While chronic hyperglycaemia resulting from poorly controlled diabetes mellitus(DM)is a well-known precursor to complications such as diabetic retinopathy,neuropathy(including autonomic neuropathy),and nephropathy,a paradoxical intensification of these complications can rarely occur with aggressive glycemic management resulting in a rapid reduction of glycated ***,acute onset or worsening of retinopathy and treatment induced neuropathy of diabetes are more common among these complications,rarely other problems such as albuminuria,diabetic kidney disease,Charcot’s neuroarthropathy,gastroparesis,and urinary bladder dysfunction are also *** World Journal of Diabetes recently published a rare case of all these complications,occurring in a young type 1 diabetic female intensely managed during pregnancy,as a case report by Huret et *** is essential to have a comprehensive understanding of the pathobiology,prevalence,predisposing factors,and management strategies for acute onset,or worsening of microvascular complications when rapid glycemic control is achieved,which serves to alleviate patient morbidity,enhance disease management compliance,and possibly to avoid medico-legal issues around this rare clinical *** editorial delves into the dynamics surrounding the acute exacerbation of microvascular complications in poorly controlled DM during rapid glycaemic control.

Despite the advent of relatively reliable modalities of diagnosing diabetic peripheral neuropathy(DPN),such as nerve conduction studies,there is still a knowledge gap about the pathophysiology,and thus limited available in-terventions for symptom control and curtailing disease *** pharma-cologic aspect of management is mainly centred on pain control,however,there are several important aspects of DPN such as loss of vibration sense,pressure sense,and proprioception which are associated with risks to lower limb health,which pharmacotherapy does not ***,published evidence suggests non-pharmacologic interventions such as glycaemic control through dietary modification and exercise need to be combined with other measures such as psychotherapy,to reach a desired,however modest *** is emerging as an important treatment modality for several chronic medical conditions including neuropathic and other pain *** their study published in the World Journal of Diabetes on the potential of acupuncture to reduce DPN symptoms and enhance nerve conduction parameters,Hoerder et al have been able to demonstrate that acupuncture improves sensory function and that this effect is likely sustained two months after treatment *** previous studies also support these findings,larger multi-center randomized control trials including a sham-controlled arm accounting for a placebo effect are ***,given the satisfactory safety profile and the positive results found in these studies,it is likely that acupuncture may become an important aspect of the repertoire of effective DPN management.

Human papillomavirus(HPV) is one of the most common sexually transmitted infections worldwide. Exposure to HPV is very common,and an estimated 65%-100% of sexually active adults are exposed to HPV in their lifetime. The majority of HPV infections are asymptomatic,but there is a 10% chance that individuals will develop a persistent infection and have an increased risk of developing a carcinoma. The International Agency for Research on Cancer has found that the following cancer sites have a strong causal relationship with HPV: cervix uteri,penis,vulva,vagina,anus and oropharynx,including the base of the tongue and the tonsils. However,studies of the aetiological role of HPV in colorectal and esophageal malignancies have conflicting results. The aim of this review was to organize recent evidence and issues about the association between HPV infection and gastrointestinal tumours with a focus on esophageal,colorectal and anal cancers. The ultimate goal was to highlight possible implications for prognosis and prevention.

Since chloroquine(CQ)and hydroxychloroquine(HCQ)can inhibit the invasion and proliferation of severe acute respiratory syndrome coronavirus 2(SARS-CoV-2)in cultured cells,the repurposing of these antimalarial drugs was considered a promising strategy for treatment and prevention of coronavirus disease(COVID-19).However,despite promising preliminary findings,many clinical trials showed neither significant therapeutic nor prophylactic benefits of CQ and HCQ against ***,we aim to answer the question of why these drugs are not effective against the disease by examining the cellular working mechanisms of CQ and HCQ in prevention of SARS-CoV-2 infections.

