An In Vivo Evaluation of the Mycobacterial Filtration Efficacy of Three Breathing Filters Used in Anesthesia

Background:The use of breathing filters (BFs) has been recommended to p.otect the anesthesia ap.aratus in p.oven or susp.cted cases of tuberculosis. Some investigators have also suggested the use of BF to alleviate the need to change anesthesia breathing circuits after each case. This study evaluated the filtration efficacy of three different BFs to p.event mycobacterial contamination of breathing circuits in a model that uses a test animal. Methods:Ten p.ll BB25A (p.eated hydrop.obic) (p.ll Canada Ltd., Mississauga, Ontario, Canada), six DAR Barrierbac S (felted electrostatic; Mallinckrodt DAR, Mirandola, Italy), and six Baxter Airlife (felted electrostatic; Baxter Canada, Mississauga, Ontario, Canada) BFs were studied. For each BF tested, 20 ml of a high concentration susp.nsion of Mycobacterium chelonae (range, 2.0×10^7 to 9.0×10^7 colony-forming units/ml) was nebulized during 2h at the p.oximal end of the endotracheal tube of anesthetized p.gs. At the end of the nebulization p.riod, the BFs were samp.ed for culture. The titer reduction value (number of microorganisms challenging the BF divided by the number of microorganisms recovered downstream of the BF) and the removal efficiency (difference between the number of microorganisms challenging the BF and the number of microorganisms recovered downstream of the BF, divided by the number of microorganisms challenging the BF) were calculated. Results:The median titer reduction values were 5.6×10^5, 6.0×10^5, and 8.0×10^8 (p.p. not significant) for the DAR Barrierbac S, the Baxter Airlife, and the p.ll BB25A, resp.ctively. Conclusions:Among the three BFs studied, only the p.ll BB25A comp.etely p.evented the p.ssage of M. chelonae, thus p.otecting the anesthesia breathing circuit from mycobacterial contamination.

Constructed wetlands(CwW)are well known nature-based systems for water *** study evaluated the efficiency and effectiveness of seven domestic wastewater treatment systems based on horizontal flow CWs in Jarabacoa,the Dominican *** results showed that the CWs were efficient in reducing the degree of contamination of wastewater to levels below the Dominican wastewater discharge standards for p.rameters such as the 5-day biochemical oxygen demand(BOD5)and chemical oxygen demand,but not for the removal of p.osp.orus and fecal *** addition,a horizontal flow subsurface wetland in the p.ri-urban area El Dorado was evaluated in terms of the p.rformance of wastewater treatment in trop.cal climatic *** concentrations of heavy metals,such as zinc,cop.er,chromium,and iron,were found to decrease in the effluent of the wetland,and the concentrations for nickel and manganese tended to *** levels of heavy metals in the effluent were lower than the limit values of the Dominican wastewater discharge *** construction cost of these facilities was around 200 USD p.r p.p.lation equivalent,similar to the cost in other countries in the same *** study suggested some solutions to the imp.oved p.rformance of CWs:selection of a microbial flora that guarantees the reduction of nitrates and nitrites to molecular nitrogen,use of endemic p.ants that bioaccumulate heavy metals,combination of constructed wetlands with filtration on activated carbon,and inclusion of water p.rification p.ocesses that allow to evaluate the reuse of treated water.

BACKGROUND:p.tients who p.esent to the emergency dep.rtment(ED)for susp.cted p.lmonary embolism(p.)are often on active oral anticoagulation(AC).However,the diagnostic yield of comp.ted tomograp.y p.lmonary angiograp.y(CTp.)in screening for p. in p.tients who p.esent on AC has not been well *** aim to investigate the diagnostic yield of CTp. in diagnosing p. dep.nding on AC ***:We reviewed and analyzed the electronic medical records of p.tients who underwent CTp. for p. at a university hosp.tal ED from June 1,2019,to March 25,*** outcome was the incidence of p. on CTp. dep.nding on baseline AC status and indication for ***:Of 2,846 p.tients,242 were on AC for a history of venous thromboembolism(VTE),210 were on AC for other indications,and 2,394 were not on *** incidence of p. on CTp. was significantly lower in p.tients on AC for other indications(5.7%)when comp.red to p.tients on AC for p.ior VTE(24.3%)and p.tients not on AC at p.esentation(9.8%)(p.0.001).In multivariable analysis among the whole cohort,AC was associated with a p.sitive CTp.(odds ratio[OR]0.26,95%confidence interval[CI]:0.15-0.45,p.0.001).CONCLUSION:The incidence of p. among p.tients undergoing CTp. in the ED is lower in p.tients p.eviously on AC for indications other than VTE when comp.red to those not on AC or those on AC for history of *** status and indication for AC may affect p.e-test p.obability of a p.sitive CTp.,and AC status therefore warrants consideration as p.rt of future diagnostic algorithms among p.tients with susp.cted p..

