Operative complications and economic outcomes of cholecystectomy for acute cholecystitis

BACKGROUND Recent management of acute cholecystitis favors same admission(SA)or emergent cholecystectomy based on overall shorter hospital stay and therefore cost *** adopted the practice of SA cholecystectomy for the treatment of acute cholecystitis at our tertiary care center and wanted to evaluate the economic benefit of this *** hypothesized that the existence of complications,particularly among patients with a higher degree of disease severity,during SA cholecystectomy could negate the cost *** To compare complication rates and hospital costs between SA vs delayed cholecystectomy among patients admitted emergently for acute *** Under an IRB-approved protocol,complications and charges for were obtained for SA,later after conservative management(Delayed),or elective cholecystectomies over an 8.5-year *** were identified using the acute care surgery registry and billing *** was retrieved via EMR,operative logs,and Revenue Cycle *** severity of acute cholecystitis was graded according to the Tokyo ***18 categorizes acute cholecystitis by Grades 1,2,and 3 representing mild,moderate,and severe,*** were analyzed withχ2,Fisher’s exact test,ANOVA,ttests,and logistic regression;significance was set at ppatients(12.3%)received Delayed *** rates were increased after SA compared to Delayed cholecystectomy(18.5%vs 4.4%,p=0.004).The complication rates of patients undergoing delayed cholecystectomy was similar to the rate for elective cholecystectomy(7.4%,p=0.35).Mortality rates were 0.6%vs 0%for SA vs *** with moderate disease(Tokyo 2)suffered more complications among SA while none who were delayed experienced a complication(16.1%vs 0.0%,ppital charges for SA cholecystectomy were increased compared to a Delayed approach($44500±$59000 vs$35300±$16700,p=

pervaporation is an important membrane separation method of chemical *** this work,silver-nanoparticles-poly(vinyl alcohol)nanocomposite membranes(AgNps-pVA)are produced for the sake of improving its potentials for pervaporation of ethanol–water mixture so that the usual opposite trend between membrane selectivity and permeation can be *** nanocomposite membranes are fabricated from an aqueous solution of poly(vinyl alcohol)with silver nanoparticles via the in-situ generation technique in the absence of any reducing *** generation of the nano size silver is measured by the UV–vis spectrum showing a single peak at 419 nm due to the plasmonic effect of silver *** nanocomposite AgNps-pVA membranes are characterized using scanning electron microscope(SEM),Fourier-transform infrared(FT-IR)spectroscopy,X-ray diffraction and thermogravimetric analysis(TGA).The pervaporation tests of our new AgNps-pVA membranes show good results since at a higher temperature and higher ethanol concentration in the feed,the prepared membranes are highly permeable for the water having stable selectivity values and therefore our membranes show better performance compared to that of the other pVA-based nanocomposite membranes.

Estimation of damage in plants is a key issue for crop ***,experts in the field manually assess the *** is a time-consuming task that can be automated thanks to the latest technology in computer vision(CV).The use of image-based systems and recently deep learning-based systems have provided good results in several agricultural *** image-based applications outperform expert evaluation in controlled environments,and now they are being progressively included in non-controlled field applications.A novel solution based on deep learning techniques in combination with image processingmethods is proposed to tackle the estimate of plant damage in the *** proposed solution is a two-stage *** a first stage,the single plants in the plots are detected by an object detection YOLO based *** a regression model is applied to estimate the damage of each individual *** solution has been developed and validated in oilseed rape plants to estimate the damage caused by flea *** crop detection model achieves a mean precision average of 91%with a mAp@0.50 of 0.99 and a mAp@0.95 of 0.91 for oilseed rape *** regression model to estimate up to 60%of damage degree in single plants achieves a MAE of 7.11,and R2 of 0.46 in comparison with manual evaluations done plant by plant by *** are deployed in a docker,and with a REST ApI communication protocol they can be inferred directly for images acquired in the field from a mobile device.

