目的在重症监护病房(Intensive Care Unit,ICU),护士难免会遇到需要限制治疗努力(limiting therapeutic effort,LTE)的患者,尽管多数护十还缺乏为这些临终患者及其家庭提供支持的准备。本综述旨在描述ICU护士在LTE中需要具备的能力。方法对2010年—2023年发表的文献进行综述。系统检索了5个数据库:Medline、Wiley Online Library SciELO、ScienceDirect和Web of Science。使用关键评估项目工具西班牙语版(Critical Appraisal Skills Programme in Spanish,CASPe)作为评判性阅读工具。使用牛津循证医学中心(Center for Evidence-Based Medicine,CEBM)评估工具确定证据级别和推荐强度。结果共纳入25篇文献。ICU护士LTE护理的能力包括对患者的直接护理、以家庭为中心的护理以及护士在团队中的作用。该综述确定了3个类别的能力:作为患者、家庭与跨学科团队之间维护者的能力,在LTE方面的决策能力,在患者生命末期治疗和护理干预的能力。结论 ICU护十在LTE方面需要具备相应的能力,这对患者的生活质量、死亡尊严以及患者家庭应对悲伤的全面支持至关重要。

Different xylogenesis responses to atmospheric water demand contribute to species coexistence in a mixed pine–oak forest

Seasonal patterns of wood formation(xylogenesis)remain understudied in mixed pine-oak forests despite their contribution to tree coexistence through temporal niche *** was assessed in three pine species(Pinus cembroides,Pinus leiophylla,Pinus engelmannii)and one oak(Quercus grisea)coexisting in a semi-arid Mexican *** main xylogenesis phases(production of cambium cells,radial enlargement,cell-wall thickening and maturation)were related to climate data considering 5-15-day temporal *** pines,cambium activity maximized from mid-march to April as temperature and evaporation increased,whereas cell radial enlargement peaked from April to May and was constrained by high evaporation and low ***-wall thickening peaked from June to July and in August-September as maximum temperature and vapour pressure deficit(VPD)*** of earlywood and latewood tracheids occurred in May-June and June-July,enhanced by high minimum temperatures and VPD in *** and *** oak,cambial onset started in march,constrained by high minimum temperatures,and vessel radial enlargement and radial increment maximized in April as temperatures and evaporation increased,whereas early wood vessels matured from May to June as VPD ***,15-day wet conditions enhanced cell radial enlargement in *** and ***,whereas early-summer high 15-day temperature and VPD drove cell-wall thickening in *** night conditions and high evaporation rates during spring and summer enhanced *** earlier growth peak in oak and a higher responsiveness to spring-summer water demand in pines contributed to their coexistence.

Background: The protective role of (poly)phenols against metabolic disorders has been extensively studied in adults but not in ***: To assess associations of dietary (poly)phenols and their subclasses with cardiometabolic health parameters in adolescents. Methods: A cross-sectional study was conducted in 944 individuals aged 11 to 14 years enrolled in the SI! Program for Secondary Schools trial (NCT03504059). (Poly)phenol intake was assessed using semi-quantitative food frequency questionnaires and the Phenol-Explorer database. The measured cardiometabolic parameters were waist circumference (WC) age-sex z-score， blood pressure (BP) age-sex z-score， blood glucose (BG)， triglycerides (TG)， and high-density lipoprotein cholesterol (HDL-c). Multilevel mixed-effect linear regression models were applied to examine the association between (poly)phenol quintiles and cardiometabolic health ***: Compared to the lowest quintile， adolescents in the highest quintile of total (poly)phenol intake had lower WC z-scores (β=-0.09， 95%CI=-0.09;-0.08， P-for trend=0.002)， mean arterial pressure z-scores (β =-0.11， 95%CI=-0.12;-0.09， P-for trend=0.206)， and HDL-c (β=-1.52， 95%CI=-2.90;-0.14， P-for trend=0.108)， after multivariable adjustment. The WC z-scores (β=-0.11， 95%CI=-0.19;-0.03， P-for trend=0.013) and HDL-c (β=-2.16， 95%CI=-3.78;-0.53， P-for trend=0.046) were lower in the highest quintile of flavonoid intake compared to the lowest quintile. The highest quintile of phenolic acid intake was associated with a lower WC z-score (β=-0.14， 95%CI=-0.26;-0.02， P-for trend=0.011) and TG levels (β=-8.83， 95%CI=-12.37;-5.30， P-for trend <0.001)， and the highest quintile of stilbene intake with lower BG (β=-1.83， 95%CI= -3.103;-0.64， P-for trend <0.001) and TG (β=-3.22， 95%CI= -4.95;-1.49， P-for trend=0.034)， and with higher HDL-c (β=2.328， 95%CI= 1.40;3.17， P-for trend=0.466) compared to the lowest quintile. Functional traits: the pathways to riverine plant resistance in times of hydropeaking

