The duration of the conventional chemoembolization for hepatocellular carcinoma:factors affecting the procedural time

Aim:The present study evaluated the duration of chemoembolization in patients with hepatocellular carcinoma,analyzing possible factors affecting the procedural ***:In total,175 patients who underwent chemoembolization have been prospectively *** procedural length was considered the time between the insertion and the removal of the angiographic *** features related to the tumor burden and angiographic procedures,which could be related to the procedural time,were ***:The chemoembolization time resulted in a mean of 58.1 *** longer procedural time was associated with a number of nodules treated per patient≥2(P<0.001),a number of segments with nodules≥2(P<0.001),the presence of more than 1 nodule in the same segment(P<0.001),the location of the tumor in the left lobe(P=0.001),the exclusion from the Milan criteria(P<0.001),and a number of segments treated≥2(P<0.001).Only the number of nodules treated per patient resulted significantly in multivariate analysis(OR 2.927,95%CI:2.015-4.251,P<0.001).Conclusion:The factors related to longer procedural time are the number of nodules treated≥2,the number of segments with nodules≥2,the involvement of the left lobe,the tumor burden outside the Milan criteria,and the number of segments treated≥*** these characteristics,known in the pre-procedural phase,represent useful tools for a correct planning of the angiographic room’s workflow during the pandemic era as well as in the future to reduce downtime and increase productivity.

Background:Platinum chemotherapy(CT)remains the backbone of systemic therapy for patients with smallcell lung cancer(SCLC).The nucleotide excision repair(NER)pathway plays a central role in the repair of the DNA damage exerted by platinum *** in this repair mechanism may affect patients’*** and Methods:We conducted a retrospective analysis of data from 38 patients with extensive disease(ED)-SCLC who underwent platinum-CT at the Clinical Oncology Unit,Careggi University Hospital,Florence(Italy),from 2015 to *** expression analysis and single nucleotide polymorphism(SNP)characterization of three NER pathway genes—namely ERCC1,ERCC2,and ERCC5—were performed on patient tumor ***:Overall,elevated expression of ERCC genes was observed in SCLC patients compared to healthy *** with low ERCC1 and ERCC5 expression levels exhibited a better median progression-free survival(mPFS=7.1 vs.4.9 months,p=0.39 for ERCC1 and mPFS=6.9 vs.4.8 months,p=0.093 for ERCC5)and overall survival(mOS=8.7 vs.6.0 months,p=0.4 for ERCC1 and mOS=7.2 vs.6.2 months,p=0.13 for ERCC5).Genotyping analysis of five SNPs of ERCC genes showed a longer survival in patients harboring the wild-type genotype or the heterozygous variant of the ERCC1 rs11615 SNP(p=0.24 for PFS and p=0.14 for OS)and of the rs13181 and rs1799793 ERCC2 SNPs(p=0.43 and p=0.26 for PFS and p=0.21 and p=0.16 for OS,respectively)compared to patients with homozygous mutant ***:The comprehensive analysis of ERCC gene expression and SNP variants appears to identify patients who derive greater survival benefits from platinum-CT.

A non-compact polyhedron P is Tucker if, for any compact subset K begong to P, the fundamental group π1 （P - K） is finitely generated. The main result of this note is that a manifold which is proper homotopy equivalent to a Tucker polyhedron is Tucker. We use Poenaru＇s theory of the equivalence relations forced by the singularities of a non-degenerate simplicial map.

In the present work the aerodynamic performances of an innovative rotor blade row have been experimentally investigated. Measurements have been carried out in a large scale low speed single stage cold flow facility at a Reynolds number typical of aeroengine cruise, under nominal and off-design conditions. The time-mean blade aerodynamic loadings have been measured at three radial positions along the blade height through a pressure transducer installed inside the hollow shaft, by delivering the signal to the stationary frame with a slip ring. The time mean aerodynamic flow fields upstream and downstream of the rotor have been measured by means of a five-hole probe to investigate the losses associated with the rotor. The investigations in the single stage research turbine allow the reproduction of both wake-boundary layer interaction as well as vortex-vortex interaction. The detail of the present results clearly highlights the strong dissipative effects induced by the blade tip vortex and by the momentum defect as well as the turbulence production, which is generated during the migration of the stator wake in the rotor passage. Phase-locked hot-wire investigations have been also performed to analyze the time-varying flow during the wake passing period. In particular the interaction between stator and rotor structures has been investigated also under off-design conditions to further explain the mechanisms contributing to the loss generation for the different conditions.

A bump is typically used in the inlet system of an aircraft engine to compress the incoming airflow and to reduce boundary layer thickness developed over *** this work,the turbulent flow over a three-dimensional bump is experimentally *** bump model is mounted in a closed return wind tunnel operated at the nominal velocity 10 m/s,corresponding to a friction Reynolds number of *** flow field upstream the bump,along the bump centerline and at two different spanwise planes is measured with Particle Image Velocimetry(PIV).It is observed that a favorable pressure gradient develops until the suction peak of the bump,and that the average turbulence intensity within boundary layer is attenuated due to this favorable pressure *** boundary layer thickness identified by examining profiles of streamwise velocity decreases significantly along the *** wall-normal position of the Turbulent/Non-Turbulent Interface(TNTI)identified with the vorticity criterion is also observed to decrease along the *** studying the behavior of the boundary layer thickness at different spanwise positions,we found that it tends to be larger in planes away from the centreline,which suggests that the bump diverts the flow.

The effects of Sr addition and pressure increase on the microstructure and casting defects of a low-pressure die cast （LPDC） AISi7Mg0.3 alloy have been studied. Metallographic and image analysis techniques have been used to quantitatively examine the microstructural changes and the amount of porosity occurring at different Sr levels and pressure parameters. The results indicate that an increase in the filling pressure induces lower heat dissipation of the liquid close to the die/core surfaces, with the formation of slightly greater dendrite arms and coarser eutectic Si particles. On the other hand, the increase in the Sr level leads to finer microstructural scale and eutectic Si. The analysed variables, within the experimental conditions, do not affect the morphology of eutectic Si particles. Higher applied pressure and Sr content generate castings with lower amount of porosiW. However, as the filling pressure increases the flow of metal inside the die cavity is more turbulent, leading to the formation of oxide films and cold shots. In the analysed range of experimental conditions, the design of experiment methodology and the analysis of variance have been used to develop statistical models that accurately predict the average size of secondary dendrite arm spacing and the amount of porosity in the low-pressure die cast AISiTMg0.3 alloy.

In this paper,it is shown that the performances of a class of high-gain practical observers can be improved by estimating the time derivatives of the output up to an order that is greater than the dimension of the system, which is assumed to be in observability form and, possibly, time-varying. Such an improvement is achieved without increasing the gain of the observers, thus allowing their use in a wide variety of control and identification applications.