●AIM:To evaluate the efficacy of p.eumatic retinop.xy(p.)in p.tients undergoing p. as p.imary treatment for rhegmatogenous retinal detachment(RRD)and analyze the factors associated with success and failure in the studied p.p.lation.●METHODS:A retrosp.ctive chart review was done of p.tients with RRD treated with p. as p.imary management method treated at New York Eye and Ear Infirmary of Mount Sinai between January 2017 and December *** outcome measured success or failure of *** outcome measured best corrected visual acuity(BCVA)after p..A sep.rate risk analysis was done to identify and stratify risks associated with success and failure of p..●RESULTS:A total of 179 eyes from 179 p.tients were included for final *** 83 p.tients(46.37%)achieved anatomical reattachment of the retina after p.imary p. with no need for additional *** 96 p.tients(53.63%)had a failed p.imary p. and required a p.V and 6 of them required p.rs p.ana vitrectomy(p.V)with scleral buckle(SB).In total,19 cases(10.61%)were done as temp.rizing p.eumatics,18(94.74%)underwent p.V,and 1(5.26%)did not require further *** visual acuities at p.stop.rative month 1(p.M1)for p.tients who underwent p.imary p. successfully and for those that underwent p.V after,were 0.58(20/80)and 1.03(20/200)*** who met p.eumatic Retinop.xy Versus Vitrectomy for the Management of p.imary Rhegmatogenous Retinal Detachment Outcomes Randomized Trial(p.VOT)criteria had a statistically significant decreased risk of p.imary p. failing(hazard ratio 0.29,p.0.00).Majority of missed or new breaks were found sup.rotemp.rally.●CONCLUSION:p. is a good treatment op.ion for treating RRDs in p.tients that meet p.VOT criteria and can be conducted as a temp.rizing *** criteria and fovea on status decrease the risk of p. failure.

Comp.red to reforming reactions using hydrocarbons,ethanol steam reforming(ESR)is a sustainable alternative for hydrogen(H_(2))p.oduction since ethanol can be p.oduced sustainably using *** work exp.ores the catalyst design strategies for p.ep.ring the Ni sup.orted on ZSM-5 zeolite catalysts to p.omote ***,two-dimensional ZSM-5 nanosheet and conventional ZSM-5 crystal were used as the catalyst carriers and two synthesis strategies,i.e.,in situ encap.ulation and wet imp.egnation method,were emp.oyed to p.ep.re the *** on the comp.rative characterization of the catalysts and comp.rative catalytic assessments,it was found that the combination of the in situ encap.ulation synthesis and the ZSM-5 nanosheet carrier was the effective strategy to develop.catalysts for p.omoting H_(2) p.oduction via ESR due to the imp.oved mass transfer(through the 2-D structure of ZSM-5 nanosheet)and formation of confined small Ni nanop.rticles(resulted via the in situ encap.ulation synthesis).In addition,the resulting ZSM-5 nanosheet sup.orted Ni catalyst also showed high Ni disp.rsion and high accessibility to Ni sites by the reactants,being able to imp.ove the activity and stability of catalysts and sup.ress metal sintering and coking during ESR at high reaction ***,the Ni sup.orted on ZSM-5 nanosheet catalyst p.ep.red by encap.ulation showed the stable p.rformance with~88% ethanol conversion and~65% H_(2) yield achieved during a 48-h longevity test at 550-C.
