This case report describes a woman with spontaneous rectal perforation from decidualized endometriosis in pregnancy.A 37-year-old woman was admitted to our hospital at 30 wk of pregnancy with symptoms suggestive of pyelonephritis,which persisted until 33 wk of gestation when delivery of a premature male baby was performed through a cesarean section. On postoperative day 2,an abdominal computed tomography showed free air in the peritoneal cavity and a pelvic *** celiotomy revealed a diffuse severe fecaloid peritonitis that originated from a 3-cm wide rectal perforation.A Hartmann operation was then *** findings wereconsistent with decidualization of the rectal *** 20 cases of intestinal perforation due to endometriosis have been reported in the *** report is believed to be the first case of spontaneous rectal perforation from endometriosis in pregnancy,and it shows the potential occurrence of serious and unexpected complications of the disease.

AIM: To report the long-term outcomes of combined excisional goniotomy and manual small incision cataract surgery(MSICS).METHODS: This is a retrospective case series of patients with open angle glaucoma and visually significant cataracts that underwent combined excisional goniotomy and MSICS with one-year follow-up. The medical history, demographic information, and clinical characteristics of each case were recorded. Data regarding changes in vision, intraocular pressure(IOP), the number of glaucoma medications, and the evolution of the disease after surgery were reported. RESULTS: Three patients, with open angle glaucoma and cataracts underwent combined excisional goniotomy and MSICS without adverse events. All patients had improvement in vision compared to baseline measurements. The range of IOP at baseline was from 14 to 18 mm Hg and decrease to a range of 10 to 14 mm Hg after one year of follow-up. Additionally, two patients also decreased their dependence on IOP-lowering medications at the last follow up visit with one patient maintaining baseline level of medication ***: A combination of excisional goniotomy and MSICS illustrates both the safety and efficacy to treat patients with visually significant cataract and glaucoma. This procedure allows for a more cost-effective surgical approach that matches the needs of resource strained territories around the globe.

Pancreatic ductal adenocarcinoma is one of the most aggressive solid tumours of the pancreas, characterised by a fve-year survival rate less than 8%. Recent reports that pancreatic cancer stem cells （PCSCs） contribute to the tumorigenesis, progression, and chemoresistance of pancreatic cancer have prompted the investigation of new therapeutic approaches able to directly target PCSCs. In the present paper the non-cancer related drugs that have been proposed to target CSCs that could potentially combat pancreatic cancer are revi-ewed and evaluated. The role of some pathways and deregulated proteins in PCSCs as new therapeutic tar-gets are also discussed with a focus on selected speci-fic inhibitors. Finally, advances in the development of nanoparticles for targeting PCSCs and site-specifc drug delivery are highlighted, and their limitations considered.

The first objective of this paper is to study the influence of the orthotic device on the maximum values of stresses in knee cartilages by using Ansys Workbench 14.5 software and applying the Finite Element Analysis (FEA) on a virtual assembly composed by an orthotic device and osteoarthritic knee (OAK). The second objective consists into quantifying and investigating the nonlinear motion of the human knee joint for OAK patients, with and without the orthotic device mounted on OAK, using tools of dynamics stability analysis. The short Lyapunov Exponents (LEs) are calculated, as measures of human knee and ankle joints stability, based on the experimental time series collected by using the biometrics acquisition system during walking on horizontal and inclined treadmills from a sample of healthy subjects and a sample of patients suffering by OAK disease. The values of LEs obtained for OAK patients are larger on the inclined treadmill than on horizontal treadmill and are larger than those obtained for healthy knees, being associated with more divergence and less stability. The results confirm that the influence of an orthotic device mounted on OAK on its stability is significant, the values obtained for LEs being smaller than those calculated for OAK, and closer to the values of normal knees of patients and of healthy subjects.

Vocal divergence within species often corresponds to morphological, environmental, and genetic differences between populations. Wolf howls are long-range signals that encode individual, group, and subspecies differences, yet the factors that may drive this variation are poorly understood. Furthermore, the taxonomic division within the Canis genus remains contended and additional data are required to clarify the position of the Himalayan, North African, and Indian wolves within Canis lupus. We recorded 451 howls from the 3 most basal wolf lineages-Himalayan C. lupus chanco-Himalayan haplotype, North African C. lupus lupaster, and Indian C. lupus pallipes wolves-and present a howl acoustic description within each clade. With an additional 619 howls from 7 Holarctic subspecies, we used a random forest classifier and principal component analysis on 9 acoustic parameters to assess whether Himalayan, North African, and Indian wolf howls ex- hibit acoustic differences compared to each other and Holarctic wolf howls. Generally, both the North African and Indian wolf howls exhibited high mean fundamental frequency （F0） and short duration compared to the Holarctic clade. In contrast, the Himalayan wolf howls typically had lower mean F0, unmodulated frequencies, and short howls compared to Holarctic wolf howls. The Himalayan and North African wolves had the most acoustically distinct howls and differed significantly from each other and to the Holarctic wolves. Along with the influence of body size and environmental differences, these results suggest that genetic divergence and/or geographic distance may play an important role in understanding howl variation across subspecies.

Technological advances are increasing the range of applications for artificial intelligence, especially through its embodiment within humanoid robotics platforms. This promotes the development of novel systems for automated screening of neurological conditions to assist the clinical practitioners in the detection of early signs of mild cognitive impairments. This article presents the implementation and the experimental validation of the first robotic system for cognitive assessment, based on one of the most popular platforms for social robotics, Softbank "Pepper", which administers and records a set of multi-modal interactive tasks to engage the user cognitive abilities. The robot intelligence is programmed using the state-of-the-art IBM Watson AI Cloud services, which provide the necessary capabilities for improving the social interaction and scoring the tests. The system has been tested by healthy adults (N= 35) and we found a significant correlation between the automated scoring and one of the most widely used Paper-and-Pencil tests. We conclude that the system can be considered as a screening instrument for cognitive assessment.

Intratumoral sampling variability in hepatocellular carcinoma: A case report

The differential diagnosis between hepatocellular carcinoma （HCC） and regenerative liver nodules and other primary liver tumors may be very difficult, particularly when performed on liver biopsies. Difficulties in histological typing may be often minimized by immunohistochemistry. Among the numerous markers proposed, CK18, Hep Par1 and glypican 3 （GPC3） are considered the most useful in HCC diagnosis. Here we report a case of HCC in a 72-year-old male with HBV- related chronic liver disease, characterized by a marked morphological and immunohistochemical intratumoral variability. In this case, tumor grading ranged from areas extremely well differentiated, similar to regenerative nodule, to undifferentiated regions, with large atypical multinucleated cells. While almost all sub nodules were immunostained by Hep Par 1, immunoreactivity for glypican 3 and for Ckl8 was patchy, with negative tumor region adjacent to the highly immunoreactive areas. Our case stresses the relevance of sampling variability in the diagnosis of HCC, and indicates that caution should be taken in grading an HCC and in the interpretation of immunohistochemical stains when only small core biopsies from liver nodules are available.