1 Introduction Ischemic heart disease is caused by atherosclerotic and/or thrombotic obstruction of coronary arteries. Clinical spec- trum of ischemic heart disease expands from asymptomatic atherosclerosis of coronary arteries to acute coronary syn- dromes （ACS） including unstable angina, acute myocardial infarction （non-ST elevation myocardial infarction and ST elevation myocardial infarction）. Stable ischemic heart dis- ease （SIHD） refers to patients with known or suspected SIHD who have no recent or acute changes in their symp- tomatic status, suggesting no active thrombotic process is underway.

A Revisit to the Swedish Wet Compaction Method—A Case Study of the Burvattnet Dam Reconstruction

The Swedish Wet compaction method allows soil compaction at higher water content than conventional Dry compaction methods and can be used to advantage when difficulties arise in keeping to a certain Dry compaction water content. Wet compaction was frequently applied for dam core soils of glacial till (moraine) up until late 1970s, and despite several advantages it is since no longer used in engineering practice. During the reconstruction of Burvattnet Main Dam in Sweden, the lack of dry core soil together with severe weather conditions made Dry compaction almost impossible. On the basis of laboratory compaction tests performed in compliance with the standard from the 1950s, and field compaction trials on site, this paper describes the steps taken to revisit the Wet compaction method, which made it possible to continue the filling works in keeping with the timeline of the project.

Design of an Evacuation Demand Forecasting Module for Hurricane planning Applications

This paper discusses the development and implementation of an evacuation demand forecasting module that was incorporated into a comprehensive decision support system for the planning and management of contraflow operations in the Gulf of Mexico. Contraflow implies the reversing of one direction of a highway in order to permit a substantially increased travel demand exiting away from an area impacted by a natural disaster or any other type of catastrophic event. Correctly estimating the evacuation demand originated from such a catastrophic event is critical to a successful contraflow implementation. One problem faced by transportation officials is the arranging of the different stages of this complex traffic procedure. Both the prompt deployment of resources and personnel as well as the duration of the actual contraflow affect the overall effectiveness, safety and cost of the evacuation event. During this project, researchers from the University of Alabama developed an integral decision support system for contraflow evacuation planning to assist the Alabama Department of Transportation Maintenance Bureau in the evaluation and planning of contraflow operations oriented to mitigate the evacuation burdens of a hurricane event. This paper focuses on the design of the demand forecasting module of such a decision support system.

Background:plants respond to changes in vapour pressure deficit(VpD)between the leaf and the atmosphere through changes in stomatal response,which can consequently affect transpiration,photosynthesis,and leaf-level water use *** projected warmer air temperatures,changes in rainfall distribution and altered VpD in future climates,it is important to understand the potential effect of VpD on leaf-level physiology of field-grown *** aim of this study was to assess the impact of altered VpD on leaf-level physiology of field-grown cotton to improve the current understanding of the plant-by-environment interaction,thereby contributing to validation and improvement of physiological and yield response *** VpD environments in the field were generated by planting cotton on three dates within the sowing window(early-season(S1)=5th October 2011;mid-season(S2)=9th November 2011;and late-season(S3)=30th November 2011).VpD was also modified by altering crop ***:VpDL accounted for the largest proportion of the explained variation in both stomatal conductance(32%∼39%)and photosynthetic(16%∼29%)responses of ***,smaller percentages of variation were attributed to other main factors such as the individual plant(plant),and accumulated temperature stress hours(ASH;a measure of plant water status over time)and interactive factors,including leaf vapour pressure deficit(VpDL)×plant and plant×ASH;however,a proportion of variation was *** addition,the Asat/E(instantaneous transpiration efficiency,ITE)model developed based on cotton grown in the glasshouse was applied to cotton grown in the *** found that the modelled Asat/E and field-measured Asat/E were very similar,suggesting that the mechanistic basis for ITE was similar in both ***:This study highlights the importance of accounting for VpD in climate change research,given that stomata are highly responsive to changes in *** experiment pr