Hydropeaking, which refers to rising or falling discharges caused by the turning on or off of hydro-turbines to generate electricity, is a topic of growing interest due to its impact on fluvial ecosystems. To date, most hydropeaking studies have focused on the impact of peak fluctuations on invertebrate and fish communities, but little attention has been paid to its impact on riverine plants and how functional traits may make them resistant to hydropeaking. We performed a review to assess how a set of 32 plant functional traits can be expressed in riverine plant species, and found evidence of how such expression affects their capacity to cope with common sources of hydropeaking disturbance(i.e., inundation, fast water drawdown, and rapid water fluctuations linked to up-ramping and downramping hydropeaking operations). The information here presented can simplify the evaluation of impacts on riverine plant communities worldwide, as well as to detect suitable species to successfully restore rivers affected by hydropower production.

Influence of free fatty acid content and degree of fat saturation on production performance,nutrient digestibility,and intestinal morphology of laying hens

This study was conducted to evaluate the effects of dietary free fatty acid(FFA)content and degree of fat saturation on production performance,lipid and calcium digestibility,and intestinal function of laying *** a 15-week period,a total of 144 laying hens(19 weeks old)were randomly assigned to 8 dietary treatments,which were obtained by gradually replacing crude soybean oil with soybean acid oil(AO),or crude palm oil with palm fatty acid distillate(FAD).Thus,there were 4 soybean and 4 palm diets with 6%added fat varying in their FFA percentage(10%,20%,30%,and 45%),following a 2×4 factorial *** treatment included 6 replicates with 3 birds per *** daily feed intake and final body weight were significantly higher in palm diets(P<0.001),while no differences were found in egg mass and feed conversion *** levels of FFA in soybean diets resulted in lower egg production and higher egg weight(linear,P<0.01).Regarding the degree of fat saturation,hens fed soybean diets presented higher digestibility of ether extract(EE),fatty acids,and calcium than palm diets(P<0.001).The dietary FFA percentage negatively affected the digestibility of EE and calcium(P<0.01),while having little effect on FA *** was a significant interaction in the AME;lower values were reported in soybean diets as the dietary FFA percentage increased(linear,P<0.01),whereas palm diets remained *** experimental diets had little effect on gastrointestinal weight and ***,the jejunum of soybean diets showed higher villus height and higher villus height-to-crypt depth ratio than palm diets(P<0.05),and the dietary FFA percentage increased the crypt depth and decreased the villus height-to-crypt depth ratio(linear,P<0.05).It was concluded that varying dietary FFA content did not affect fat utilization as much as the degree of saturation did,supporting the use of AO and FAD as alternative fat ingredients.

New transcriptomic insights in two RNAi wheat lines with the gliadins strongly down-regulated by two endosperm specific promoters

The gluten proteins of wheat grain are responsible for visco-elastic properties of flour,but they also trigger the immune-response of celiac *** this work,two low-gliadin RNA interference(RNAi)wheat lines that differ for the promoter driving the silencing(D-hordein andγ-gliadin promoters for D783 and D793 lines,respectively),were characterized at transcriptomic,and protein fraction levels in the *** of gliadins provides a readjustment in the grain protein fractions that also affects to the nongluten proteins(NGP),which were increased in both RNAi *** of wheat gluten by means of the R5 monoclonal antibody also showed a strong reduction in the content of gluten in both RNAi ***,fructans,an oligosaccharide linked with the development of non-celiac wheat sensitivity(NCWS)were also significantly decreased in RNAi *** down-regulation of gliadins fractions also impacts to other metabolic processes,particularly on carbohydrate metabolism,enzyme regulator activity and response to *** and transcription factors regulated by ABA were up-regulated,which could suggest the implication of this phytohormone on the stress response observed in the RNAi lines.

Clinical relevance of stem cells in lung cancer

Lung cancer is the major cause of cancer-related deaths worldwide,it has one of the lowest 5-year survival rate,mainly because it is diagnosed in the late stage of the *** cancer is classified into two groups,small cell lung cancer(SCLC)and non-SCLC(NSCLC).In turn,NSCLC is categorized into three distinct cell subtypes:Adenocarcinoma,squamous cell carcinoma,and large cell *** is the most common lung cancer,accounting for 85%of all lung *** for lung cancer is linked to the cell type and stage of the disease,involving chemotherapy,radiation therapy,and *** improvements in therapeutic treatments,lung cancer patients show high rates of recurrence,metastasis,and resistance to *** stem cells(SCs)are undifferentiated cells capable of self-renewal and proliferation,are resistant to chemotherapy and radiotherapy and,due to their properties,could be involved in the development and progression of lung *** presence of SCs in the lung tissue could be the reason why lung cancer is difficult to *** identification of lung cancer stem cells biomarkers is of interest for precision medicine using new therapeutic agents directed against these cell *** this review,we present the current knowledge on lung SCs and discuss their functional role in the initiation and progression of lung cancer,as well as their role in tumor resistance to chemotherapy.
