Effectiveness of a home-environmental intervention package and an early child development intervention on child health and development in high-altitude rural communities in the Peruvian Andes: a cluster-randomised controlled trial

Background: Unsafe drinking water,poor sanitation and hygiene,exposure to household air pollution and low cognitive and socio-emotional stimulation are risk factors affecting children in low-and middle-income *** implemented an integrated home-environmental intervention package(IHIP),comprising a kitchen sink,hygiene education and a certified improved biomass cookstove,and an early child development(ECD)programme to improve children´s health and developmental outcomes in the rural high-altitude Andes of ***: We conducted a one-year cluster-randomised controlled trial among 317 childrenmonths divided into 4 arms(IHIP+ECD,IHIP,ECD,and Control)and 40 clusters(10 clusters per arm).ECD status(socio-emotional,fine and gross motor,communication,cognitive skills,and an overall performance)measured with the Peruvian Infant Development Scale and the occurrence of self-reported child diarrhoea from caretakers were primary *** outcomes included the occurrence of acute respiratory infections and the presence of thermo-tolerant faecal bacteria in drinking *** trial was powered to compare each intervention against its control arm but it did not allow pairwise comparisons among the four *** analysis followed the intention-to-treat *** the statistical analysis,we employed generalised estimating equation models with robust standard errors and an independent correlation ***: We obtained ECD information from 101 children who received the ECD intervention(individually and combined with IHIP)and 102 *** who received the ECD intervention performed better in all the domains compared to *** found differences in the overall performance(64 vs.39%,odd ratio(OR):2.8;95%confidence interval(CI):1.6–4.9)and the cognitive domain(62 vs 46%,OR:1.9;95%CI:1.1–3.5).Data analysis of child morbidity included 154 children who received the IHIP intervention(individually and combined with ECD)and 156 ***

Comprehensive review of hepatocellular carcinoma with portal vein tumor thrombus:State of art and future perspectives

Background:Despite advances in the diagnosis of patients with hepatocellular carcinoma(HCC),70%-80%of patients are diagnosed with advanced stage *** vein tumor thrombus(PVTT)is among the most ominous signs of advanced stage disease and has been associated with poor survival if *** sources:A systematic search of mEDLINE(Pubmed),Embase,Cochrane Library and Database for Systematic Reviews(CDSR),Google Scholar,and National Institute for Health and Clinical Excellence(NICE)databases until December 2022 was conducted using free text and meSH terms:hepatocellular carcinoma,portal vein tumor thrombus,portal vein thrombosis,vascular invasion,liver and/or hepatic resection,liver transplantation,and systematic ***:Centers of surgical excellence have reported promising results related to the individualized surgical management of portal thrombus versus arterial chemoembolization or systemic *** elements to the individualized surgical management of HCC and portal thrombus include precise classification of the portal vein tumor thrombus,accurate identification of the subgroups of patients who may benefit from resection,as well as meticulous surgical *** review addressed five specific areas:(a)formation of PVTT;(b)classifications of PVTT;(c)controversies related to clinical guidelines;(d)surgical treatments versus non-surgical approaches;and(e)characterization of surgical techniques correlated with classifications of ***:Current evidence from Chinese and Japanese high-volume centers demonstrated that patients with HCC and associated PVTT can be managed with surgical resection with acceptable results.

Intraocular pressure before and after capsulorhexis using two viscoelastic substances and two surgical approaches in enucleated porcine eyes

AIm:To investigate the influence of ophthalmic viscoelastic devices(OVDs)and different surgical approaches on the intraocular pressure(IOP)before and after creation of the curvilinear circular capsulorhexis(CCC)as a measure for anterior chamber stability during this ***:Prospective experimental WetLab study carried out on enucleated porcine *** was measured before and after CCC with the iCare Rebound tonometer(iCare ic200;iCare Finland Oy,Vantaa,Finland).The OVDs used were a cohesive one[Z-Hyalin,Carl Zeiss meditec AG,Germany;hyaluronic acid(HA)]and a dispersive[Z-Celcoat,Carl Zeiss meditec AG,Germany;hydroxy propylmethylcellulosis(HPmC)].The CCC was created using Utrata forceps or 23 g microforceps in different combinations with the ***:Using the Utrata forceps the IOP dropped from 63.65±6.44 to 11.25±3.63 mm Hg during the *** use of different OVDs made no *** the 23 g microforceps the IOP dropped from 65.35±8.15 to 36.55±6.09 mm *** difference between IOP drop using either Utrata forceps or 23 g microforceps was highly significant regardless of the OVD ***:Using the sideport for the creation of the capsulorhexis leads to a lesser drop in IOP during this maneuver compared to the main incision in enucleated porcine *** use of different OVD has no significant influence on IOP drop.

Assessment of scale interactions associated with wake meandering using bispectral analysis methodologies

Large atmospheric boundary layer fluctuations and smaller turbine-scale vorticity dynamics are separately hypothesized to initiate the wind turbine wake meandering phenomenon,a coherent,dynamic,turbine-scale oscillation of the far *** interactions,the mechanism of energy transfers between scales,manifest as triples of wavenumbers or frequencies and can be characterized through bispectral *** bispectrum,which correlates the two frequencies to their sum,is calculated by two recently developed multi-dimensional modal decomposition methods:scale-specific energy transfer method and bispectral mode ***-eddy simulation of a utility-scale wind turbine in an atmospheric boundary layer with a broad range of large length-scales is used to acquire instantaneous velocity *** bispectrum from both methods identifies prominent upwind and wake meandering interactions that create a broad range of energy scales including the wake meandering *** coherent kinetic energy associated with the interactions shows strong correlation between upwind scales and wake meandering.

Fundamental error in tree-based machine learning model selection for reservoir characterisation

Over the past two decades,machine learning techniques have been extensively used in predicting reservoir *** this approach has significantly contributed to the industry,selecting an appropriate model is still challenging for most *** solely on statistical metrics to select the best model for a particular problem may not always be the most effective *** study encourages researchers to incorporate data visualization in their analysis and model selection *** evaluate the suitability of different models in predicting horizontal permeability in the Volve field,wireline logs were used to train Extra-Trees,Ridge,Bagging,and XGBoost *** Random Forest feature selection technique was applied to select the relevant logs as inputs for the *** on statistical metrics,the Extra-Trees model achieved the highest test accuracy of 0.996,RmSE of 19.54 mD,and mAE of 3.18 mD,with XGBoost coming in ***,when the results were visualised,it was discovered that the XGBoost model was more suitable for the problem being *** XGBoost model was a better predictor within the sandstone interval,while the Extra-Trees model was more appropriate in non-sandstone *** this study aims to predict permeability in the reservoir interval,the XGBoost model is the most *** contrasting results demonstrate the importance of incorporating data visualisation techniques as an evaluation *** the heterogeneity of the subsurface,relying solely on statistical metrics may not be sufficient to determine which model is best suited for a particular problem.
