing the self-consumption within a community,particularly in regulatory contexts in which shared energy is *** many countries,the absence of a metering infrastructure that provides data at an hourly or sub-hourly resolution level for low-voltage users(e.g.,residential and commercial users)makes the design of a new energy community a challenging *** study proposes a non-intrusive machine learning methodology that can be used to generate residential electrical consumption profiles at an hourly resolution level using only monthly consumption data(i.e.,billed energy),with the aim of estimating the energy shared by *** proposed methodology involves three phases:first,identifying the typical load patterns of residential users through k-Means clustering,then implementing a Random Forest algorithm,based on monthly energy bills,to identify typical load patterns and,finally,reconstructing the hourly electrical load profile through a data-driven rescaling *** effectiveness of the proposed methodology has been evaluated through an REC case study composed by 37 residential users powered by a 70 kWp photovoltaic *** Normalized Mean Absolute Error(NMAE)and the Normalized Root Mean Squared Error(NRMSE)were evaluated over an entire year and whenever the energy was shared within the *** Relative Absolute Error was also measured when estimating the shared energy at both a monthly(MRAE)and at an annual basis.(RAE).A comparison between the REC load profile reconstructed using the proposed methodology and the real load profile yielded an overall NMAE of 20.04%,an NRMSE of 26.17%,and errors of 18.34%and 23.87%during shared energy timeframes,***,our model delivered relative absolute errors for the estimation of the shared energy at a monthly and annual scale of 8.31%and 0.12%,respectively.

Beginning with precise data on the ratio of structure functions in deep inelastic scattering(DIS)from ^3He and ^3H,collected on the domain 0.19≤xB≤0.83,where xBis the Bjorken scaling variable,we employ a robust method for extrapolating such data to arrive at a model-independent result for the xB=1 value of the ratio of neutron and proton structure *** this with information obtained in analyses of DIS from nuclei,corrected for target-structure dependence,we arrive at a prediction for the proton valence-quark ratio:d_(v)/u_(v)|_(xB→1)=0.230(57).Requiring consistency with this result presents a challenge to many descriptions of proton structure.
