Colorectal cancer metastases to the thyroid gland:A case report

BACKGROUND secondary malignancies of the thyroid gland are a rare finding in clinical practice.In addition,colorectal metastasis to the thyroid(CMT)is even more infrequently diagnosed.The source of the primary tumor follows demographic and ethnic patterns,which reflects the most prevalent malignancies in the different populations.Colorectal cancer is one of the most common types of cancer worldwide;nevertheless,CMT is infrequently diagnosed.Most of them are identified during the follow-up of gastrointestinal primary malignancies.Due to the improvement of image techniques,oncological treatment,and follow-up,survival and consequent diagnosis of metastatic disease are more frequent.Those facts make this entity a diagnostic and therapeutic challenge,due to the lack of information and the difficulties performing clinical trials and research.CAsE sUMMARY Here,we present a case report of a patient diagnosed with CMT of adenocarcinoma of the rectum evidenced during follow-up,4 years after neoadjuvant chemoradiotherapy,who had subsequent curative surgical treatment of the primary tumor and inter-current lung bilateral metastases.CONCLUsION Thyroid metastases of extra-thyroid origin are an uncommon finding,even rarer in cases of CMT.The diagnostic process,as well as survival of oncologic patients is improving,and consequently the number of metastases to the thyroid gland is increasing.

Assessing the energy saving potential of using adaptive setpoint temperatures:The case study of a regional adaptive comfort model for Brazil in both the present and the future

It has been found in recent years that using setpoint temperatures based on adaptive thermal comfort models is a successful method of energy conservation.Recent studies using adaptive setpoint temperatures incorporate international models from AsHRAE standard 55 and EN16798-1.This study,however,has instead considered a regional Brazilian adaptive comfort model.This study investigates the energy demand arising from the use of a local Brazilian comfort model in order to assess the energy implications from the use of the worldwide AsHRAE standard 55 adaptive model and various fixed setpoint temperatures.All of Brazil’s climate zones,full air-conditioning,mixed-mode building operating modes,present-day climate change scenarios,and future scenarios—specifically Representative Concentration Pathways(RCP)2.6,4.5,and 8.5 for the years 2050 and 2100—have all been taken into account in building energy simulations.The use of adaptive setpoint temperatures based on the Brazilian local model considering mixed-mode has been found to significantly reduce energy consumption when compared to static setpoint temperatures(average energy-saving values ranging from 52%to 58%)and the AsHRAE 55 adaptive model(average values ranging from 15%to 21%).Considering climate change and the mixed-mode Brazilian model,the overall energy demand for the three groups of climatic zones(annual average outdoor temperatures≤21℃,>21 and≤25℃and>25℃)ranged between 2%decrease and 5%increase,4%and 27%increase,and 13%and 45%increase,respectively.It is concluded as a consequence that setting setpoint temperatures based on the Brazilian local adaptive comfort model is a very efficient energy-saving method.

Teaching a robot to use electric tools with regrasp planning

This study presents a straightforward method to teach robots to use tools.Teaching robots is crucial in quickly deploying and reconfiguring robots in next-generation factories.Conventional methods require third-party systems like wearable devices or complicated vision system to capture,analyse,and map human grasps,motion,and tool poses to robots.These systems assume lots of experience from their users.Unlike the conventional methods,this study does not involve learning human motion and skills.Instead,it only learns the object goal poses from the human user whilst employs regrasp planning to generate robot motion.The method is most suitable for a robot to learn the usage of electric tools that can be operated by simply switching on and off.The proposed method is validated using a dual-arm robot with hand-mounted cameras and several tools.Experimental results show that the proposed method is robust,feasible,and simple to teach robots.It can find a collision-free and kino-dynamic feasible grasp sequences and motion trajectories when the goal pose is reachable.The method allows the robot to automatically choose placements or handover considering the surrounding environment as intermediate states to change the pose of the tool and use tools following human demonstrations.

Background:New fuid biomarkers for Alzheimer’s disease(AD)that reveal synaptic and neural network dysfunc‑tions are needed for clinical practice and therapeutic trial design.Dense core vesicle(DCV)cargos are promising cerebrospinal fuid(CsF)indicators of synaptic failure in AD patients.However,their value as biomarkers has not yet been determined.Methods:Immunoassays were performed to analyze the secretory proteins prohormone convertases PC1/3 and PC2,carboxypeptidase E(CPE),secretogranins sgIII and sgII,and Cystatin C in the cerebral cortex(n=45,provided by Bellvitge University Hospital)and CsF samples(n=66,provided by The sant Pau Initiative on Neurodegeneration cohort)from AD patients(n=56)and age-matched controls(n=55).Results:In AD tissues,most DCV proteins were aberrantly accumulated in dystrophic neurites and activated astro‑cytes,whereas PC1/3,PC2 and CPE were also specifcally accumulated in hippocampal granulovacuolar degeneration bodies.AD individuals displayed an overall decline of secretory proteins in the CsF.Interestingly,in AD patients,the CsF levels of prohormone convertases strongly correlated inversely with those of neurodegeneration markers and directly with cognitive impairment status.Conclusions:These results demonstrate marked alterations of neuronal-specifc prohormone convertases in CsF and cortical tissues of AD patients.The neuronal DCV cargos are biomarker candidates for synaptic dysfunction and neurodegeneration in AD.