Gallbladder diseases are very common in developed countries. Complicated gallstone disease represents the most frequent of biliary disorders for which surgery is regularly advocated. As regards, cholecystectomy represents a common abdominal surgical intervention; it can be performed as either an elective intervention or emergency surgery, in the case of gangrene, perforation, peritonitis or sepsis. Nowadays, the laparoscopic approach is preferred over open laparotomy. Globally, numerous cholecystectomies are performed daily; however, little evidence exists regarding assessment of post-surgical quality of life (QOL) following these interventions. To assess post-cholecystectomy QOL, in fact, documentation of high quality care has been subject to extended discussions, and the use of patientreported outcome satisfaction for quality improvement has been advocated for several years. However, there has been little research published regarding QOL out-comes following cholecystectomy; in addition, much of the current literature lacks systematic data on patientcentered outcomes. Then, although several tools have been used to measure QOL after cholecystectomy, diffi culty remains in selecting meaningful parameters in order to obtain reproducible data to reflect postoperative QOL. The aim of this study was to review the impact of surgery for gallbladder diseases on QOL. This review includes Medline searches of current literature on QOL following cholecystectomy. Most studies demonstrated that symptomatic patients profi ted more from surgery than patients receiving an elective intervention. Thus, the gain in QOL depends on the general conditions before surgery, and patients without symptoms profi t less or may even have a reduction in QOL.

Effect of different pigmented cooked common beans on glucose and lipid metabolism in obese rats and 3T3 L1 cells

The effect of Azufrasin (yellow coat seed) and Flor de Junio (FJ) Dalia (pink-cream coat seed) cooked bean varieties was evaluated in high fat and fructose diet-fed rats to identify their health beneficial effects on obesity-related metabolic alterations. FJ Dalia and Azufrasin daily supplementation (10%) for six months decreased body weight (5-7%), serum triglycerides (28.5-36.4%), HOMA insulin resistance index (59.6-62.3%), and serum TNF-alpha (40.5-45.0%);moreover, FJ Dalia increased serum adiponectin (77.5%). Polyphenol-, phytosterol-, and saponin-rich extracts from Azufrasin and FJ Dalia were evaluated in 3T3 L1 cells. Both varieties up-regulated, Cpt1a and Acadm (fatty acid beta-oxidation), and Glut4, Irs1, and Pi3k (insulin signaling cascade) and down-regulated Pparg and Cebpa (adipogenesis) and Fasn and Acaca (lipogenesis). These activities were associated with their high content of soyasaponins A3, Af, and Be for adipogenesis and lipid metabolism, as well as erio-dictyol, ellagic acid, hesperidin and (+)-catechin and (-)-epicatechin and their derivatives, and beta-campesterol hexoside for insulin signaling cascade. FJ Dalia and Azufrasin cooked beans could be consumed to ameliorate obesity and its metabolic complications.

Ability of aqueous extract of Phoenix dactylifera to effectively alleviate paracetamol-induced hepatotoxicity in experimental Wister albino rats

Objective:To investigate the preventive,protective and ameliorative activity of the aqueous extract of Phoenix dactylifera L.(***)against paracetamol-induced ***:A total of 50 male albino rats were used for the study and 2 g/kg body weight of paracetamol and 400 mg/kg body weight of aqueous extract of *** were administered orally for the *** were divided into 5 groups,namely group A(vehicle control),group B(paracetamol control),group C(preventive),group D(ameliorative)and group E(protective),with 10 rats in each *** B was administered with paracetamol for 7 days;group C was administered with the extract for 7 days before administering with paracetamol for 7 days;group D was administered with paracetamol for 7 days,then the extract for 7 days;while group E was administered with paracetamol and the extract simultaneously for ***:The study revealed that the extracts of date palms contained active chemical compounds such as anthocyanins,phenolics,sterols,carotenoids and *** levels of antioxidant enzymes activity such as superoxide dismutase,catalase,peroxidase were found to be reduced while malondialdehyde level was significantly increased in the paracetamol-treated *** trend was reversed in groups where the extract was administered,as the antioxidant enzymes level in the liver was ***:This study has shown that the aqueous extract of *** can mitigate the hepatotoxicity effect of paracetamol with a better ameliorating effect than protective or preventive.
