Laparoscopic distal pancreatectomy in Italy:a systematic review and meta-analysis

BACKGROUNd： The use of laparoscopic distal pancreatectomy（LdP） increased in the past twenty years but the real diffusion of this technique is still unknown as well as the type of centers（high or low volume） in which this procedure is more frequently *** SOURCe： A systematic review was performed to evaluate the frequency of LdP in Italy and to compare indications and results in high volume centers（HVCs） and in low volume centers（LVCs）.ReSULTS： From 95 potentially relevant citations identified, only5 studies were included. A total of 125 subjects were analyzed, of whom 95（76.0%） were from HVCs and 30（24.0%） from *** mean number of LdPs performed per year was 6.5. The mean number of patients who underwent LdP per year was 8.8in HVCs and 3.0 in LVCs（P0.001）. The most frequent lesions operated on in HVCs were cystic tumors（62.1%, P0.001） while,in LVCs, solid neoplasms（76.7%, P0.001）. In HVCs, malignant neoplasms were treated with LdP less frequently than in LVCs（17.9% vs 50.0%, P〈0.001）. Splenectomy was performed for non-oncologic reason frequenter in HVCs than in LVCs（70.2%vs 25.0%, P0.004）. The length of stay was shorter in HVCs than in LVCs（7.5 vs 11.3, P0.001）. No differences were found regarding age, gender, ductal adenocarcinoma treated, operative time, conversion, morbidity, postoperative pancreatic fistula,reoperation and margin ***： LdPs were frequently performed in *** ＂HVC approach＂ is characterized by a careful selection of patients undergoing LdP. The ＂LVC approach＂ is based on the hypothesis that LdPs are equivalent both in short-term and long-term results to laparotomic approach. These data are not conclusive and they point out the need for a national register of laparoscopic pancreatectomy.

Synthesis of Some New Arylazothiophene and Arylazopyrazole derivatives as Antitumor Agents

The starting 1-phenylbutane-1,3-dione (1) was used as key intermediate for the synthesis of several new thiophene and pyrazole derivatives. The newly synthesized compounds were evaluated for in vitro cytotoxicity against an ehrlich ascites cells and in vivo cytotoxicity for compound 10d using eAC assay and 5-fluorouracil is used as reference drug. Compounds 7c, e and 10c, d showed significant activity in certain cancer cell and have been targeted for further studies, compound 10d is more effective and showed the highest activity. Structures of the newly prepared compounds were confirmed by both spectral, analytical data and molecular calculations.

Temperature influences the distribution, range, and phenology of plants. The key transcriptional activators of heat shock response in eukaryotes, the heat shock factors （HSFs）, have undergone large-scale gene amplification in plants. While HSFs are central in heat stress responses, their role in the response to ambient temperature changes is less well understood. We show here that the warm ambient temperature transcriptome is dependent upon the HSFA1 clade ofArabidopsis HSFs, which cause a rapid and dynamic eviction of H2A.Z nucleosomes at target genes. A transcriptional cascade results in the activation of multiple downstream stress-responsive transcription factors, triggering large-scale changes to the transcriptome in response to elevated temperature. H2A.Z nucleosomes are enriched at temperature-responsive genes at non-inducible temperature, and thus likely confer inducibility of gene expression and higher responsive dynamics. We propose that the antagonistic effects of H2A.Z and HSF1 provide a mechanism to activate gene expression rapidly and precisely in response to temperature, while preventing leaky transcription in the absence of an activation signal.

Novel 3-substituted pyridothienopyrimidine derivatives have been synthesized via the reaction of 2-{7,9-dimethyl-4-oxopyrido[3′,2′：4,5]thieno[3,2-d]pyrimidin-3（4H）-yl}acetohydrazide（5） with a variety of activereagents and chemicals. Structures of the newly synthesized compounds were established based on spectral *** resulting pyridothienopyrimidine derivatives were evaluated for their possible antimicrobial activity and some ofthem represent a new class of potentially antimicrobial compounds, especially compounds 9 and 18 which displayedthe highest activity against Gram-positive bacteria, Gram-negative bacteria, and Fungi in MIC range of 0.12--1.95 μg/mL.

A Comparison of the Clinical and Molecular diagnosis of Herpes Simplex Keratitis

Purpose: To compare the clinical and molecular diagnoses of Herpes Simplex Keratitis (HSK). Materials and Methods: Conjunctival swabs (after fluorescein and anaesthetic wash out) and detailed questionnaire data were obtained from 146 participants. Corneal rims and conjunctival epithelial cells were infected with Herpes Simplex Virus (HSV) type 1 or HSV2 and supernatant collected. HSV1;HSV2;Varicella Zoster Virus (VZV) and Adenovirus (AdV) dNA was assessed using two real time Polymerase Chain Reaction (PCR) methods. Results: Of the 146 participants recruited, 54 were clinically diagnosed with typical epithelial lesions and 38 with atypical epithelial lesions, 17 with old inactive HSK and 37 healthy volunteers. HSV1 dNA was detected in 28 (30%) of the 92 participants with clinically suspect HSK. Patients who presented with typical epithelial lesions had a higher positive rate (46%) than those who presented with atypical type lesions (8%), when using primers against the Glycoprotein (Gp) G region of the virus. When the same samples were retested with primers against the GpB region, the positive rate for the typical and atypical cases increased to 52% and 11% respectively. Antiviral use at the time of sampling reduced the rate of PCR positivity by 20% (p dV dNA was detected in 6% of the typical cases and 8% of the atypical cases. All control participants with no history of HSK were negative for HSV1 dNA. Sample quantity was confirmed by testing for housekeeping control genes, beta-actin and beta-2 macroglobulin. PCR results from in vitro control investigations of HSV1 and 2 infected corneal rims and conjunctival epithelial cells were 100% positive for infected and 100% negative for uninfected samples when assessed using both PCR methods. Conclusions: Clinical diagnosis of typical HSK is not always confirmed by PCR. Concomitant use of an antiviral reduces levels of PCR positivity.
