Most patients with non-hypertensive diseases at a critical care resuscitation unit require arterial pressure monitoring: a prospective observational study

BACKGROUND:Blood pressure(BP)monitoring is essential for patient *** arterial BP(IABP)is more accurate than non-invasive BP(NIBP),although the clinical signifi cance of this diff erence is *** hypothesized that IABP would result in a change of management(COM)among patients with non-hypertensive diseases in the acute phase of ***:This prospective study included adults admitted to the Critical Care Resuscitation Unit(CCRU)with non-hypertensive disease from February 1,2019,to May 31,*** plans to maintain a mean arterial pressure>65 mmHg(1 mmHg=0.133 kPa)were recorded in real time for both NIBP and IABP measurements.A COM was defined as a discrepancy between IABP and NIBP that resulted in an increase/decrease or addition/discontinuation of a medication/*** and regression tree analysis identified significant variables associated with a COM and assigned relative variable importance(RVI)***:Among the 206 patients analyzed,a COM occurred in 94(45.6%[94/206])*** most common COM was an increase in current infusion dosages(40 patients,19.4%).Patients receiving norepinephrine at arterial cannulation were more likely to have a COM compared with those without(45[47.9%]vs.32[28.6%],P=0.004).Receiving norepinephrine(relative variable importance[RVI]100%)was the most signifi cant factor associated with a *** complications were identifi ed with IABP ***:A COM occurred in 94(45.6%)non-hypertensive patients in the *** vasopressors was the greatest factor associated with *** should consider IABP monitoring more often in non-hypertensive patients requiring norepinephrine in the acute resuscitation *** studies are necessary to confi rm the risk-to-benefi t ratios of IABP among these high-risk patients.

Lower-grade gliomas(including low-and intermediate-grade gliomas,World Health Organization grades II and III)are diffusely infiltrative neoplasms that arise most often in the cerebral hemispheres of adults and have traditionally been classified based on their presumed histogenesis as astrocytomas,oligodendrogliomas,or *** the histopathologic classification of lower-grade glioma has been the accepted standard for nearly a century,it suffers from high intra-and inter-observer variability and does not adequately predict clinical *** on integrated analysis of multiplatform genomic data from The Cancer Genome Atlas,lower-grade gliomas have been found to segregate into three cohesive,clinically relevant molecular *** classes were closely aligned with the status of isocitrate dehydrogenase(IDH)mutations,tumor protein 53 mutations and the co-deletion of chromosome arms 1 p and 19q,but were not closely aligned with histologic *** findings emphasize the potential for improved definition of clinically relevant disease subsets using integrated molecular approaches and highlight the importance of biomarkers for brain tumor classification.

Objectives:To detail the US multi-institutional experience with the Occlutech©(Occlutech International AB,Helsingborg,Sweden)atrial flow regulator(AFR)in children and adults with acquired or congenital heart ***:The creation of a long-term atrial communication is desirable in several cardiovascular disease phenotypes,most notably pulmonary arterial hypertension,disorders of increased left ventricular filling and increased cavopulmonary pressures in patients with a Fontan type ***:Patients were identified for inclusion from the AFR device manufacturer *** was collected using a RedCap database following IRB approval.8 weeks of follow up data was sought for each patient based on available *** was analyzed and summarized using ***:We report the experience of 6 US centers in the implantation of AFR devices in 15 patients,across a wide age range,with different disease phenotypes and a variety of *** was technically successful in all patients and improvement was noted in both clinical and hemodynamic *** were no immediate or intermediate term complications reported.3 patients died remote from *** deaths were not felt to be related to the AFR device or related procedural ***:Compassionate use of the AFR device in children and adults with congenital&acquired heart disease is technically feasible and produces beneficial short term hemodynamic and symptomatic *** uptake of this technique and treatment at specialist centers has the potential to provide significant benefits to a variety of complex patients with currently limited treatment options and indeterminate prognosis.

The resistance of wear protective coatings against oxidation is crucial for their use at high ***,three nanocomposite AlCr(Si)N coatings with a fixed Al/Cr atomic ratio of 70/30 and a varying Si-content of 0 at.%,2.5 at.% and 5 at.% were analyzed by differential scanning calorimetry,thermogravimetric analysis and X-ray in order to understand the oxidation behavior depending on their ***,a partially oxidized AlCrSiN coating with 5 at.%Si on a sapphire substrate was studied across the coating thickness by depth-resolved cross-sectional X-ray nanodiffraction and scanning trans-mission electron microscopy to investigate the elemental composition,morphology,phases and residual stress evolution of the oxide scale and the non-oxidized coating *** results reveal enhanced oxidation properties of the AlCr(Si)N coatings with increasing Si-content,as demonstrated by a retarded onset of oxidation to higher temperatures from 1100℃ for AlCrN to 1260℃ for the Si-containing coatings and a simultaneous deceleration of the oxidation *** annealing of the AlCrSiN sample with5 at.%Si at an extraordinary high temperature of 1400℃ for 60 min in ambient air,three zones developed throughout the coating strongly differing in their composition and structure:(i)a dense oxide layer comprising an Al-rich and a Cr-rich zone formed at the very top,followed by(ii)a fine-grained transition zone with incomplete oxidation and(iii)a non-oxidized zone with a porous *** varying elemental composition of these zones is furthermore accompanied by micro-structural variations and a complex residual stress development revealed by cross-sectional X-ray *** results provide a deeper understanding of the oxidation behavior of AlCr(Si)N coatings depending on their Si-content and the associated elemental,microstructural and residual stress evolution during high-temperature oxidation.