Usefulness of Impella support in different clinical settings in cardiogenic shock

BACKGROUND The Impella pump has emerged as a promising tool in patients with cardiogenic shock(Cs).Despite its attractive properties,there are scarce data on the specific clinical setting and the potential role of Impella devices in Cs patients from routine clinical practice.METHODs This is an observational,retrospective,single center,cohort study.All consecutive patients with diagnosis of Cs and undergoing support with Impella 2.5?,Impella CP?or Impella 5.0?from April 2015 to December 2020 were included.Baseline characteristics,management and outcomes were assessed according to Cs severity,age and cause of Cs.Main outcome measured was in-hospital mortality.REsULTs A total of 50 patients were included(median age:59.3±10 years).The most common cause of Cs was acute coronary syndrome(ACs)(68%),followed by decompensation of previous cardiomyopathy(22%).A total of 13 patients(26%)had profound Cs.Most patients(54%)improved pulmonary congestion at 48 h after Impella support.A total of 19 patients(38%)presented significant bleeding.In-hospital mortality was 42%.Among patients with profound Cs(n=13),five patients were previously supported with venoarterial extracorporeal membrane oxygenation.A total of eight patients(61.5%)died during the admission,and no patient achieved ventricular recovery.Older patients(≥67 years,n=10)had more comorbidities and the highest mortality(70%).Among patients with ACs(n=34),35.3%of patients had profound Cs;and in most cases(52.9%),Impella support was performed as a bridge to recovery.In contrast,only one patient from the decompensated cardiomyopathy group(n=11)presented with profound Cs.In 90.9%of these cases,Impella support was used as a bridge to cardiac transplantation.There were no cases of death.CONCLUsIONs In this cohort of real-life Cs patients,Impella devices were used in different settings,with different clinical profiles and management.Despite a significant rate of complications,mortality was acceptable and lower than those observed in other series.

Mesenchymal stem cells(MsCs)are promising candidates for bone regeneration therapies due to their plasticity and easiness of sourcing.MsC-based treatments are generally considered a safe procedure,however,the long-term results obtained up to now are far from satisfactory.The main causes of these therapeutic limitations are inefficient homing,engraftment,and osteogenic differentiation.Many studies have proposed modifications to improve MsC engraftment and osteogenic differentiation of the transplanted cells.several strategies are aimed to improve cell resistance to the hostile microenvironment found in the recipient tissue and increase cell survival after transplantation.These strategies could range from a simple modification of the culture conditions,known as cell-preconditioning,to the genetic modification of the cells to avoid cellular senescence.Many efforts have also been done in order to enhance the osteogenic potential of the transplanted cells and induce bone formation,mainly by the use of bioactive or biomimetic scaffolds,although alternative approaches will also be discussed.This review aims to summarize several of the most recent approaches,providing an up-to-date view of the main developments in MsCbased regenerative techniques.

Differences in Judo Attack systems Between Weight Categories

Purpose In judo,athletes compete based on weight categories,which are required to adapt the training process accordingly,but not enough research is present to understand their particularities.The present study aimed to verify the differences in judo attack systems performed between the seven male weight categories in world-class athletes.Methods A cohort of 5847 attack systems from 1106 international judo matches was analyzed.The Chi-square auto-matic interaction detector was used to identify the association between the variables tori approach(attacker),uke approach(defender),tori’s grip,stance and direction of attack.Results The main results showed that:(1)the grip is the main predictor of attack systems;(2)laterality exerts a strong influence on attack systems;(3)the≤81 kg and>100 kg categories were differentiated by the use of unconventional grips;(4)the≤60 kg used more kenka-yotsu(opposite sides)right stance with the Only Right Hand grip;(5)the≤81 kg category used more the ai-yotsu(same side)left stance with the Only Left Hand grip.Conclusion The results reported from this study can help coaches and athletes in choosing the best technical-tactical actions when developing combat strategies.

An Ensemble Kalman Filter(EnKF)-based assimilation algorithm was implemented to estimate root-zone soil moisture(RZsM)using a soil-Vegetation-Atmosphere Transfer(sVAT)model during a complete growing season of corn in Central Mexico.synthetic and field soil moisture(sM)observations and NAsA sMAP sM retrievals were used to understand the effect of vertically spatial updates and uncertainties in meteorological forcings on RZsM estimates.Assimilation of RZsM every 3 days using sM observations at 4 depths lowered the averaged standard deviation(AsD)and the root mean square error(RMsE)by 60%and 50%,respectively,compared to the open-loop AsD.The assimilation of synthetic sM at the top 0-5 cm obtained RZsM closer to observations compared to THEXMEX-18 sM measurements and sMAP sM retrievals.Differences between EnKF estimates and sM observations and sMAP sM retrievals are mainly due to misrepresentation of vegetation conditions.The results improved sM estimates up to 10-cm depth using sMAP sM retrievals;however,additional studies are needed to improve sM at deeper layers.The implemented methodology can estimate sM at the top 10 cm of the soil every 3 days to mitigate the impact of the climate change on agricultural production over rainfed areas,particularly in developing countries.

Exploring seven hundred years of transhumance, climate dynamic, fire and human activity through a historical mountain pass in central spain

A high-altitude peat sequence from the heart of the spanish Central system(Gredos range) was analysed through a multi-proxy approach to determine the sensitivity of high-mountain habitats to climate, fire and land use changes during the last seven hundred years, providing valuable insight into our understanding of the vegetation history and environmental changes in a mountain pass close to a traditional route of transhumance. The pollen data indicate that the vegetation was dominated by shrublands and grasslands with scattered pines in high-mountain areas, while in the valleys cereals, chestnut and olive trees were cultivated. strong declines of high-mountain pines percentages are recorded at 1540, 1675, 1765, 1835 and 1925 cal AD, which may be related to increasing grazing activities and/or the occurrence of anthropogenic fires. The practice of mountain summer farming and transhumance deeply changed and redesigned the landscape of the high altitudes in central spain(Gredos range) since the Middle Ages, although its dynamics was influenced in some way by climate variability of the past seven centuries.
