Achalasia is an uncommon primary motor disorder of the esophagus with an annual incidence of 0.8/100,000. Very few cases of coexistent Barrett’s esophagus (BE) and achalasia in patients without prior surgical myotomy or pneumatic dilation have been reported. We report the case of a 65 year old female who was diagnosed with achalasia in j.ne 2002. Endoscopy at that time revealed biopsy-confirmed normal esophageal mucosa. The patient subsequently underwent two trials of botox inj.ction with progressively worsening dysphagia. A repeat endoscopy two years later showed a short segment of salmon-colored mucosa in the distal esophagus which was biopsy-confirmed Barrett’s epithelium with no dysplasia. The patient eventually underwent laparoscopic Heller myotomy and Toupet fundoplication. Postoperatively, she recovered well and with significant alleviation of her dysphagia. This study reviews reported cases of coexistent achalasia and BE, and discusses possible etiologies of concurrent BE and achalasia, and implications for treatment.

Constructed wetlands(CwW)are well known nature-based systems for water *** study evaluated the efficiency and effectiveness of seven domestic wastewater treatment systems based on horizontal flow CWs in j.rabacoa,the Dominican *** results showed that the CWs were efficient in reducing the degree of contamination of wastewater to levels below the Dominican wastewater discharge standards for parameters such as the 5-day biochemical oxygen demand(BOD5)and chemical oxygen demand,but not for the removal of phosphorus and fecal *** addition,a horizontal flow subsurface wetland in the peri-urban area El Dorado was evaluated in terms of the performance of wastewater treatment in tropical climatic *** concentrations of heavy metals,such as zinc,copper,chromium,and iron,were found to decrease in the effluent of the wetland,and the concentrations for nickel and manganese tended to *** levels of heavy metals in the effluent were lower than the limit values of the Dominican wastewater discharge *** construction cost of these facilities was around 200 USD per population equivalent,similar to the cost in other countries in the same *** study suggested some solutions to the improved performance of CWs:selection of a microbial flora that guarantees the reduction of nitrates and nitrites to molecular nitrogen,use of endemic plants that bioaccumulate heavy metals,combination of constructed wetlands with filtration on activated carbon,and inclusion of water purification processes that allow to evaluate the reuse of treated water.

An Assessment of Saltwater Intrusion in Coastal Regions of Lagos, Nigeria

This paper explains various factors that contribute to saltwater intrusion, including overexploitation of freshwater resources and climate change as well as the different techniques essential for effective saltwater intrusion management. The impact of saltwater intrusion along coastal regions and its impact on the environment, hydrogeology and groundwater contamination. It suggests potential solutions to mitigate the impact of saltwater intrusion, including effective water management and techniques for managing SWI. The application of A.I (assessment index) serves as a guideline to correctly identify wells with SWI ranging from no intrusion, slight intrusion and strong intrusion. The challenges of saltwater intrusion in Lagos and the salinization of wells were investigated using the hydro-chemical parameters. The study identifies four wells (“AA”, “CMS”, “OBA” and “VIL”) as having high electric conductivities, indicating saline water intrusion, while other wells (“EBM”, “IKj. and “IKO”) with lower electric conductivities, indicate little or no salt-water intrusion, and “Aj. well shows slight intrusion. The elevation of the wells also played a vital role in the SWI across coastal regions of Lagos. The study recommends continuous monitoring of coastal wells to help sustain and reduce saline intrusion. The findings of the study are important for policymakers, researchers, and practitioners who are interested in addressing the challenges of saltwater intrusion along coastal regions. We assessed the SWI across the eight (8) wells using the Assessment Index to identify wells with SWI. Wells in “CMS” and “VIL” has strong intrusions. A proposed classification system based on specific ion ratios categorizes water quality from good (+) to highly (-) contaminated (refer to Table 4). These findings underscore the need for attention and effective management strategies to address groundwater unsuitability for various purposes.

After spinal cord inj.ry, there is an extensive infiltration of immune cells, which exacerbates the inj.ry and leads to further neural degeneration. Therefore, a maj.r aim of current research involves targeting the immune response as a treatment for spinal cord inj.ry. Although much research has been performed analyzing the complex inflammatory process following spinal cord inj.ry, there remain maj.r discrepancies within previous literature regarding the timeline of local cytokine regulation. The obj.ctives of this study were to establish an overview of the timeline of cytokine regulation for 2 weeks after spinal cord inj.ry, identify sexual dimorphisms in terms of cytokine levels, and determine local cytokines that significantly change based on the severity of spinal cord inj.ry. Rats were inflicted with either a mild contusion, moderate contusion, severe contusion, or complete transection, 7 mm of spinal cord centered on the inj.ry was harvested at varying times post-inj.ry, and tissue homogenates were analyzed with a Cytokine/Chemokine 27-Plex assay. Results demonstrated pro-inflammatory cytokines including tumor necrosis factor α, interleukin-1β, and interleukin-6 were all upregulated after spinal cord inj.ry, but returned to uninj.red levels within approximately 24 hours post-inj.ry, while chemokines including monocyte chemoattractant protein-1 remained upregulated for days post-inj.ry. In contrast, several anti-inflammatory cytokines and growth factors including interleukin-10 and vascular endothelial growth factor were downregulated by 7 days post-inj.ry. After spinal cord inj.ry, tissue inhibitor of metalloproteinase-1, which specifically affects astrocytes involved in glial scar development, increased more than all other cytokines tested, reaching 26.9-fold higher than uninj.red rats. After a mild inj.ry, 11 cytokines demonstrated sexual dimorphisms; however, after a severe contusion only leptin levels were different between female and male rats. In conclus
