Colonoscopy is an effective screening procedure in colorectal cancer prevention programs;however,colonoscopy practice can vary in terms of lesion detection,classification,and *** intelligence(AI)-assisted decision support systems for endoscopy is an area of rapid research and *** systems promise improved detection,classification,screening,and surveillance for colorectal polyps and *** recently developed applications for AIassisted colonoscopy have shown promising results for the detection and classification of colorectal polyps and ***,their value for real-time application in clinical practice has yet to be determined owing to limitations in the design,validation,and testing of AI models under real-life clinical *** these current limitations,ambitious attempts to expand the technology further by developing more complex systems capable of assisting and supporting the endoscopist throughout the entire colonoscopy examination,including polypectomy procedures,are at the concept ***,further work is required to address the barriers and challenges of AI integration into broader colonoscopy practice,to navigate the approval process from regulatory organizations and societies,and to support physicians and patients on their journey to accepting the technology by providing strong evidence of its accuracy and *** article takes a closer look at the current state of AI integration into the field of colonoscopy and offers suggestions for future research.

Systematic review of colorectal cancer screening guidelines for average-risk adults: Summarizing the current global recommendations

AIM To summarize and compare worldwide colorectal cancer(CRC) screening recommendations in order to identify similarities and *** A systematic literature search was performed using MEDLINE, EMBASE, Scopus, CENTRAL and ISI Web of knowledge identifying all average-risk CRC screening guideline publications within the last ten years and/or position statements published in the last 2 years. In addition, a hand-search of the webpages of National Gastroenterology Society websites, the National Guideline Clearinghouse, the BMJ Clinical Evidence website,Google and Google Scholar was performed. RESULTS Fifteen guidelines were identified. Six guidelines were published in North America, four in Europe, four in Asia and one from the World Gastroenterology Organization. The majority of guidelines recommend screening average-risk individuals between ages 50 and 75 using colonoscopy(every 10 years), or flexible sigmoidoscopy(FS, every 5 years) or fecal occult blood test(FOBT, mainly the Fecal Immunochemical Test, annually or biennially). Disparities throughout the different guidelines are found relating to the use of colonoscopy, rank order between test, screening intervals and optimal age ranges for screening. CONCLUSION Average risk individuals between 50 and 75 years should undergo CRC screening. Recommendations for optimal surveillance intervals, preferred tests/test cascade as well as the optimal timing when to start and stop screening differ regionally and should be considered for clinical decision making. Furthermore, local resource availability and patient preferences are important to increase CRC screening uptake, as any screening is better than none.

Comparing outcomes for endoscopic submucosal dissection between Eastern and Western countries:A systematic review and meta-analysis

AIM To compare endoscopic submucosal dissection(ESD) outcomes between Eastern and Western *** A systematic review and meta-analysis was performed using Pub Med,MEDLINE,Web of Science,CINAHL and EBM reviews to identify studies published between 1990 and February 2016. The primary outcome was the efficacy of ESD based on information about either curative resection,en bloc or R0 resection rates. Secondary outcomes were complication rates,local recurrence rates and procedure times. RESULTS Overall,238 publications including 84318 patients and 89512 gastrointestinal lesions resected using ESD were identified. 90% of the identified studies reporting ESDon 87296 lesions were conducted in Eastern countries and 10% of the identified studies reporting ESD outcomes in 2216 lesions were from Western countries. Meta-analyses showed higher pooled percentage of curative,en bloc,and R0 resection in the Eastern studies; 82%(CI: 81%-84%),95%(CI: 94%-96%) and 89%(CI: 88%-91%) compared to Western studies; 71%(CI: 61%-81%),85%(CI: 81%-89%) and 74%(CI: 67%-81%) respectively. The percentage of perforation requiring surgery was significantly greater in the Western countries(0.53%; CI: 0.10-1.16) compared to Eastern countries(0.01%; CI: 0%-0.05%). ESD procedure times were longer in Western countries(110 min vs 77 min).CONCLUSION Eastern countries show better ESD outcomes compared to Western countries. Availability of local ESD expertise and regional outcomes should be considered for decision making to treat gastrointestinal lesions with ESD.