Effect of calcination temperature on steam reforming activity of Ni-based pyrochlore catalysts

This work served as the second part of a study evaluating the effect of calcination temperature(700-1000℃) on Ni-based lanthanum zirconate pyrochlore catalysts for methane steam reforming.A previous study(Haynes et ***.2017(43) 16744) provided a thorough characterization of the material properties for the catalysts used here,and this study focuses on the evaluation of catalytic *** activity was assessed by two different experimental studies:the effect of reaction temperature using a temperature programmed surface reaction(TPSR),and the effect of reaction *** results demonstrate a complex interaction between the Ni particles and surface LaOx species under the methane steam reforming ***,the material calcined at the lowest temperature(700℃) possesses the highest activity and selectivity,which is attributed to smaller and more welldispersed Ni particles on the surface,and,more importantly,a lesser degree La enrichment at the *** catalysts were deactivated by steam to NiO under all conditions tested,but at certain low reaction pressure(p=0.23 MPa) conditions the materials calcined at 700-900℃ are able to completely recover equilibrium activity in-situ that is then robust and stable under both low and high reaction pressures(p=1.8 MPa) suggesting the formation of a synergistic relationship between Ni and La for syngas ***,exposure of a fresh material to high reaction pressures leads to a rapid and irreversible loss in both CH4 conversion and syngas selectivity whether in the fresh(no pretreatment),or pretreated(steam,H2 or Ar only at 800℃) form for any *** mechanism for deactivation appears to be due to the presence of LaOx species that become mobile,possibly by the formation of La-OH,and covers the active Ni particles and inhibits sites responsible for the CH4 decomposition.

To take an indirect route(detour)in order to reach a specific target requires complex cognitive *** more demanding,from the cognitive point of view,is when the goal is only visible at the beginning of the *** spiders from the family Sa lticidae,vision is a key sensory modal-ity mediating navigation and prey *** acute vision allows them to perform complicated detours,possibly as a consequence of the multitude of potential routes in their typically complex 3-dimensional *** used a 4-route choice test,in which routes differed in being either short or long and in the presence or absence of a lure of a prey item,to investigate route assessment in 2 salticid species,Trite planiceps and Marpissa *** both species showed evidence of motivation to follow lured-routes,judging by the number of times they re-oriented toward them while detouring,we found that Trite chose short routes in preference to long routes,but did not prefer the *** contrast,Marpissa exhibited random route choice,although it ori-ented toward lured-routes more often than control routes(lure absent).Our results suggest that decision-making processes about which route to take occurs before embarking on a route,but this is cognitively *** exhibited cognitive limitations in which the lack of visibility of the goal affected ***,the severity of cognitive limitations depended on *** suggest that variability in spatial ability across the Salticidae may be related to the habitat complexity inhabited by each species.

Patterns,procedures,and indications for pediatric surgery in a Tanzanian Refugee Camp:a 20-year experience

Background There are 103 million displaced people worldwide,41%of whom are *** on the provision of surgery in humanitarian settings are *** scarcer is literature on pediatric surgery performed in humanitarian settings,particularly protracted humanitarian *** We reviewed patterns,procedures,and indications for pediatric surgery among children in Nyarugusu Refugee Camp using a 20-year retrospective *** A total of 1221 pediatric surgical procedures were performed over the study *** between the ages of 12 and 17 years were the most common age group undergoing surgery(n=991;81%).A quarter of the procedures were performed on local Tanzanian children seeking care in the camp(n=301;25%).The most common procedures performed were cesarean sections(n=858;70%),herniorrhaphies(n=197;16%),and exploratory laparotomies(n=55;5%).Refugees were more likely to undergo exploratory laparotomy(n=47;5%)than Tanzanian children(n=7;2%;p=0.032).The most common indications for exploratory laparotomy were acute abdomen(n=24;44%),intestinal obstruction(n=10;18%),and peritonitis(n=9;16%).Conclusions There is a significant volume of basic pediatric general surgery performed in the Nyarugusu *** are used by both refugees and local *** hope this research will inspire further advocacy and research on pediatric surgical services in humanitarian settings worldwide and illuminate the need for including pediatric refugee surgery within the growing global surgery movement.

Decomposition research is still in its infancy,but significant advances have occurred within forensic anthropology and other disciplines in the past several *** research in forensic anthropology has primarily focused on estimating the postmortem interval(PMI),detecting clandestine remains,and interpreting the context of the ***,while much of the work has focused on forensic-related questions,an interdisciplinary focus on the ecology of decomposition has also advanced our *** purpose of this article is to highlight some of the fundamental shifts that have occurred to advance decomposition research,such as the role of primary extrinsic factors,the application of decomposition research to the detection of clandestine remains and the estimation of the PMI in forensic anthropology *** research in decomposition should focus on the collection of standardized data,the incorporation of ecological and evolutionary theory,more rigorous statistical analyses,examination of extended PMIs,greater emphasis on aquatic decomposition and interdisciplinary or transdisciplinary research,and the use of human cadavers to get forensically reliable data.