Recurrence rates after endoscopic resection of large colorectal polyps:A systematic review and meta-analysis

The last search was performed in October *** microbiome alterations exist among several body sites including oral, gut, and pancreatic tissue, in patients with pancreatic cancer compared to healthy *** study successes in non-invasive screening strategies warrant further investigation for future translational application in early diagnostics and to learn modifiable risk factors relevant to disease prevention. Pre-clinical investigations exist in other tumor types that suggest microbiome manipulation provides opportunity to favorably transform cancer response to existing treatment protocols and improve survival.

Pancreatic acinar cell carcinoma: A review on molecular profiling of patient tumors

Pancreatic carcinomas with acinar differentiation are rare,accounting for 1%-2% of adult pancreatic tumors; they include pancreatic acinar cell carcinoma(PACC),pancreatoblastoma,and carcinomas of mixed differentiation. Patients with PACC have a prognosis better than pancreatic ductal adenocarcinomas but worse than pancreatic neuroendocrine tumors. Reports of overall survival range from 18 to 47 mo. A literature review on PACCs included comprehensive genomic profiling and whole exome sequencing on a series of more than 70 patients as well as other diagnostic studies including immunohistochemistry. Surgical resection of PACC is the preferred treatment for localized and resectable tumors. The efficacy of adjuvant treatment is unclear. Metastatic PACCs are generally not curable and treated with systemic chemotherapy. They are moderately responsive to chemotherapy with different regimens showing various degrees of response in case reports/series. Most of these regimens were developed to treat patients with pancreatic ductal adenocarcinomas or colorectal adenocarcinomas. Review of PACC's molecular profiling showed a number of gene alterations such as: SMAD4,BRAF,BRCA2,TP53,RB1,MEN1,JAK-1,BRCA-1,BRCA-2,and DNA mismatch repair abnormalities. PACCs had multiple somatic mutations with some targetable with available drugs. Therefore,molecular profiling of PACC should be an option for patients with refractory PACC.

Biochar-induced changes in metal mobility and uptake by perennial plants in a ferralsol of Brazil’s Atlantic forest

Subspace clustering addresses an important problem in clustering multi-dimensional data. In sparse multi-dimensional data, many dimensions are irrelevant and obscure the cluster boundaries. Subspace clustering helps by mining the clusters present in only locally relevant subsets of dimensions. However, understanding the result of subspace clustering by analysts is not trivial. In addition to the grouping information, relevant sets of dimensions and overlaps between groups, both in terms of dimensions and records, need to be analyzed. We introduce a visual subspace cluster analysis system called ClustNails. It integrates several novel visualization techniques with various user interaction facilities to support navigating and interpreting the result of subspace clustering. We demonstrate the effectiveness of the proposed system by applying it to the analysis of real world data and comparing it with existing visual subspace cluster analysis systems.

Biochar colloids and their use in contaminants removal

In this study, we report on the extraction, characterization, and potential applications of colloidal biochar derived from pyrolyzed wood-an untapped source of carbonaceous particles.A series of characterizations was performed on biochar colloids to unravel their colloidal properties and surface chemistry through which it was found that they have a net negative charge and are stable between pH 3 and ***,our initial toxicity tests showed that biochar colloids themselves are not toxic and they can be used in remediation applications,which led us to investigate(1)their copper sorption,a model inorganic contaminant,in a scenario that biochar colloids are released into the environment and(2)their potential use in organic pollutants adsorption and *** sorption studies showed that biochar colloids have a copper sorption capacity as high as 22 mg g^(−1) in sub-ppm copper *** increased the acute 48 h lethal concentration(LC50)of copper for Daphnia magna by 21 ppb,which is comparable to the previously reported effect by dissolved organic *** and degradation of methylene blue(MB),an often-used proxy for organic contaminants in water,were studied by coupling the biochar colloids to positively charged TiO_(2) nanoparticles and using it as a *** hybrid MB photodegradation efficiency was 21%higher than that of TiO_(2) nanoparticles *** of demethylation is proposed as the main degradation mechanism of MB,as confirmed by liquid chromatography-mass spectroscopy(LC/MS),and the positive impact of biochar colloids is ascribed to their abundant adsorption sites,which may facilitate MB adsorption and its photocatalytic degradation.
